Tetanus antigen modulates the gene expression profile of aluminum phosphate adjuvant in spleen lymphocytes in vivo.

Adjuvants play an important role in stimulation of the immune response to antigens. Very little is known about the molecular mechanisms of this stimulation. Here we address this issue by studying gene expression profiles from spleen lymphocytes after in vivo immunization of mice with a clinically relevant vaccine, tetanus toxoid formulated with aluminum phosphate as adjuvant (TT(ADJ)), or the adjuvant alone (ADJ). The Th1/Th2 response to TT(ADJ) was obtained from a combination of up- and downstream markers to conventional cytokines, which were in good agreement with cytokine protein levels. A clustering algorithm revealed that ADJ elicited expression of 47 genes active in cytotoxic lymphocytes, inflammation, oncogenesis, stress, toxicity and cell cycle regulation. In TT(ADJ) these adjuvant-elicited genes were expressed at lower levels and a compensatory onset of protective and inhibitory genes was observed. We conclude that the antigen, to a larger extent than previously recognized, modulates the molecular mechanism of the aluminum phosphate adjuvant and that the identified genes may serve as predictive biomarkers in the development of new adjuvants and vaccines.

[Spinal manipulation--treatment associated with a high risk of complications].

The article consists in a review of injuries for which claims were submitted to one or another of three insurance companies during a two-year period, and which had arisen in conjunction with manipulation of the cervical spine (n = 21), thoracic spine (n = 6), lumbar spine (n = 13), or the sacro-iliac joint (n = 14). Cervical spine manipulation had caused injury of the vertebral artery with resulting paralysis in three cases, and cervical disc herniation in three cases. Lumbar spine manipulation had caused disc herniation in six patients, of whom three had severe persistent problems. Of the 14 patients who had undergone manipulation of the sacroiliac joint, six had disc herniation which was operated with varying outcome. One patient with a compression fracture of an osteoporotic L3 vertebra developed paraparesis.

[Patient insurance in preventive activities. Wealth of injury material demonstrates risks of complications].

The patient insurance system in Sweden has manifest advantages where the mapping out of complications to medical treatment is concerned. The insurance data base contains a wealth of information which can be used for preventive purposes--not only overall statistics, but also patients' records and expert opinions in damage cases. During the 20 years since patient insurance was introduced, cases of special fundamental interest have been published, injury statistics have reported to clinics and departments, and certain issues have undergone special scrutiny--e.g. neural damage during surgery, infections arising from cataract surgery, bile duct injuries, injuries at delivery and damage resulting from physiotherapy.

Total hip arthroplasty in patients 80 years of age and older.

One hundred fifty-seven consecutive patients (162 total hip arthroplasties) 80 years of age and older were observed for one year after total hip arthroplasty (THA). Clinical results and complications were recorded. The indication for surgery was degenerative joint disease (DJD) in one half of the patients and complications to proximal femoral fractures in the other half. The mean age was 83 years old in both groups. In 80% of the patients, no complications were recorded during the first year. Three patients died during the first three months. Two deep infections occurred (1.2%). The dislocation rate was 9.2% (15/162). There was a lower dislocation rate (4/84) in the DJD group compared to the fracture group (11/78). All nine recurrent dislocations occurred in the fracture group and were treated with either trochanteric osteotomy (five) or removal of the prosthesis (four). In the patients operated on with trochanteric osteotomy, no further dislocations occurred. The mean hospital stay was 13 days. After one year, 88% (112/127) of the patients who could be observed had good or excellent results. Total hip arthroplasty in the elderly is a reasonably safe method and yields good functional results. Dislocations, however, were common in patients operated on for complications from proximal femoral fractures, and the risk for recurrent dislocation was high (9/11). In these patients, trochanteric osteotomy is recommended.

Combination treatment for adhesive capsulitis of the shoulder.

Twenty-two patients (23 shoulders) with arthrographically verified adhesive capsulitis of the glenohumeral joint were treated by a combination of distention-arthrography, local anesthetics and steroids intraarticularly, and manipulation. The mean duration of the disease at the time of treatment was 14 months, and all patients suffered from disabling pain and stiffness. A rapid improvement was seen after treatment and at four to six weeks: 91% (21/23) of the patients had no or slight pain and 83% (19/23) of the patients had normal, or almost normal, range of motion. The treatment was well tolerated and no complications were recorded. The combination treatment for adhesive capsulitis of the shoulder is safe, yields immediate results, and is cost effective.
