Ventricular arrhythmias late after aortic and/or mitral valve replacement.

Since it is not yet known whether ventricular arrhythmias in patients with valve replacement are associated with an increased risk of sudden death, as in patients with coronary artery disease, a total of 46 patients who were long-term survivors of aortic and/or mitral valve replacement were examined with 24-hour ambulatory electrocardiographic monitoring, and the factors influencing the occurrence of ventricular arrhythmias were analyzed. The significance of ventricular arrhythmias in the prognosis of valve replacement is discussed. The occurrence of ventricular arrhythmias was significantly higher: 1) in patients with aortic stenosis than in those with aortic regurgitation, 2) in patients with multiple valve surgery than in those with single valve replacement and 3) in patients with larger heart size. Pre- and postoperative hemodynamics, including left ventricular function, were not significantly related to the incidence of ventricular arrhythmias. A review of the patients who died suddenly, late after valve replacement suggests that frequent ventricular arrhythmias and thromboembolism are the most important factors in the late mortality of these patients. The prognostic significance of postoperative ventricular arrhythmias in patients with valve replacement requires additional study.

[Long-term evaluation of valvuloplastic surgery in congenital heart diseases].

Valvuloplastic surgery was performed on the 89 cases of congenital aortic and mitral regurgitation between 1966 and 1983. There were 4 operative deaths and 4 late deaths. Reoperation was performed on the 5 aortic and the 6 mitral cases due to residual regurgitation. In the same period, all 4 cases died after valve replacement for the congenital aortic and mitral regurgitation. Long-term evaluation over 3 years was done for the 35 cases of aortic regurgitation with ventricular septal defect (AR + VSD) and the 38 cases of congenital mitral regurgitation except endocardial cushion defect, who were treated by valvuloplastic surgery at the initial operation. Statistical studies on the cases with AR + VSD showed 100% actuarial survival rate, 83% reoperation free rate and 91% valve replacement free rate after 17 years. Of the cases with congenital mitral regurgitation, 95% survival rate and 78% reoperation free rate after 16 years were revealed. In conclusion, the author believes that the operative results and the long-term stability of valvuloplastic surgery for regurgitant valvular lesions associated with congenital heart diseases can be more improved by keeping the precise indication and using the appropriate valvuloplastic procedures for the individual cases.

Estimate of the calculation of the cardiac flux by the intra-aortic diastolic counterimpulse in cardiac patients and cardiac ischaemic dogs.

During the treatment of cardiacs with the diastolic counterimpulse, or during the diastolic counterimpulse experimentation among animals (6 dogs), we calculated the cardiac flux. At the clinic, by comparison with the cardiac flux obtained by the conventional method, the cardiac flux obtained by the intra-aortic differential pulse pressure contour method showed the correlation coefficient r = 0.83 through 20-time calculations (n) and the regression equation Y = 0.385X + 1.99. During the experimentation among animals, by comparison with the cardiac flux by means of the electromagnetic meter, this method showed the correlation coefficient r = 0.77, n = 21 and the regression equation Y = 0.586X + 0.579. If we note the coefficient between the cardiac flux at the abnormally - high value and the diastolic increase wave form, we found an inverse correlation. That is to say, the higher cardiac flux is parallel to the narrowness of the wave form. As shown above, this method would seem reliable, but it is most important to establish clearly the condition; and at the moment, for the cardiac flux of more than 7.0, it would be necessary to analyse the diastolic wave form. The study on the peripheral vessels and the venous system is quite important with the necessity of a knowledge on the differential pulse pressure contour method.
