[Angiotensin1-receptor antagonist losartan. Part I. Basic clinical pharmacology].

This is the initial part in a series of papers dealing with various aspects of clinical pharmacology of the first AT(1)-receptor antagonist losartan and its therapeutic use in hypertension, diabetic nephropathy, chronic heart failure, and acute phase of myocardial infarction. Mechanisms of action, pharmacological effects of losartan and other AT(1)-receptor antagonists, as well as data on pharmacokinetics of losartan, its long term tolerability and safety are reviewed in this communication.

[Interaction of angiotensin converting enzyme inhibitors with nonsteroidal antiinflammatory drugs].

Analysis of results of randomized controlled studies revealed pronounced attenuation of vasodilating, natriuretic, and cardioprotective effects of angiotensin converting enzyme inhibitors (ACEI) by concomitant use with of indomethacin and aspirin in patients with hypertension, chronic heart failure, and acute myocardial infarction. In some studies beneficial effects of ACEI were shown to be weakened by concomitant use with of other nonsteroidal antiinflammatory drugs, including selective inhibitors of cyclooxygenase-2.

[Aspirin in primary prevention of ischemic heart disease].

High efficacy of aspirin in secondary prevention after myocardial infarction and stroke is well established. However the place of aspirin in primary prevention of coronary heart disease and other cardiovascular diseases remains questionable. Analysis of results of large controlled studies of efficacy and safety of aspirin in high risk subjects shows that low doses of aspirin should be used in men with high risk of coronary heart disease but without main risk factors of stroke.

[The effect of long-term uncontrolled antihypertensive therapy on the echocardiographic signs of left-heart hypertrophy in patients with mild and moderate arterial hypertension].

The data are presented on the absence of a stable positive trend in echocardiographic characteristics of the left heart and intracardiac hemodynamics after 8 months of controlled antihypertensive therapy, 12-month follow-up and uncontrolled outpatient treatment. 12 months after discontinuation of controlled antihypertensive therapy arterial pressure was much higher than it was at the end of the treatment in patients taking antihypertensive drugs irregularly or not taking them at all. Intracardiac hemodynamics returned to the baseline. As shown by echo-CG, frequency of left ventricular hypertrophy 12 months after the end of antihypertensive treatment rose from 25.8 to 45.2% (p < 0.01), of right ventricular hypertrophy from 14.5 to 17.7%.

[Differences in hemodynamic effects of the cardioselective beta-adrenoblocker acebutolol and non-selective beta-adrenoblocker propranolol in long-term antihypertensive therapy].

The cardioselective beta-adrenoblocker acebutolol used as a course therapy for 12 weeks was found to be a highly beneficial antihypertensive agent. The antihypertensive effect of the agent given in doses of 400-800 mg/day was as pronounced and prolonged as that of propranolol, 80-160 mg/day, though there is a tendency for acebutolol to show its complete or partial antihypertensive effect rather at the end of monotherapy than propranolol. At the same time the bradycardiac effect was more pronounced in propranolol therapy. The antihypertensive effect of acebutolol, 400-800 mg/day, was revealed after 2 weeks of its use and persisted within the entire 12 weeks of therapy. The drug was well tolerated. In contrast to propranolol, a non-selective beta-adrenoblocker having no intrinsic sympathomimetic activity, acebutolol failed to produce adverse effects, such as by decreasing cardiac output and increasing total peripheral vascular resistance. The agent had a less negative chronotropic effect.

[Left ventricular myocardial hypertrophy in patients with arterial hypertension: the clinical characteristics and prognostic significance].

Blood pressures in the out- and inpatient settings, ECG findings and 24-hour Holter monitoring data, hemodynamic parameters, exercise test results, carbohydrate tolerance, blood lipid levels, and renal function were studied in 21 males with arterial hypertension and left ventricular hypertrophy as evidenced by echocardiography and 20 patients of the same age who had the same disease length. The patients with left ventricular hypertrophy generally showed a more severe course of arterial hypertension: more elevated and stable blood pressure, more renal affliction than those without left ventricular hypertrophy. ECG monitoring indicated that they had ventricular premature contraction and atypical episodes of ST-segment depression. These patients displayed lower heart rate and its lower increment, but higher diastolic blood pressure at the peak of exercise. A five-year follow-up suggests that the patients with left ventricular hypertrophy (38.1%) have higher incidence rate of cardiovascular events than those without this abnormality (5%) (p < 0.01).

[Prolapse of the tricuspid valve].

Of 113 patients who were investigated by echocardiography with regard to mitral prolapse, simultaneous tricuspidal prolapse was detected in 13.3% (15 patients), isolated tricuspidal prolapse (TP) was detected in one patient. Incomplete right bundle branch (RBB) was detected in 11 TP patients, complete RBB in one that might be associated with the involvement in the degenerative process of the proximal part of the right bundle branch in the zone adjoining the tricuspid valve. The characteristic feature of MP and TP was the presence of two systolic clicks.

[Chordal rupture syndrome of the mitral valve].

Fifty-eight patients with chordal ruptures of the mitral valve were examined. The main echocardiographic signs of chordal ruptures of the anterior and posterior mitral cusp were described. The authors provide the results of the use of two-dimensional echocardiography for diagnosing chordal ruptures of the mitral valve depending on the etiological factor of chordal pathology (mesenchymal abnormalities, rheumatic fever, infective endocarditis, coronary heart disease).

[Clinical echocardiography characteristics of cardiac myxomas].

Thirteen patients with heart myxomas (11 myxomas of the left atrium, 1 myxoma of the right atrium, 1 myxoma of the right ventricle) were placed under observation. The clinical manifestations of the myxomas were many and varied and depended on the site, size and mobility of tumor, the presence or lack of autoimmune processes (arthritis, pericarditis, myocarditis), and embolic complications. A high diagnostic value of one- and two-dimensional echocardiography is noted in the diagnosis of myxomas. In view of a possibility of radical surgical treatment early diagnosis of myxomas assumes the leading role for the disease prognosis.

[Echocardiographic study of the coronary vessels].

The coronary arteries were studied in normal subjects and in patients with coronary heart disease, using two-dimensional echocardiography. The authors succeeded in obtaining the image of the major trunk of the left and of the opening of the right coronary artery and in ascertaining various forms of coronary artery lesions. In twenty cases the findings of echocardiography and coronary angiography were compared.

[Current capabilities of the echocardiographic study of coronary arteries].

Literature data are presented on the possibility of ultrasound diagnosis of coronary artery lesions. Current methods of coronary artery examination are described and echocardiographic criteria of stenosis, atherosclerotic lesions and coronary artery aneurysm are delineated. The authors have examined 10 healthy subjects, 21 patients with coronary heart disease, 1 patient with left main coronary stenosis, 2 patients with proximal stenosis of the right coronary artery, documented by coronaroangiography.

[Clinical aspects and diagnosis of the mitral valve prolapse syndrome].

The work discusses the results of the examination of 108 patients with the syndrome of mitral valve prolapse encountered in the presence of various diseases (cardiomyopathy, acquired heart diseases, ischemic heart disease, pericarditis, etc.). The methods for the diagnosis of this syndrome are discussed: electro- phono-, echocardiography, radiography of the heart, and ventriculography. The clinical manifestations of the syndrome are analyzed.
