Outcome at discharge for clients in a psychiatric day care program.

The effectiveness of a psychiatric day care program was assessed in terms of the outcome at discharge for 79 clients. One-third of the clients were employed, either full-time or part-time (in case of students, at school), during the postprogram period of three months or more. Another one-third left the program prematurely with or without exacerbation of psychotic symptoms. Clients having premature termination were significantly less adherent to outpatient clinic visits and/or psychotropic medication. Family's understanding of and cooperation in the program were significantly less favorable in clients with early termination. The possible strategies for reducing the dropouts were discussed.

Comparative immunohistochemical identification and relative distribution of immunocompetent cells in sections of frozen or formalin-fixed tissue from human periapical inflammatory lesions.

The presence of immunocompetent cells (B, Th/i and Ts/c cells and macrophages) and the ratios of these cell populations in periapical lesions (radicular granulomas, radicular cysts and apical scars) were demonstrated immunohistochemically using paraffin and cryo-sections. Thirty-four human periapical lesions were examined for the presence of immunocompetent cells by monoclonal antibodies and the biotin-avidin-horseradish peroxidase method. The T/B cell ratio of radicular cysts was significantly higher than that of both radicular granulomas and apical scars, and the average number of Th/i cells was greater than that of Ts/c cells in the radicular granulomas. However, no significant difference was found between radicular cyst and apical scar. The number of macrophages in radicular granulomas was significantly higher than that of the other lesions. These findings suggested that periapical lesions develop as a result of both humoral and cell-mediated immunological response and indicated that the ratios of immunocompetent cells are different in the different types of periapical lesion.

[Application of proton pump inhibitor to special conditions; Zollinger-Ellison syndrome].

Before the mid-1970s, the most reliable therapy for Zollinger-Ellison syndrome (Z-E syndrome) was total gastrectomy since complete excision of gastrinoma is difficult in many cases due to multifocus and/or metastases. With the advent of H2-blockers gastric acid secretion can be controlled at safe levels (less than 10 mEq/h), and patients with Zollinger-Ellison syndrome can be managed medically. The problem of therapy with H2-blockers however, is that large doses are usually required and that a marked tachyphylaxis is frequently noted during long-term treatment. Treatment with new antisecretory drug, proton pump inhibitor, has been shown to be highly effective with relatively high doses and without tachyphylaxis, although long-term experience is still limited.

[Study on adrenergic acid secretion using isolated parietal cells of guinea-pigs].

In order to investigate the effect of adrenergic nerve system on acid secretion at the cell-levels, acid secretion of isolated parietal cells stimulated by various adrenergic agonists was observed by measuring the accumulation of 14C-aminopyrine (A.P.). Both isoproterenol, beta receptor agonist, and salbutamol, beta-2 receptor selective agonist, increased A.P. accumulation, whereas dobutamine, beta-1 receptor selective agonist, showed no effect. And beta-2 receptor on parietal cells was detected by binding assay. These data may suggest that parietal cells have beta-2 receptors through which acid secretion will occur.

[A case of fibrous dysplasia (McCune Albright syndrome) associated with acromegaly].

We report a case of a 47-year-old woman with McCune-Albright syndrome associated with unusual growth-hormone and prolactin hypersecretion. Acromegaly was suspected on clinical examination, and she was referred to us. She had no history of precocious puberty or pathological fracture. She was 154cm tall, weighing 62kg with so-called acromegalic facies. There were two lumps, one of which was on her right lateral forehead. The other on the right lower extremity seemed to be due to a bone deformity. Some brown pigmented macules with irregular borders were present on her lips and oral mucosa. Endocrine examination revealed elevated basal levels of plasma GH (54ng/ml) and PRL (36.2ng/ml), which paradoxically increased after injection of TRH. Plasma levels of these hormones did not change after LHRH test, and plasma GH level increased after GHRH test. A skull X-ray film showed a double floor of the sella turcia and hyperostotic formation of the right sphenoid bone. MRI and CT showed a tumor shadow in the right sella turcica. Bone roentogenography of the right fibula and tibia showed a large centrally expanding lesion with a ground-glass or cystic appearance and a thin cortex. The left fibula showed a similar lesion. We recognized similar findings in other bones which also showed abnormal accumulation in a radionuclide bone scan with 99mTc. On the basis of physical, endocrinological and roentogenographical examination, we diagnosed her as displaying McCune-Albright syndrome. We operated on her by transsphenoidal surgery and confirmed a functioning chromophobe adenoma which was removed during surgery. She has been free of GH and PRL hypersecretion since then. The pathology of the sphenoidal sinus affected with fibrous bone was in accordance with the findings of fibrous dysplasia. McCune-Albright syndrome (MCAS) is known as a syndrome characterized by polyostotic fibrous dysplasia and cutaneous pigmentation and multiple endocrinopathy. Its etiology is unknown. This syndrome with acromegaly is rare, and although 26 cases have been described in literature, pathological examinations have been undertaken in only three cases. This case was very rare and valuable because, by operating on her by the transsphenoidal route in spite of the sphenoidal sinus being affected with fibrous bone, we were able to confirm a chromophobe adenoma secreting both GH and PRL. We recognized that acromegaly with MCAS was endocrinologically, morphologically and immunohistologically the same as acromegaly without MCAS. Therefore, we concluded that acromegaly with MCAS is much the same as acromegaly without MCAS.

Perceptual dysfunction in hemiplegia and automobile driving.

Sixteen post-stroke, hemiplegic patients, who requested that they be allowed to return to driving, were extensively evaluated by a physiatrist, psychologist, occupational therapist, and driver training specialist. After they completed a driver training program, the patients were divided into three groups based on their driving program success and follow-ups. Significant differences in the psychological test results were found between Group I (those who drove without any difficulty) and Groups II and III (those who drove limited distance or who failed driver training). Results suggest that patients with significant cognitive and perceptual problems who take training and limit their driving can be safe drivers when they return to driving.

Adsorption of anti-dsDNA antibodies by immobilized polyanionic compounds.

Antibodies to dsDNA are characteristically present in serum from patients with systemic lupus erythematosus (SLE), and have been shown to have the capacity to react with various molecules bearing repeating negative charges. After a number of polymeric or monomeric molecules with differently charged groups and hydrophobic molecules had been coupled covalently as ligands on cellulose gel, the adsorption capacities of the ligands for anti-dsDNA antibodies were evaluated. It was found that gels coupled with polyanionic dextran sulphate (DXS) and polyacrylic acid (PA) and monoanionic sulphanilic acid (SA) absorbed anti-dsDNA antibodies effectively. DXS gel also adsorbed antibodies to ssDNA and heparan sulphate, antigens with repeating negatively charged moieties, while no ligand was able to adsorb anti-nRNP antibodies. The finding that DXS gels adsorbed anti-dsDNA antibody in proportion to their charge density, and that the interaction between anti-dsDNA and DXS gel is broken readily by an increase in ionic strength, indicated that the binding is ionic in nature. Moreover, virtually all F(ab')2 anti-dsDNA became adsorbed onto the DXS gels, suggesting that the binding occurred via specific antigen-binding sites on the antibody molecule. Binding of these polyanion-binding autoantibodies with anionic sites in the glomerular basement membrane may therefore cause the tissue damage observed in SLE.

[Studies on the contractile mechanism of isolated gastric smooth muscle cells of guinea pig--investigation of the histamine receptor].

Investigation about the histamine receptors of gastric smooth muscle cells was done using isolated gastric smooth muscle cells of guinea pig. Isolated gastric smooth muscle cells were prepared from the stomach of guinea pig by percoll density gradient centrifugation after enzymatic digestion with collagenase and dyspase. Contraction of the cells was determined by image of phase-contrast microscopy and digitizer and expressed as the mean per cent of control in cell axis length. The concentration of intracellular Ca2+ ([Ca2+]i) was measured with the calcium-sensitive dye fura-2. Tested drugs were added to the incubation medium containing 2 mM CaCl2. Both contractile response and [Ca2+]i increased following histamine stimulation in a dose-dependent manner from 10(-7) to 10(-4) M, and those peaks were obtained in application of 10(-4) M pyridylethylamine, a H1 receptor agonist, made an increase in [Ca2+]i as much as 10(-4) M histamine. But 10(-4) M dymaprit, a H2 receptor agonist, made only small increase in [Ca2+]i, whose value was similar to that in application of only 2 mM CaCl2. 10(-4) M pyrilamine, a H1 receptor antagonist, inhibited the response to 10(-4) M histamine, and the value was similar to that in application of only 2 mM CaCl2. Cimetidine, a H2 receptor antagonist, did not inhibit the response to 10(-4) M histamine stimulation at all. From those results it is concluded that the action of histamine to the gastric smooth muscle cells of guinea pig may be mediated through H1 receptor, but not through H2 receptor.

Isolation of guinea pig gastric smooth muscle cells.

A method of isolating guinea pig gastric smooth muscle cells was established in order to investigate gastric contractile mechanisms at the cellular level. Strips of gastric muscle were digested enzymatically with collagenase and dispase, and the smooth muscle cells isolated on a percoll density gradient, with a purity of 80-90%. The muscle cells were over 90% viable and demonstrated a remarkable contractile response to carbachol stimulation in a dose-dependent manner, from 10(-7) M to 10(-4) M. Intracellular Ca2+ concentration following carbachol stimulation was monitored, since Ca2+ is essential to the contractile process, and it was found to increase almost in parallel with contraction. These results suggest that the measurement of intracellular Ca2+ concentration is useful for analyzing the contractile mechanism of isolated smooth muscle cells.

[A case of Hoffmann's syndrome with peripheral neuropathy and various pathological findings in muscle biopsy].

A 58-year-old hypothyroid man developed numbness in the extremities and muscular hypertrophy without pain. Needle electromyography was normal. Nerve conduction study revealed severe entrapment and impaired conduction velocity. Muscle biopsy demonstrated small group atrophy, scattered hypertrophic fibers, interstitial fibrosis and subsarcolemmal vaculoes. These vacuoles were stained dark with NADH-TR. On electron microscopy these vacuoles were filled with aggregated mitochondria and glycogen particles. These myopathological findings have been rarely described in Hoffmann's syndrome or Kocher-Debré-Semelaigne syndrome in the literature. Sural nerve biopsy revealed a loss of myelinated fibers in large diameter and relative increase in small diameter fibers. Teased fiber examination demonstrated segmental demyelination. The nerve biopsy findings indicate slowly progressive axonopathy. In Hoffmann's syndrome, associated peripheral neuropathy may play some role in the manifestation of clinical signs and symptoms including muscle weakness and some sensory disturbance.

Epidemiological observation of coxsackieviruses group B in sewage.

Different types of coxsackieviruses group B were isolated from the sewage consecutively annually from January 1982 to March 1987: type 3 in 1982, type 4 in 1983, type 2, 4 and 5 in 1984, type 3 in 1985, and type 4 in 1986 were found. These viruses caused large epidemics among children aged 1 to 4 and proximities. Herald waves for the epidemics were observed six times during this period, and the viruses were detected in a brief period of isolation. With respect to the periodicity of these viruses, the cycles for types 3 and 4 were 2 to 3 years, and longer cycles seemed to be exhibited by types 1, 2, and 5. The onset and termination of the epidemics will only be grasped by surveys throughout the year. HEp-2 cells exhibited the best sensitivity to the viruses.

Selective removal of anti-double-stranded DNA antibodies by immunoadsorption with dextran sulphate in a patient with systemic lupus erythematosus.

A dextran sulphate gel column removed anti-double-stranded DNA antibodies (anti-dsDNA) selectively, efficiently, and safely from the circulation of a patient with systemic lupus erythematosus (SLE). The mechanism of the removal is thought to be due to cross reactivity of anti-dsDNA with dextran sulphate, which has negatively charged units. Selective removal of anti-dsDNA is expected to contribute not only to the treatment but also to elucidation of the pathogenesis of SLE.

Effect of pirenzepine on aminopyrine uptake by isolated guinea pig parietal cells.

The inhibitory action of pirenzepine on acid secretion of isolated guinea pig parietal cells was investigated by the aminopyrine method. Pirenzepine markedly inhibited acid secretion of isolated parietal cells induced by carbachol in a dose-dependent manner but showed no inhibition on acid secretion stimulated by histamine. These results may suggest a direct action of pirenzepine on muscarinic receptors in parietal cells.

[Gastric acid secretion and cellular immunity in patients with gastric cancer].

Three hundred and one gastric cancer patients have been examined preoperatively to investigate their gastric acid secretions after stimulation by tetragastrin, and serum gastrin stimulation by a test meal, as well as for skin reactions and an evaluation of their serum glycoproteins. The results have indicated that their gastric secretions and serum gastrin response were found to be reduced, according to the advancement of their cancer, and that the gastric acid secretion of patients with signet ring cell carcinoma was higher than that of patients with other histological carcinomas. Gastric acid secretions of patients with an ulcerated type of cancer, that is, type IIc and type III in an early cancer stage and type IIc of an advanced Borrmann V type, was higher than in patients with other types, and there were significant correlationships between gastric secretions and PHA skin test and gastric secretions and the IAP and the sialic acid.

Adenocarcinoma, Mucinous

[Gastric acid secretions and serum gastrin levels in patients with mucosal and submucosal gastric cancers].

Gastric acid secretions and serum gastrin levels have been examined in 128 patients with early gastric cancer and in 98 gastric ulcer patients. Gastric cancer patients were found to have lower acid secretions than did gastric ulcer patients, and those with elevated types of a differentiated adenocarcinoma had lower acid secretions than did those with depressed types of an undifferentiated adenocarcinoma. Gastric acid secretions in patients with both a gastric ulcer and cancer were found to decrease with aging. However, the serum gastrin levels were found to be decreased in patients with a gastric ulcer and to be increased in patients with a gastric cancer. Incidences of a differentiated adenocarcinoma increased with aging. From these observations, it has been speculated that the carcinogenesis of a differentiated adenocarcinoma may be related to increasing endogenous gastrin levels and decreasing gastric acid secretions. These results suggest that a continuous check of the serum gastrin levels might be a good marker for cancer detection and that gastrin antibodies might be useful for treatment.
