[Infective endocarditis in patients on program hemodyalisis].

Ten (1.8%) out of 558 patients with end-stage chronic renal failure on program hemodialysis developed infective endocarditis (IE). The average length of hemodialysis before IE was 17.0 +/- 14.5 months. The main cause of IE was infection of the site of vascular approaches. The diagnosis was difficult; Duke criteria modified by J. Li et al. (2000) allowed evidenced diagnosis of IE only in 30% of cases, while modified criteria offered by the authors did it in 50% of cases. The article discusses rational antibacterial therapy. Hospital lethality in IE is as high as 50%.

[A case of primary amyloidosis].

Diagnostics of primary (AL) amyloidosis is difficult enough; treatment of this disease in not less difficult or more adequate. Because of similarity of the pathogenesis of AL-amyloidosis and that of multiple myeloma, similar therapeutic regimens, directed towards depression of plasma cell dyscrasia, are used in both cases: administration of melphalan in various doses together with prednisolone, administration of vincristine, adriablastine and dexamethasone, as well as high-dose chemotherapy with melphalan and autologic stem cell transplantation. This therapeutic approach makes it possible to reach clinico-laboratory remission and prolong the life of patients with AL-amyloidosis. The article contains a case description of a patient with AL-amyloidosis, who underwent a successful high-dose melphalan therapy with subsequent autologic stem cell transplantation.

[Clinicomorphyological peculiarities of pneumonias complicating the course of oncohematological diseases].

The modern concept of pneumonia should not be considered to be a logically accomplished theory. The diagnostic aspects remain complicated; the reasons for an increasing frequency of complication and lethality are not quite clear. Hospital pneumonia is under careful study, which is connected with its severity, complications, high lethargy, and a significantly higher cost of treatment. The morphological peculiarities of pneumonia in myelodepressed oncohematological patients deserve to be studied. In this study, 73 postmortem records of patients who had died of hemoblastoses and had had pneumonia were analyzed. The study showed that the first place in the clinical manifestations of pneumonia in myelo-depressed patients with hematoblastomas belongs to respiratory failure and toxic infective syndrome, which makes difficulties for diagnostic search of the infective process localization. Chest radiograms do not always make it possible to verify an inflammatory process in the lung tissue. Lung tissue infiltration in patients with myelodepression is within the limits of anatomic structures, which is also explained by the absence of cell elements forming the inflammatory bank. This morphological study demonstrates the absence of classic histological picture of pneumonia in the form of neutrophilic infiltration in patients with myelodepression, which often leads to the diagnosis of pulmonary edema or hemorrhage. Matching of clinicoradiological picture to the data of histological studies of the autopsy samples suggests that pneumonia in patients with myelodepression is atypical and implies forming serohemorrhagic and/or fibrinogenous exudate in the alveols.

[Pneumonia in patients with terminal renal failure on programmed hemodialysis].

Of 183 patients with terminal renal failure (TRF) on programmed hemodialysis 12(6.5%) were diagnosed to have acute pneumonia. As a rule, the disease develops in hyperhydration and hypertension of the lesser circulation, more rarely concomitant heart disease is diagnosed which runs with general circulation insufficiency. This indicates that pneumonia in dialysis patients is secondary. Pulmonary congestion in pneumonia debut may mask infiltrative alterations. Therefore, in obscure cases it is desirable to combine x-ray examination with polyposition tomoscintigraphy of the lungs. The agent was verified in 33.3% cases. The sputum contained for the most part gram-positive bacteria. Antibacterial therapy was combined with measures against hyperhydration and hypertension. Mean duration of the treatment was 21.25 +/- 13.6 days.
