Psychotherapy with cancer patients.

Cancer is associated with significant psychosocial morbidity. Although psychodynamic psychotherapy is a valuable intervention for some cancer patients, this modality has been underutilized because psychotherapy with medically ill patients has been linked historically to work with patients who have psychosomatic illnesses. Psychotherapy with patients who have cancer has unique features, such as the prominence of illness-related issues during the initial phase of treatment, the mixture of supportive and interpretive therapy, focused goals, and special issues pertaining to the transference/countertransference field. Understanding of those special issues will improve psychiatrists' ability to use long-term psychotherapy as an effective psychosocial intervention for patients with cancer.

Phantom limb pain. A review.

Phantom limb pain, which affects a majority of amputees, must be distinguished from phantom limb sensation, a universal consequence of limb amputation. Although the characteristics and time course of phantom limb pain are well described in the literature, no single theoretical approach can fully account for the contradictory aspects of this condition, thus its underlying mechanisms remain unclear. Theories concerning the etiology of phantom limb pain categorized as peripheral, central and psychological have given rise to a myriad of treatment approaches.

Alexithymia in chronic pain patients.

This article compares the prevalence of alexithymia among chronic pain patients, their spouses, and psychoneurotic outpatients using the Beth Israel Questionnaire (BIQ) and the MMPI subscale for alexithymia. Both instruments showed the pain patients to be significantly more alexithymic than the psychoneurotic controls, but not more alexithymic than their normal spouses. Comparison of the two instruments showed that the MMPI subscale did not correlate highly with the BIQ. When demographic variables were assessed, age was found to be correlated with alexithymia. Theoretical and treatment implications of these findings are discussed.
