[Studies on acute surgical therapy for DeBakey III aortic dissection: especially on the CT images and the operative indication].

Nine patients with DeBakey type III aortic dissection (type III a; 6, type III b; 3) that underwent acute surgical therapy were studied with particular emphasis on the preoperative CT images and operative indication. It took 6 approximately 192 hours (mean 40.6 +/- 56.0) from onset of the disease to the operation. The reasons why operations were required in acute phase were prolonged pain; 2, fluid retensionin in thoracic cavity; 3, increasing fluid retension; 3, shock state; 1, anuria; 1.6 (75%) of 8 thoracic aortic dissection cases showed intra-thoracic fluid retension in chest roentogenogram or CT image. We performed prosthetic interposition of descending thoracic aorta in 8 patients and Y-graft interposition of abdominal aorta in a patient. During the operations, we found fluid retension in the thoracic and pericardial cavity in 83% (5 cases) of 6 non-ruptured cases. To diagnose ruptured aneurysm, CT image played very important role. But we could not discriminate between ruptured aneurysm and intrapleural serous fluid retension on the CT image. Puncture of intra-thoracic fluid is effective procedure to diagnose rupture of the aneurysms but it is impossible in some cases and traumatic tap may lead misdiagnosis. Therefore we think moderate or increasing fluid retension in the thoracic cavity should be added to the operative indication of acute DeBakey type III aortic dissection and that the early operation will make mortality rate lower.

[Surgical therapy of aortic regurgitation due to aortitis syndrome--effectiveness and extended indication of Bentall's operation].

Fourteen patients with aortic regurgitation due to aortitis syndrome that underwent surgical therapy were studied with particular emphasis on the long term merits of Bentall's operations over the conventional valve replacement. They were divided into two groups; Group A: 6 aortic valve replacements (AVRs) and Group B: 8 Bentall's operations. Post operative complications related to aortitis syndrome developed in 3 cases; a case of paravalvular leakage (Group A) and two cases of aneurysm formation at the coronary artery anastomotic sites (Group B). We speculated that these complications were due to abnormal healing and recurrent inflammatory process of the disease. Therefore we think that conventional valve replacement is not suited to cope with these problems. Since 1984, we have adopted the policy to perform Bentall's operations for all patients with aortitis requiring valve replacement. Bentall's operation required longer aortic cross-clamp time (212.1 +/- 53.9 min) than AVRs (73.0 +/- 25.0) and was more aggressive. However no patients with our current technique to reimplant the coronary artery button and distal graft to aorta anastomosis, which has been used since 1984, has developed pseudoaneurysm or valvular leakage. We conclude that our modified Bentall's operation is effective even for patients who have active aortitis syndrome or need redo operations.

Characterization of complex formation between lipopolysaccharide and lysozyme.

The binding of lysozyme (LZM) to bacterial lipopolysaccharide (LPS) inhibited the biological activities of LPS as well as the enzymic activity of LZM. The mode of binding has been characterized by using dansylated LZM and enzyme inhibition. The binding of LPS to LZM significantly increased the fluorescence intensity (Fl-intensity) of the danyl group and was found to be time-dependent; the complex was produced gradually and became stabilized within 20 min at 37 degrees, 10 min at 50 degrees, and 1 min at 70 degrees. The maximum level of binding was also dependent on the reaction temperature, and more complex was formed at higher temperatures. Complexation was strongly dependent on the salt concentration and was not observed at greater than 0.5M NaCl. From collected evidence of the Fl-intensities of various dansyl derivatives and amphiphiles, it is concluded that LZM interacts with LPS by multiple binding-modes, the first being strongly related to the enzyme inhibition, the second being close to the Fl-intensity, and the third being dependent on the inhibition of immunopharmacological activities. For the amphiphiles used in this study, sodium dodecyl sulfate (SDS), 3-[(3-cholamidopropyl)dimethylammonio]-1-propanesulfonate (CHAPS), 3-[(3-cholamidopropyl)dimethylammonio]-2-hydroxy-propanesulfonate (CHAPSO), decansulfonic acid, and cardiolipin have binding modes similar to that of LPS.

High-molecular-weight Ly-5 isoforms expressed on T cells: activation-dependent expression.

Interleukin 2 (IL-2)-dependent granular T lymphocyte (GTL) lines derived from murine spleen were shown to express a Ly-5 antigen with the so-called 'B220-epitope' recognized by 6B2 mAbs, which were normally exhibited on B-lineage cells. Immunoprecipitation analysis revealed that the Ly-5 isoform on GTL lines represented the highest-molecular-weight Ly-5 so far described (260 kd), distinct from the B220 isoform on a pre-B cell line (240 kd). The size difference between them was also evident after the endoglycosidase treatment (210 versus 190 kd), strongly suggesting that these Ly-5 isoforms had core proteins of distinct size. Sequential immunoprecipitation by 6B2 and 14.8 mAbs further indicated that the 260 kd Ly-5 on GTL lines predominantly expressed '6B2 epitope' while '14.8 epitope' dominated in the B220 (240 kd) on a pre-B cell line. Northern blot analysis of the Ly-5 transcripts using probes for the known alternative exons failed to show any evidence for mRNA of the 260 kd Ly-5 distinct from that of B220. Polymerase chain reaction analysis, however, suggested that the 260 kd isoform mRNA might be transcribed from a distinct promoter with a yet undefined exon(s). The 6B2+ Ly-5 isoform was hardly detected on normal splenic T cells, but was shown to be induced rapidly on the majority of T cells following IL-2 stimulation in vitro, indicating that this particular Ly-5 isoform behaved as an 'activation antigen' on T cells.(ABSTRACT TRUNCATED AT 250 WORDS)

Generation of functional murine macrophage lines employing a helper-free and replication-defective SV40-retrovirus: cytokine-dependent growth.

By using a helper-free and replication-defective recombinant retrovirus encoding the SV40 early antigens (MV40), we have established continuous macrophage (M phi) lines. All of the lines were nonproducer M phi's with differentiated M phi functions such as phagocytosis, cytotoxicity, and IL-1 and TNF production. To determine the effects of several cytokines on growth of mature M phi's, the responsiveness of these established M phi lines to various cytokines was investigated in methylcellulose culture. Their response patterns to several cytokines alone and in combination were different, implying that there might be mature M phi subpopulations with distinct growth profiles regulated by several cytokines. On the other hand, all of the lines efficiently yielded a number of colonies in response to interleukin-4 (IL-4) alone. Moreover, IL-4 cooperated with interleukin-3 (IL-3) to enhance colony formation of all the lines. A similarly synergistic effect was observed in combination of IL-4 and macrophage-colony stimulating factor (M-CSF) in almost all the lines. Similar results were obtained with colony formation of fresh thioglycolate-induced M phi's. These observations suggested that IL-4 was involved in growth of mature M phi's. Our present results suggest that the helper-free and replication-defective MV40 is of use to obtain continuous and functional cell lines from primary M phi's.

Distribution of the mitogenic protein, sclerogen, in extracts from Sclerotinia sclerotiorum IFO 9395 assessed by using immunochemical analysis.

Sclerogen is a mitogenic protein isolated from the buffer extracts (3S) from sclerotia and shows significant mitogenic activity after heat denaturation accompanied with conformational changes. Experiments have been carried out to assess the distribution of sclerogen in several extracts from sclerotia and mycelia of this fungus by using the anti-serum against sclerogen. The anti-serum against the native- and heat denatured-sclerogen (anti-sclerogen-M and -MB serum, respectively) were prepared from Japanese white rabbits. These anti-sera showed different cross-reactivity to sclerogen-M and -MB from the results of immunoblotting after normal-polyacrylamide gel electrophoresis (normal-PAGE), suggesting that sclerogen-MB became to have different epitope(s) after heat denaturation of sclerogen-M. The result of immunoblotting after sodium dodecyl sulfate (SDS)-PAGE suggested that these anti-sera also recognized the random coil site(s) where the conformational specificity was not concerned, and that sclerogen (like substance(s] existed not only in mild extracts (e.g. phosphate buffer) but also in fractions extracted from mycelia of S. sclerotiorum IFO 9395. Taken together with the results of the mitogenic activity of the representative extracts from sclerotia, at least two kinds of the mitogenic substances would exist in sclerotia; one would be extracted by mild conditions, and the other would be difficult to extract and require drastic conditions such as hot water extraction.

Preparation and biological activities of sulfated derivatives of (1----3)-beta-D-glucans.

Sulfated derivatives of (1----3)-beta-D-glucans with different degree of branching (DB) i.e., curdlan (linear, DB 0), grifolan (6-O-substituted by beta-D-glucose at every third residue of the main chain, DB 1/3), and SSG (6-O-substituted by beta-D-glucose at every second residue, DB 1/2) having DS value (degree of substitution) lower than 0.6 were prepared. Biological activities [clotting of plasma, alternative pathway of complement (APC), proliferative response of murine spleen cells (mitogenic activity), and activation of murine peritoneal macrophages in vitro] of these derivatives were examined. Clotting of plasma was inhibited by the derivatives having DS above 0.2 APC was inhibited by incubation with derivatives and was strongly dependent on substitution. Inhibition of APC was the most significant in sulfated SSG having DS 0.6 [SSG(3)]. Mitogenic activity was observed by most of the derivatives and the highest activity was shown by SSG(1) (DS 0.2). Macrophage was also activated but SSG(1) would lose the capability to recognize the receptor for (1----3)-beta-D-glucans. From these results, it appears that the biological activities of (1----3)-beta-D-glucans were significantly modulated by sulfation and the effect was dependent on both DS and DB.

Oxidative degradation of an antitumor (1-3)-beta-D-glucan, grifolan.

A (1-3)-beta-D-glucan, grifolan (GRN), recovered from the peritoneal cavity after 1 d from i.p. injection contained a significant amount of anionic metabolite(s) having lower Mr than the parent GRN. In parallel with this observation, GRN induced peritoneal exudate cells exhibiting a higher level of oxidative burst than the non-stimulated, resident peritoneal cells. Chemical oxidation of GRN by active oxygen species such as (a) ascorbic acid-CuSO4, (b) hydrogen peroxide, (c) hydrogen peroxide-CuSO4, or (d) hypochlorous acid also produced anionic as well as lower Mr degradation products. Under these experimental conditions the structural changes were remarkable and in the order of a less than b less than c less than d. The products formed under the conditions (a) and (b) retained significant antitumor activity but those of (c) and (d) lost the activity. However, oxidation product(s) of curdlan, an antitumor inactive (1-3)-beta-D-glucan having no branch, by condition (d) induced significant antitumor activity. These results suggested that oxidative degradation of (1-3)-beta-D-glucans produced some temporary active metabolites and then gradually changed to the inactive form. However, these active metabolites contribute less than the parent glucan on the whole activation mechanisms of the host by (1-3)-beta-D-glucans.

Isolation of mitogenic substance from sclerotia of Sclerotinia sclerotiorum IFO 9395 extracted with phosphate buffer.

The buffer extracts (3S) of sclerotia of Sclerotinia sclerotiorum IFO 9395 contained mitogenic substance(s) to murine splenocytes (Shinohara et al. Chem. Pharm. Bull., 38, 2219 (1990)). Although the native 3S was slightly mitogenic, heating of 3S induced significant mitogenic activity. To isolate the mitogen, we separated 3S by ion-exchange and gel filtration chromatographies. The isolated mitogen, named sclerogen, has a molecular mass of 32 kilodaltons (kDa) on sodium dodecyl sulfate-polyacrylamide gel electrophoresis (SDS-PAGE) and the isoelectric point (pI) of 5.9 by chromatofocusing. Sclerogen was significantly mitogenic in vitro against murine splenocytes after heat denaturation, and also showed the augmentation of the primary mixed lymphocyte reaction (MLR) in vitro. However, sclerogen did not show the activation of an alternative pathway of complement and hemagglutination activity. These results suggest that sclerogen is a unique mitogen which differs from lectins and shows mitogenicity after heat denaturation.

Studies on human laminin and laminin-collagen complexes.

Intact human laminin and laminin type IV collagen complexes were extracted from term placental membranes and their structures were examined by electroimmunoblot and by rotary shadowing electron microscopy. Rotary shadowing electron microscopy revealed a structure of human laminin which is essentially similar to the cruciform structure of the mouse tumor (EHS) laminin, but with some notable differences. The observed lengths of the short arms in the human laminin are different. One short arm has an average length of 34 nm and another short arm a length of 42 nm. In the mouse laminin all three short arms are of equal length. The average length of the long arm is 97 nm, which is longer than that of mouse tumor laminin (77 nm). The distal portion of the long arm has two clearly separated globular domains instead of the single one observed with tumor laminin. Human laminin was found in various states of aggregation including dimers, trimers and higher aggregates. Laminin molecules appeared to attach to a point in type IV collagen located 87 nm from C-terminus, or at 174 nm from C-terminus and one with less frequency, at 251 nm from C-terminus. A small number of molecules appeared to bind at the N-terminus of the collagen. These laminin-collagen interactions occurred via the distal globular domains of both the short and long arms of laminin. The data suggest that human laminin extracted from placenta is structurally different from that isolated from the mouse EHS tumor.

[Treatment of mediastinal infection after coronary artery bypass surgery by transposition of the greater omentum].

From January, 1986 to May, 1990 twenty one adult patients (men 16, women 5, age 64 +/- 7 years old) underwent transposition of the greater omentum to control mediastinal infection after coronary artery bypass surgery. Upon diagnosing mediastinitis, the mediastinum was drained open and irrigated with 0.5% povidone iodine-saline solution until the omental transposition. The interval between the diagnosis of mediastinitis and the omental transposition ranged from 0 to 171 (mean 19) days. Three quarters of the patients had the omentum transposed within 14 days. In nineteen of 21 patients (90%) the mediastinitis was effectively controlled. In the remaining two patients the infection could not be controlled and proceeded to succumb from multiple organ failure. There was no complication related to the omental transposition in itself. We conclude that transposition of the greater omentum is a safe and effective method for treating mediastinal infection after coronary artery bypass surgery.

[A case of solitary bronchial papilloma and a review of the literature].

A case of solitary bronchial papilloma which is one of the rarest of pulmonary benign tumors is reported. A 25-year-old male was admitted to our hospital with an abnormal chest shadow. Bronchoscopy revealed a tumor at the orifice of left B6. The biopsy specimen of the tumor was diagnosed as a papilloma. Left S6 sleeve segmentectomy was performed to preserve lung function. The tumor was like a cauliflower and 4 x 4 x 4 cm in size. The histological diagnosis was bronchial papilloma. Since there are some reported cases of solitary bronchial papilloma with carcinoma in situ, such cases must be treated surgically when possible.

[Bilateral diaphragmatic plication for an adult patient].

Bilateral phrenic nerve paralysis is a very rare complication in open-heart surgery. An 65-year-old woman sustained respiratory distress after coronary artery bypass grafting because of bilateral phrenic nerve paralysis. Bilateral diaphragmatic plication was performed on the 43rd postoperative day. She successfully weaned from ventilator support without difficulty a few days after plication. We believe that surgical plication of the diaphragm is a safe and effective technique for a distressed adult patient with paralyzed diaphragm.

[Coronary artery bypass surgery for chronic total occlusion of the left main coronary artery by means of intermittent antegrade cold blood cardioplegia].

From January, 1984 to May, 1990 eleven patients (men 9, women 2) underwent coronary artery bypass surgery for chronic total occlusion of the left main coronary artery by means of intermittent antegrade cold blood cardioplegia. The ages ranged from 33 to 74 (mean 56) years. The causes of the total occlusion of the left main coronary artery were atherosclerosis in 10 patients and aortitis syndrome in one. Four patients had history of a previous myocardial infarction. Preoperative selective coronary arteriography revealed well developed collateral vessels from the RCA to the LCA in all patients. One to five coronary arteries were bypassed. Myocardial protection was obtained in the usual fashion: antegrade intermittent cold blood cardioplegia with topical cardiac cooling. All patients were successfully weaned off from cardiopulmonary bypass without the need of IABP assist. No patient developed perioperative myocardial infarction. All grafts were patent postoperatively. Treadmill testing was negative in all patients. We believe that coronary artery bypass surgery for chronic total occlusion of the left main coronary artery can be performed safely with intermittent antegrade cold blood cardioplegia.

Reactivity of Limulus amoebocyte lysate towards (1----3)-beta-D-glucans.

The structure activity relationship for beta-D-glucans for the gelation of the amoebocyte lysates of the horseshoe crab (Limulus) has been investigated. beta-D-Glucans that had no (1----3) linkages induced little or no gelation. The (1----3)-beta-D-glucans curdlan (unbranched), grifolan (approximately 33% branched), schizophyllan (approximately 33% branched), lentinan (approximately 40% branched). SSG (approximately 50% branched), and OL-2 (approximately 66% branched) induced significant gelation. The optimum concentration for gelation was correlated with the content of branching. Single chain (rather than a triple helix) conformation and higher molecular weight were associated with higher reactivity.

Distribution of fluoride concentration in the rat's bone.

The present study was undertaken to determine the fluoride distribution profile in the rat femur. Male Wistar rats were divided into four groups and given water containing 0, 25, 50, and 100 ppm of fluoride, respectively, for 10 weeks. The fluoride distribution from the periosteum to the endosteum was determined in each specimen after sampling using the abrasive microsampling technique. In the outer circumferential lamellae, the concentration of fluoride was relatively high in the periosteal layer and then decreased gradually towards its interior. In the haversian and interstitial lamellae, it was lower and roughly constant through the tissue. In the inner circumferential lamellae, it rose again to reach the highest level towards the endosteal surface. Fluoride concentration throughout the cortex increased significantly with increasing intake of fluoride. These increases were especially marked at periosteal and endosteal surfaces.

Enhancement of clot formation of human plasma by beta-glucans.

Effects of (1----3)-beta-D-glucans on clot formation of human plasma were examined. Gel forming 6 branched (1----3)-beta-D-glucans, SSG, branched at every other main chain glucosyl unit, grifolan (GRN) and schizophyllan, branched at one third of main chain glucosyl unit, enhanced clot formation of human plasma. GRN derivatives also increased the contents of clot, but less effectively than GRN. This effect was dependent on dose, molecular weight (greater than 20K), side chain (branching), and ultrastructure (single chain conformer) of the beta-glucans. It may be important for the mechanism of (1----3)-beta-D-glucan induced enhancement of plasma clotting to activate factor XII, to bind with fibrinogen, and to increase the local concentration of the clotting system by steric exclusion.
