Three cases of delivery under sodium valproate--placental transfer, milk transfer and probable teratogenicity of sodium valproate.

Here is a report of three cases of delivery from women who have taken sodium valproate (VPA). One neonate was born with polydactylism, and we reviewed the congenital malformations in children of mothers who have taken VPA. In the other two cases, we also examined the levels of VPA in the cord serum and breast milk. The levels of VPA in the cord serum were 50 micrograms/ml at 12 hours after the last maternal dose and 75 micrograms/ml at 14 hours after the last maternal dose. The blood levels of VPA in the two mothers were 48-59 micrograms/ml and 55-85 micrograms/ml in their late pregnancy, respectively. The levels of VPA in the breast milk were 2.0-3.5 micrograms/ml and the ratio of VPA in the breast milk to blood was 2-3%. These results suggest the high rate of placental transfer and the low rate of milk transfer of VPA.

Renal hypophosphatemic rickets. Growth and mineral metabolism after treatment with calcitriol (1,25-dihydroxyvitamin D3) and phosphate supplementation.

To delineate the mineral metabolism of renal hypophosphatemic rickets and to update progress in linear growth after calcitriol (1,25-dihydroxyvitamin D3) therapy, the medical records of 19 patients were examined retrospectively from January 1978 through December 1985. With a mean (+/- SD) follow-up period of 42.0 +/- 5.4 months after calcitriol had been administered for at least 12 months, the growth measurements were as follows: the percentile weight (mean +/- SD) remained unchanged, with the initial being 12.3% +/- 17.3% and the final being 15.3% +/- 18.6%, and the length/height percentiles were -2.7% +/- 5.9% and -2.4% +/- 4.4%. The growth velocity index showed a significant improvement from mean values of 61.7% (at age 2 years) to mean values of 101.2% (at age 11.2 years). Serum phosphate concentrations rose from the initial value of 2.9 +/- 0.6 to 3.5 +/- 0.8 mg/dL (0.9 +/- 0.2 to 1.1 +/- 0.3 mmol/L). The effects of calcitriol on renal function were tested by creatinine clearance values, which were 127 +/- 22 mL/min/1.73 m2 (2.12 +/- 0.37 mL/s/1.73 m2) at the conclusion of the study, compared with 128 +/- 25 mL/min/1.73 m2 (2.13 +/- 0.41 mL/s/1.73 m2) obtained at the initiation of calcitriol therapy. We conclude that calcitriol treatment of renal hypophosphatemic rickets in children results in improvement of growth velocity and serum phosphate concentration without deterioration of renal function.

Personality and neuropsychological aspects of temporal lobe epileptics and schizophrenics.

The present study was designed to ascertain and explore whether distinct personality profiles and neuropsychological features characterized temporal lobe epilepsy (TLE) vs. schizophrenic patients (Sc). The subjects were 20 epileptics and 19 schizophrenics matched with an overall IQ. They underwent various personality and neuropsychological tests. These results showed that the personality of the left-TLE patients tended to be more sensitive, aggressive, hostile and emotionally labile than the right-TLE and Sc patients. Finally, the Sc patients showed poor performance in verbal and nonverbal delayed tasks, especially on the right visual hemifield ones. These results suggested that they have the frontal lobe dysfunction, especially of the left hemisphere.

Multiple cutaneous neurilemmomas as a skin manifestation of neurilemmomatosis.

Four cases of neurilemmomatosis with multiple cutaneous neurilemmomas are described, including clinical, histologic, and electron microscopic features. Thirteen definite cases and sixteen probable cases of neurilemmomatosis with cutaneous tumors were collected from the Japanese literature. The skin lesions of multiple cutaneous neurilemmomas are nodules, infiltrated pigmented lesions, and plaque-like lesions consisting of papules. From the findings presented here, neurilemmomatosis is a clinical entity clearly distinguished from the neurofibromatosis of von Recklinghausen's disease.

Changes in subcortical visual and auditory evoked potentials following amygdaloid kindling in cats.

In this study we dealt with the changes in visual and auditory evoked potentials following kindling, to reveal the distant effects of epileptic activity. The experiments were performed using cats. Visual and auditory evoked potentials were obtained initially. Daily stimulation of 60 Hz (rectangular wave, 1 ms in duration, 500 microA in peak current, 2-s train) was given to the right amygdala, for kindling. After the completion of kindling, evoked responses were recorded again. In the auditory system the changes of responses in the cortex, medial geniculate nucleus, and cochlear nucleus were distinguished. The changes of potentials in the subcortex were larger than those at the cortical level. For visual evoked potentials there was a discrepancy between stimulation with light and electrical stimulation of the optic chiasm. There was no significant change in amplitude of visual evoked potentials by flash. In the case of optic chiasm stimulation, an enlargement of evoked responses was obtained. These results indicate modality-specific change of the auditory system and widespread subcortical change. These results might be caused by some vulnerability of the auditory system in the case of amygdaloid kindling, as a result of the epileptogenic process.

Bullous Darier's disease.

A 50-year-old woman, with multiple, hyperkeratotic, brown papules on the so-called seborrheic areas of the body, had had vesicles and bullae on the inner aspects of the lower extremities and the flexor aspects of the upper extremities each summer since childhood. Her father, brother, and daughter had similar papular lesions. Histologically, suprabasilar lacunae and dyskeratosis of epidermal cells were observed in the hyperkeratotic papules as well as in the vesicular and bullous lesions. The major difference between the histologic features of the hyperkeratotic papules and those of the vesicles or bullae was the size of the lacunae.

Alterations of amygdaloid kindling phenomenon following repeated electroconvulsive shocks in rats.

In this study we investigated the effect of the electroconvulsive shock (ECS) soon after each amygdaloid stimulation (KS) upon the development of a kindling phenomenon. Thirty male Wistar albino rats were used in this study. Once daily stimulation of 200 microA, 1 msec, 60 Hz, 2 sec train by the right amygdaloid stimulation electrodes and the recording of the EEG were done. Rats of the Kindling + ECS group received the ECS through the clipped ears for five minutes after the KS. Behavioral manifestations, the duration of afterdischarge and the numbers of spikes were counted for five minutes at the time of every stimulation. The numbers of times the KS required to develop each stage were stage 1:4.8, stage 2:8, stage 3:8.6, stage 4:9.6, stage 5:12.6, respectively, on the kindling group. On the ECS-treated group, those were stage 1:8, stage 2:17, stage 3:18, stage 4: more than 20. The ECS soon after the amygdaloid stimulation retarded the process of kindling significantly. This result is consistent with the hypothesis that the electroconvulsive shocks disturb the information of the synaptic pathway before the consolidation of a central neuroplastic change underlying kindling.

Phylogenesis and kindling.

In regard to the phylogenesis and the kindling phenomenon, we reported on the kindling phenomenon in several species, behaviorally and electroencephalographically. In accomplishing the kindling phenomenon, the time courses of the lower animals seem to be faster than that of the higher animals. Afterdischarges and spontaneous hypersynchronous potentials were described electroencephalographically. For the phylogenetic study, the spontaneous hypersynchronous potential (interictal spike) and its secondary epileptogenesis to the secondary site (mirror focus formation) were useful. Secondary epileptogenesis in the lower animals seem to be established faster than in the higher animals. The order in the time courses of the behavioral kindling phenomenon in several species might be presumably concomitant with the order of the secondary epileptogenesis to the contralateral site. Finally we pointed out several factors to influence secondary epileptogenesis.
