[Diagnostic criteria and classification of unstable stenocardia and lesions in the coronary bed].

A total of 128 patients with unstable angina pectoris (UAP) have been examined. Personal data of clinical examinations and the literature data enabled the authors to elaborate diagnostic criteria of UAP. Classification of UAP with consideration of its history, the clinical course of the disease and changes in the coronary arteries found with coronarography is offered. Almost half of patients with UAP begin to feel it while in perfect health. In 1/3 of cases only therapeutic treatment made the symptoms regress. It is established that UAP occurs mainly due to the changes in the system of left coronary artery. Very often stenosis of the main branch of this artery is observed.

[Automated information system on ischemic heart disease].

An information system on the results of clinical examination of patients with chronic ischemic heart disease is described. The bulk of information contains data on 590 patients who had undergone coronarography. A standarized chart of block structure of the comprehensive examination of the patient was used to obtain full and homogeneous information. With this information system large volumes of data may be grouped and stored, available whenever necessary, data may be selected from the overall material, and groups of patients may be formed according to some predetermined sign or complex of signs.

[Smoking and coronary arteriosclerosis in ischemic heart disease (clinical coronary x-ray studies)].

A relationship was studied between the intensity of smoking and changes revealed in the coronary arteries by coronarography in 498 patients with ischemic heart disease. Marked atherosclerosis was found in 94% of "regular" smokers and in 69% of the persons who did not smoke or smoked only a little. A particularly clear connection between the intensity of smoking and affection of the coronary arteries was registered in patients with ischemic heart disease under the age of 40. All of them had grades III and IV stenosis of the coronary arteries. The results discussed confirm that smoking is a serious "risk factor" of the development of ischemic heart disease, especially in young individuals.

[Diagnosis and clinical course of occlusive lesions of the main trunk of the left coronary artery].

A total of 50 patients with the left coronary artery trunk stenosed by more than 50 per cent were examined. General clinical methods, ECG under physical efforts and selective coronarography were employed. According to the authors this amounts to 6.4% of all corronarographically investigated patients. A comparison of the results derived from these investigations furnished fairly reliable clinical signs suggesting the existence of the stenosed trunk prior to coronarography. The authors feel that a preliminary diagnosis of such a lesion dictates an urgent coronarography. A careful and reserved approach to the investigation and a number of modifications in the procedure used by the authors are factors substantially diminishing the risk of this investigation.

[Clinical characteristics of angina pectoris in relation to disease of the coronary arteries].

Clinical and coronary angiographic comparison conducted in 164 patients with typical angina pectoris demonstrated that the nature of the pains may serve as a criterion of the degree of stenosis and the number of diseased arteries. The development of typical anginal seizures indicates the presence of clear changes in the coronary arteries. A daily intake of over 10 pills of nitroglycerine is a sign of a narrowing of the diameter of the coronary arteries exceeding 75%. A growing number of daily taken pills of nitroglycerine indicates an increase in number of diseased coronaries. Pains occurring during or after meals, when lying down, awakening the patient at night are typical for coronary narrowings exceeding 75% of their diameter. Usually such pains are observed in patients with 2 and more cornary arteries affected by the disease. Rightward irradiation and especially strictly localized pains are often associated with the lesion of the right coronary artery.
