Bilateral congenital anterior diaphragmatic hernia: a case report.

The anteromedial diaphragmatic hernias of Morgagni and Larrey are the least common defects that occur in this organ. Simultaneous occurrence has been described but remains an absolute rarity. We herein present a case of this bilateral herniation with a unique presentation. The paper refers to the basic anatomic, diagnostic and therapeutic considerations for this entity.

Cystic adenomyoepithelioma of the breast.

Cystic adenomyoepithelioma of the breast is a rare tumor. We report a case of this entity in a 42-year-old woman who presented with a lump behind her left nipple. Clinical examination suggested the diagnosis of a cyst. Ultrasound showed a cyst with an intracystic mass. The lesion was biopsied and found to be a cystic adenomyoepithelioma.

Schwannoma in the inguinal canal masquerading an inguinal hernia.

Benign neoplasms originating from the neuronal sheath are usually found along the distribution of the cranial nerves and in the flexor surfaces of the upper and lower extremities. We herein present a case of a 65-year-old man with a schwannoma located in the inguinal canal, which presented as an irreducible hernia. This is the first reported case in the English literature of a schwannoma with such a presentation.

A peroperative test of the function of the sphincter of Oddi.

A simple peroperative flow test has been developed to help detect organic stenosis of the Ampulla of Vater. The influence of glucagon and propantheline bromide on the flow of saline through the Ampulla during operation in 79 patients was measured. By measuring the flow rate of saline before and after the administration of glucagon to patients the effective diameter of the sphincter of Oddi and its ability to relax could be deduced. Glucagon was found to be most effective in relaxing the Ampulla. For the test to be successful impacted stones must be excluded by cholangiography, but free lying stones do not affect the test. In 7 of the 62 patients receiving glucagon flow was slow and the sphincter failed to relax. Two of these patients had impacted stones and were treated by ampullary dilatation and the other 5 (who had no stones showing on X-rays) were treated by trans-duodenal sphincterotomy; 2 of these patients were found to have small non-obstructing stones impacted at the Ampulla.

Diagnosis of pancreatic cancer.

Fifty-five patients underwent thorough laboratory and clinical investigations for pancreatic cancer. The disease was diagnosed in 32 patients. Thirty of these plus three others who did not undergo the sequence of tests underwent exploratory surgery. Pancreatic cancer was found in all. The high diagnostic accuracy was mainly due to the secretin test. We used Dreiling's technic for this test, which we think is the most popular and accurate technic.
