[Characteristics of a clinical course of acute mastoiditis].

The analysis of 138 case records of acute mastoiditis (AM) patients treated for the last 5 years has demonstrated that mastoiditis incidence grows, it shows the trend to arise shortly after development of acute otitis media, classic symptoms occur less frequently while such symptoms as pain in the ear, pulse reflex, hypoacusis, elevated ESR are apparent. Most of the patients (71.7%) have undergone resection of the mastoid process. The resultant large retroauricular wound slowly granulated.

[Clinical course of mastoiditis as reported by the specialized emergency care hospital].

In the last decade diverse clinical manifestations of mastoiditis have been reported. This paper describes specific features of the clinical development, diagnostic value of individual symptoms, and complications in 90 cases of mastoiditis treated in the otorhinolaryngological department of the Clinical Hospital for Emergency Medical Care, city of Sverdlovsk, during 1984-1988. The observations allow the conclusion that during acute otitis media the duration of mastoiditis development reduced and many classical symptoms of mastoiditis, e. g. protrusion of the posterior-superior wall of the external acoustic meatus, profuse purulent discharge from the ear, hyperemia, swelling of the behind-the-ear area, occurred less frequently. The incidence of such life-threatening complications as meningitis and meningoencephalitis increased noticeably. It means that the list of indications for hospitalization of patients with acute otitis media should be extended.

[Otogenic intracranial complications in an urban emergency service hospital].

The problem of otogenic intracranial complications is very important for medical practice. This paper presents data on the incidence, clinical development and outcome of intracranial complications of purulent otitis media. During 1979-1988 the ENT Department of the City First Aid Hospital treated 82 patients with otogenic intracranial complications, which developed often in the case of acute than chronic otitis media. The lethality rate caused by otogenic meningitis and meningoencephalitis was 0.95% in the patients with acute otitis media and 0.5% in the patients with chronic otitis media. In view of this, it is recommended to extend the list of indications for hospitalization of patients with acute otitis media. It is strongly recommended to admit to hospitals patients with acute otitis media caused by severe flu, pregnant women, patients with severe visceral pathologies, and old age patients.
