Prenatal diagnosis of genitourinary tract malformations.

Ninety cases of genitourinary tract anomalies diagnosed before birth are reviewed. Factors stressed are the relationship between the site of the anomaly and the time of the diagnosis. Low level urinary tract obstructions tend to be diagnosed early, high level lesions late. Overall, the diagnostic accuracy was 88% with urethral obstruction being most often accurately diagnosed (19 of 20 cases). The frequency with which associated anomalies are found is stressed as is the lack of correlation between amniotic fluid volume and high or mid level obstructions. Ovarian cysts are usually diagnosed late in gestation and are seldom associated with other anomalies.

Antenatal diagnosis and obstetric management of Dandy-Walker syndrome.

Real-time ultrasound equipment has the potential for investigating the fetal neural axis and allows the diagnosis of many anomalies arising from this area. Five cases of Dandy-Walker syndrome, one of the major causes of congenital hydrocephalus, were diagnosed antenatally. In all cases ultrasound allowed specific recognition of the defect in the cerebellar vermis. This defect connected the fourth ventricle to a posterior fossa cyst in four cases and to an occipital meningocele in one case. In three cases the diagnosis was made prior to viability, and the parents elected termination of pregnancy. In two cases recognized in the third trimester, neurosurgical care was provided soon after delivery.

Ultrasound monitoring of ovarian follicular development: a comparison real-time and static scanning techniques.

Follicular growth was monitored in 19 patients during 20 spontaneous and induced cycles by different examiners in a blind trial using a real-time linear array, a mechanical sector scanner, and a gray scale compound scanner. This was done to compare the efficacy of different ultrasound equipment in the study of ovarian cycle. The mean maximum follicular diameter was calculated with each equipment and the quality of follicular images was classified as poor, sufficient, and good as defined in the study. The overall best results in ovary and follicle visualization were achieved with the real-time sector scanner. Measurements of the same dominant follicle using realtime and compound scanners were highly correlated (linear array/compound scanner: r = 0.894, p less than 0.001; sector/compound scanner: r = 0.928, p less than 0.001). Following ovulation, some unexpected differences in the distribution of the postovulatory patterns were noted with each instrument. Except in difficult patients, real-time examination, preferably performed using a sector scanner, appears to be the optimal first ultrasonic approach for monitoring follicular growth.

Reproduction in Down syndrome.

A new case of pregnancy in a 29-year-old woman with trisomy 21 is described. She gave birth to a male infant, chromosomally and phenotypically normal, who died the day after delivery due to prematurity. Thirty pregnancies of 26 affected mothers, including the present woman, resulted in 10 children with Down syndrome, 18 children (1 set of twins) without Down syndrome, and 3 spontaneous abortions. Although rare, pregnancies in women with Down syndrome could become more frequent, increasing the importance of genetic, reproductive, and obstetric problems. These problems are reviewed with special reference to the occurrence of nonspecific abnormalities in chromosomally normal children, difficulties in labor and delivery, and frequency of prematurity and low birth weight.

Estimation of gestational age during the first trimester by real-time measurement of fetal crown-rump length and biparietal diameter.

A series of 237 patients in the first trimester of pregnancy were examined to derive normal growth curves for fetal crown-rump length (CRL) and biparietal diameter (BPD) using sector scanner real-time ultrasound equipment. Conventional B-scan CRL measurements were obtained from 50 of these patients: In 41 (82%), the values fell within a range +/- 5 mm of those of real-time. Another group of 97 patients between 7 and 13 wk of gestation was then examined in a blind trial to confirm the accuracy of real-time measurements in estimating gestational age. An estimate was made in 95% of cases to within 4.6 days with CRL and to within 5 days with BPD. A similar comparison was made with both of these dimensions using a comparative nomogram, giving a prediction within 3.9 days in 95% of cases. This method is now used routinely.

Successful treatment of essential thrombocythemia in a pregnancy with recombinant interferon-alpha 2a.

Essential thrombocythemia (ET) in pregnancy is associated with adverse perinatal outcome, which is likely due to thrombosis of the uteroplacental circulation. Obstetrical management is still controversial. Different therapeutic protocols have been adopted, including the use of aspirin, plateletpheresis, and interferon. We herein report a case of ET treated with interferon-2 alpha from 13 weeks gestation until term. Therapy was well tolerated, leading to a linear reduction of platelet and white blood cell count that rapidly returned within normal limits. A healthy, 3,020-g male infant was delivered at 40 weeks gestation. Albeit further experience is required, recombinant interferon-alpha 2a may play an important role in preventing complications in pregnant patients with ET.

Genetic amniocentesis in twin pregnancy.

Among 1041 pregnancies 13 twin gestations were detected by routine ultrasonography prior to genetic amniocentesis at the Department of Prenatal Physiopathology of the University of Bologna. Clear amniotic fluid from both sacs was obtained in 12 of 13 sets of twins. All 12 sets were cytogenetically normal with normal levels of alpha-fetoprotein. Only one patient spontaneously aborted liveborn immature twins 30 days after the procedure. The technique used to obtain samples of fluid is described in detail and the need for additional counselling prior to amniocentesis in twins is stressed.

Prenatal diagnosis of endocardial fibroelastosis.

In a case of fetal heart failure caused by endocardial fibroelastosis, prenatal echocardiography clearly demonstrated a thickened endocardium. We therefore suggest that an abnormal endocardium may be detected in utero by ultrasound, thus representing an important clue in the differential diagnosis of fetal nonimmune hydrops and in the evaluation of pregnancies at risk for endocardial fibroelastosis.

[The evaluation of neuropsychiatric lupus erythematosus by functional neuroimaging. Preliminary results].

OBJECTIVE: The aim of this study was to investigate the performance of functional neuro-imaging for describe neurological involvement in Lupus Erithematosus Systemicus. METHODS: 20 SLE patients were included into this study (18 females / 2 males). Median age was 40.5 years (range 16 -66 ys), 9 patients with a clear neurological involvement, 7 with aspecific neurological symptoms and 4 were asymptomatics, according to 1999 ACR Classification. All patients were underwent to conventional resonance imaging (RM-FLAIR), RM perfusion ( RM-PWI), RM diffusion (RM-DWI) and cerebral SPECT. The RM techniques was performed using a 1 Tesla "Signa-Horizon" Tomograph by General Electric: The data analysis was performed from two independent neuroradiologist and than trough coordinated evaluation after coregistration of acquired volumes. RESULTS: In 11/20 patients (55%) lesions were demonstrated in RM-FLAIR evaluation, more frequent in cases with focal symptoms than in diffuse. RM-PWI was positive in 50% of cases. SPECT analysis was altered in 85 % of patients. In all patients RM-DWI evaluation was negative. 5 of 29 lesional areas (3 patients) showed by SPECT analysis were positive in RM-PWI. None of them was positive in RM-FLAIR study. After coordinated evaluation of RM-FLAIR, SPECT and RM-PWI, 7 findings were considered false positive. 6 Of patients with negative RM-FLAIR were positive in SPECT and 3 in RM-PWI. Only 1 patient was positive in SPECT and RM-PWI. CONCLUSIONS: According to the literature, the RM-FLAIR is a very sensitive procedure to describe the lesional charge, especially in patients with focal symptoms. All lesions was considered as stable outcomes due to negativity of RM-DWI analysis. The SPECT is a sensitive technique to individuate cerebral areas of altered perfusion. The coregistration seems to be an helpful method to improve the explanation of uncertain cases. e the sections are prepared for the microscopic analysis of the various histomorphometric parameters.
