[Surgical treatment for respiratory organ fistulas].

The paper pools the experience accumulated since 1965 at the Russian Surgery Research Center, Russian Academy of Medical Science, in treating 185 patients with respiratory organ fistulas. Medical and surgical treatments were performed in 23 and 162 patients, respectively. Radical operations were made in 124 patients, palliative ones in 38. The radical operation is to disconnect organs, by suturing the fistulous opening. If there were irreversible changes in the organs, their resection was simultaneously made. Palliative operations were performed as an auxiliary intervention before or during a radical operation. A palliative operation as an independent intervention was made only in a patient's severe general condition or in purulent fistulous complications. The findings suggest that radical surgery is the method of choice in the treatment of patients with respiratory organ fistulas.

[The experimental validation of the advantages of combined emergency (fluoroquinolones) and specific (EV Nalr) prevention of plague versus their sequential use].

Mice immunization with reference vaccine at the early stage of plague infection provided animals survival and prolonged mean survival period up to 2-5 days. Ciprofloxacin, ofloxacin and pefloxacin prevents development of post vaccine immunity at white mice, immunized by reference vaccine strain EV. Nalidixic acid and norfloxacin effect on post vaccine immunity was lower. Use of immunogenic strain EV Nafr (resistant to nalidixic acid and fluoroquinolones) provided antiplague immunity formation at the background of fluoroquinolones prophylaxis. Ciprofloxacin, ofloxacin and pefloxacin used for plague prophylaxis at white mice infected with Yersinia pestis (about 1000 LD50) inhibited postinfective immunity development. Nalidixic acid and norfloxacin didn't demonstrate such effect. Urgent (fluoroquinolones) and specific (EV Nalr) combined prophylaxis was evaluated as more effective for a 5-day period and provided the development of antiplague immunity.

[Experimental resistance of Vibrio cholerae el tor to nalidixic acid and fluoroquinolones].

It was shown that sensitivity of Vibrio cholerae eltor P-5879 to tetracycline, levomycetin, furazolidone, trimethoprim/sulfamethoxazole, aminoglycosides, beta-lactams, rifampicin, quinolones in vitro correlated with drugs efficacy in the treatment of experimental cholera of albino mice. Mutants of V. cholerae eltor P-5879 Nalr resistant to nalidixic acid (MIC 160-200 mg/l) formed with frequency 10(-9)-110(-8) had no cross resistance to fluoroquinolones. But the efficacy of ofloxacin, lomefloxacin, norfloxacin against these mutants in vivo reduced, though it was not changed in vitro. Mutants of V. cholerae eltor P-5879 resistant to fluoroquinolones and selected after culturing in the presence of the drugs had cross resistance to all quinolones studied. Infection caused by Cpfr mutant could not be treated with nalidixic acid and fluoroquinolones, therapeutic efficacy of rifampicin and beta-lactams, also reduced though sensitivity in vitro was not changed. The results of investigation proves the necessity of quinolones use for cholerae treatment as it is recommended for other severe enteric infections.

[Primary tumors of the trachea: their diagnosis and surgical treatment].

The authors present the analysis of the results of examination and treatment of 156 patients (84 females and 72 males aged from 7 to 69 years) with primary tumors of the trachea. Morphological types of primary benign and malignant tumors of the trachea and peculiarities of their clinical features are described. Diagnosis was based on clinical symptoms, results of roentgenologic examination and tracheobronchoscopy with biopsy. Surgical methods of treatment were endoscopic and open operations on the trachea. Endoscopic methods are of great importance in removal of benign tumors of the trachea as well as a preliminary stage before elective operation in subcompensated and decompensated neoplastic stenosis for elimination of stridor respiratory insufficiency and for decrease of secondary inflammatory disturbances in respiratory airways. In cases of extended resections of the trachea a series of special modes was provided for elimination of tension during anastomosis formation. Follow-up results of resections of the trachea for benign tumors are quite favourable. In cases of malignancies they are significantly worse. Survival of patients depends on histological type of the tumor and on the variant of resection of the trachea.

Tracheoesophageal fistulas.

Tracheoesophageal fistula is an uncommon clinical problem, and can be either congenital or acquired in origin. In this report, we present our experience in the management of 41 patients with tracheoesophageal fistula (28 male, 13 female; age ranging from 8 to 69 years) who were seen during the period spanning 1968 to 1989 at the National Research Center of Surgery, Moscow. During this time frame fewer malignant and correspondingly more posttraumatic and postoperative fistulas were observed. The most common clinical findings were cough associated with eating, production of sputum mixed with food, and weight loss with profound weakness. In the vast majority of patients, diagnosis was made using radiologic contrast studies, with endoscopic assessment being occasionally necessary. We classify our surgical approaches as "radical" (the isolation and ablation of the communication), "conditionally radical" (implying creation of a neoesophagus or thoracoplasty with muscle flap obliteration of the fistula and associated chronic empyema cavity), or "palliative" (usually entailing gastrostomy alone). Of the 32 patients undergoing surgical treatment in this series, 21 underwent radical or conditionally radical procedures; there was no operative mortality, and long-term follow-up shows that 19 of the 20 long-term survivors report satisfactory, good, or excellent status. This report summarizes the indications, timing, technique, and results of the various surgical approaches, and also delineates measures for the prevention of postoperative tracheoesophageal fistula.

[Broncho-esophageal fistula].

The authors analyse experience in the examination and treatment of 69 patients with bronchoesophageal fistulas (33 males and 36 females aged from 13 to 66). The etiological factors, pathogenesis, and clinical symptomatology of bronchial fistulas are discussed. The modern methods for the diagnosis of this pathological condition are indicated, the main ones being radiological and endoscopic methods. The indications for radical and palliative operations in bronchoesophageal fistulas are determined and their techniques are described. The methods for the prevention of complications are discussed. The late-term results of nonoperative treatment and various types of operations were studied.

[Surgical treatment of tracheal bifurcation].

Ninety-eight patients underwent operation for stenosing diseases of the bifurcation of the trachea at the Department of Lung and Mediastinal Surgery of the National Research Center of Surgery (Moscow) from 1963 to 1990. There were 44 females and 54 males. Their ages ranged from 6 to 69, average age of 38.2 years. The indications for the operation were as follows: malignant tumours in 84, benign tumours in 7, post-tuberculosis+ cicatricial tracheobronchial stenosis in 6 patients, and branchiogenic cyst in one patient. Surgical interventions were carried out in 86 and only endoscopic operations in 12 patients. The variants of radical operations were: resection of the bifurcation of the trachea with removal of apart of or the whole lung (41); resection of the bifurcation of the trachea with preservation of both functioning lungs (17); resection of the bifurcation of the trachea with exclusion of the left lung from ventilation (17); decompression of the region of the bifurcation of the trachea (2). The immediate postoperative period was uneventful in 51 patients. Complications occurred in 47 patients, 30 of them died. Three-year survival was recorded in 53%, 5-year survival in 48%, and 10-year survival in 40.3% of cases.

[Immediate and late results of the surgical treatment of tumors of the thoracic portion of the trachea].

The immediate and late results of surgical procedures, performed in 36 patients for tumors of the thoracic trachea, are presented. The survival of the patients was studied with special reference to the type of the operation and tumor. Benign tumors of the trachea were found to give good late results. Among the patients with primary malignant tumors better results were gained in case of cylindromas.
