[Cordarone treatment of parasystole].

Cordarone was used in the treatment of 15 patients with various forms of parasystole. There were 9 cases with ventricular and 5 with atrial parasystole. Cordarone was prescribed according to the following schedule: 200 mg three times daily for the first 10 days and 200 mg twice daily in the next 20 days. A clear positive effect was produced in 14 patients. In 13 cases parasystoles disappeared on the first to tenth day of treatment. The number of parasystoles reduced considerably in one case. The drug had no positive effect on one patient.

[Body impedance as a biological parameter in clinical research].

The impedance of the thoracic and abdominal organs and the integral body impedance were studied in healthy individuals and in patients with disorders of water-electrolyte metabolism (the edema syndrome, postoperative dehydration). Analysis of the impedance values made it possible to reveal characteristic changes in ascitis, in pleural effusion, and after major surgical interventions on the abdominal organs. The dynamics of these values may serve as an objective criterion of the extent of correction of disorders of water-electrolyte balance during treatment.

[Classification of non-coronarogenic myocardial damage].

A working classification of noncoronarogenic damage to the cardiac muscle is suggested in which 4 general-pathology groups are considered: myocardial dystrophies, myocarditis, cardiomyopathy, and neoplasms. The main principles are presented for grouping diseases of the myocardium not associated with ischemic heart disease, hypertension or rheumatic fever. Certain problems of the clinical picture and differential diagnosis of dystrophic, inflammatory, and idiopathic damages to the heart muscle are discussed.

[Difficulties in diagnosis of infectious-allergic myocarditis].

The work is based on observations over 168 patients with infectious-allergic myocarditis. The principal clinical, instrumental-laboratory, and morphologic criteria of this disease are discussed. The contractile function of the myocardium and central hemodynamics were studied. Intravital puncture biopsy of the myocardium was employed for the first time in this disease. Clinico-morphologic variants of infectious-allergic myocarditis are distinguished.

[Determination of the individual changes in cardiovascular system reactivity in prescribing adrenergic blockaders].

The changes occurring in hemodynamics due to the effect of alpha-adreno-blocking agent pyrroxan and beta-blocking agent obzidan were studied in 20 patients with neurocirculatory dystonia and in 20 patients with stage IIB hypertensive disease. Forty healthy persons formed the control group. The results of the investigation allow for the assumption that a pharmacological test with a record of the transthoracic rheogram and the derivatives of the central pulse tracings help in the proper choice of the drug and its dosage with the state of hemodynamics and the inotropic myocardial reaction taken into account.

[Kallikrein-kinin system of the blood plasma in patients with infectious-allergic myocarditis].

The state of the blood plasma kallikrein-kinin system in infectious-allergic myocarditis was studied for the first time. Biological methods for the determination of kallikrein, kininogen, kininases, and free kinins were used. Changes in the activity of the kinin system were revealed in 92% of 38 individuals examined: activation in the first 2-4 months and exhaustion later (in 6-8 months). It was shown that the generally accepted clinico-laboratory criteria on myocarditis are not always informative. Determination of the components of the plasma kinin system is suggested as an additional diagnostic test. The use of kinin antagonists and inhibitors of the kinin system in the complex of drug therapy in infectious-allergic myocarditis is recommended.

[Transthoracic rheography in clinical studies of hemodynamics].

The possibilities of transthoracic rheographic examinations with the aid of conventional rheographs were studied with reference to the stroke volume and minute cardiac output. A high percentage of coinciding results was found in comparison with similar studies performed on a tetrapolar rheograph. On the basis of the examinations of 60 normal individuals normal values of the systolic and minute cardiac outputs were found, as well as those of several other important haemodynamic parameters that permit a most precise noninvasive measurement of the parameters characterizing the functional state of the cardiovascular system under various conditions.
