Iron overload: detection using a micromethod for iron-binding capacity.

Depending upon the transferrin saturation value, 214 serum samples were divided into three groups--iron overloaded (46 cases), iron deficient (61 cases) and normal (107 cases)--and tested with a micromethod based upon detection of unsaturated iron binding capacity. All the samples with iron overload could be distinguished from the other two groups, the results of the normal and iron deficient groups showing wide scatter and overlap. The high prevalence of iron deficiency anaemia and thalassaemia (iron overload) syndromes in India and other developing countries emphasises the need to differentiate these disorders at the earliest opportunity. The micromethod can be of immense help as it is a simple, rapid and inexpensive.

Correlation between morphological and cytochemical heterogeneity of acute promyelocytic leukemia (APL) and its association with myelodysplasia.

Sixty one cases of acute promyelocytic leukemia (M3) were diagnosed between Jan., 1985 and Dec., 1988. Morphologically 46 cases (75.4%) were characterised as typical M3 and 15 cases (24.6%) as M3 variant. Typical M3 cases had higher number of hypergranular promyelocytes and Auer rods (P less than 0.001). The cytochemical stains of myeloperoxidase, Sudan Block and Black and chloroacetate esterase were strongly positive in M3 typical and mild to moderately positive in M3 variant cases. Alpha-naphthyl acetate esterase positivity with fluoride inhibition was seen only in M3 variant cases (80%). The clinical and haematological parameters including marrow blast count were not significantly different in the two groups. This study has shown that M3 variant cases more frequently express heteregenous cytochemical patterns and myelodysplastic changes.

Granular cell variant of acute lymphoblastic leukaemia.

A rare morphological variant of acute lymphoblastic leukaemia in a ten years old child is described. Ninety five percent of the lymphoblasts had significant number of course granules in the cytoplasm. Diagnostic and prognostic significance of this variant is discussed.

Dysplastic eosinophils in three patients with acute promyelocytic leukaemia.

Three cases of acute promyelocytic leukaemia (M3) with dysplastic eosinophils are reported. Promyelocytes from these cases showed classical features of hypergranular acute promyelocytic leukaemia. All the cases had minimal organomegaly and disturbances of coagulation were minimal. All the cases were associated with dysplastic eosinophils in the marrow but very few of these eosinophils were found in peripheral smear. One of the three patients presented with a mass in the vertebral column, a feature not uncommonly seen in a variant form of AML-M2 with eosinophilia. Dysplastic eosinophils appear to arise from leukaemic clone itself. These three cases probably represent hitherto undescribed morphological variant of acute promyelocytic leukaemia where leukaemic cells show limited differentiation capability to dysplastic eosinophils.

Effect of repeated exposure to lindane and cadmium on lindane metabolism in rats.

The effects of daily administration of cadmium and lindane for 35 days on the metabolism of lindane in rats were investigated. The results indicate that cadmium induces a significant inhibition of lindane metabolism, since the group dosed with lindane plus cadmium had a significantly higher concentration of lindane in plasma and tissues than the group dosed with lindane alone. The inhibition in the metabolic rate of lindane is associated with the cadmium-induced alterations in the disposition of essential trace elements, Zn, Cu and Fe, in the liver.

Fine needle aspiration biopsy of extramedullary leukemia.

OBJECTIVE: To study the cytologic features and role of fine needle aspiration biopsy (FNAB) in the diagnosis of extramedullary leukemia. STUDY DESIGN: Forty-one cases of extramedullary leukemia diagnosed by FNAB were analyzed along with their detailed clinical and hematologic features. RESULTS: Common sites of leukemic involvement were lymph nodes (34), skin (4), orbit (1), eyelid (1) and breast (1). The most common variety of leukemia was chronic myeloid in the chronic phase (17). Twenty-six patients were referred to the cytology clinic for FNAB as the initial screening test. In the majority of fresh cases, leukemia was not the first possibility considered, and FNAB played an important diagnostic role. No gross discrepancy was noted in any of the cases. CONCLUSION: FNAB is helpful in the diagnosis of extramedullary involvement by leukemia because of the good morphologic detail of blasts and other granulocytic cells. However, for more accurate subclassification of a hematologic disorder, other hematologic investigations are mandatory.
