[On the advisability of arterial reconstructions in conditions of the general surgery department of the Central Regional Hospital].

The analysis of operations on arteries in conditions of the general surgery department of CRH. During 2003-2004 years--27 operative interventions on arterial system are executed. Middle age sick 58.9+/-2.5 years. Postoperative lethality has made 13.3+/-8.77 % and 25+/-15.3 % after scheduled and emergency operations accordingly. The first operational experience angiolsurgery, including reconstructive arterial operations, in conditions of the general surgery branches CRH was favorably reflected in results of treatment of patients with an arterial pathology.

[Strategy of treatment of patients with recurrent spontaneous pneumothorax: is active surgical treatment necessary?].

The diagnostic accuracy of computed tomography in treatment of bullous disease as the most frequent cause of recurrent spontaneous pneumothorax was 55.5%, while that of roentgenography was 16%. The recovery of patients with recurrent spontaneous pneumothorax treated by active surgical measures takes place in 85% of cases, while with drainage of the pleural cavity--in 65%.

[Duodenal injuries in the district surgeon's practice].

Under conditions of the Pavlovsk region operations were fulfilled on 337 patients with traumas of organs of the abdominal cavity. 15 patients died. In 6 patients injuries of the duodenum were treated. The early admission, adequate operative interventions resulted in the recovery of 4 patients.

[The role of diagnostic and treatment quality in differential salary of hospital surgeons].

One of the priority trends in the work of medical institutions is the development of suggestions on the payment for the work of the medical staff depending on the volume and quality of treatment. The assessment of the work of 3 resident doctors of the department was performed in 1180 patients with special reference to the rating, complexity of the case of hospital management, working load, integral coefficient of the quality. The role of the diagnosis and treatment quality for differential payment was determined.

[The role of complex investigation of the surgeons' activities in the assessment of effectiveness and quality of medical service].

The introduction of computerization, personal computers in particular, in the system of management of medical institutions has greatly widened the possibilities of processing the routine information, and automated calculation of the cost price of treatment. The cost price of treatment of 1654 patients of a surgical unit was analyzed. In 1180 patients treated by 3 doctors the cost price of treatment was assessed with special reference to the rating, complexity of the hospital supply of the case, load, integral coefficient of the quality. The complex method of the assessment of the hospital surgeon's work allows to determine ways of economical effectiveness in treatment of patients.

[Nephrectomy in general surgical hospital].

In the central regional hospital 50 nephrectomies were performed for traumas and different diseases of the kidney, 44 of them being fulfilled through a laparoscopic access. This access allowed the volume of operation to be expanded in 58% of the patients. In 8% of the patients massive hemorrhage took place. Four patients died. Nephrectomy following pretreatment of the renal vessels allows to reduce risk of the operation.

[The treatment of patients with acute appendicitis by a short-term stay in the hospital].

An analysis of results of treatment of 102 patients with acute appendicitis has shown that use of non-direct endolymphatic therapy decreases the amount of antibiotics and analgetics administered after surgery, leads to less frequency of postoperative complications and the patients can be discharged from the hospital within the first 6 days after operation.

[Determination of the volume of infusion therapy].

A method of determining the diurnal volume of infusion therapy is described which takes into account the age and weight of the patients, normal indices of the diurnal requirements of water, values of morbid losses of water in enteroparesis, fever, dyspnea and vomiting. The calculation normogram proposed is based on the Aberdeen normogram and is recommended for wide clinical practice.
