Early detection of sternal dehiscence by conventional chest X-ray.

BACKGROUND: Diagnosis of sternal dehiscence after sternotomy for cardiac surgery is still made clinically. The aim of this study was to identify radiographic signs of sternal dehiscence by routine chest X-ray (CXR) in patients with and without clinically diagnosed sternal dehiscence. METHODS: 75 patients (group I: 65 +/- 9.3 years, f/m = 12/63) with clinically diagnosed sternal dehiscence, necessitating surgical revision and 75 patients with uneventful sternal healing (matched to group I by age, sex, preoperative risk factors and surgical procedures; group II: 66 +/- 9.0 years, f/m = 12/63) were included in this study. Serial CXRs immediately after surgery until re-intervention or discharge were analyzed by a radiologist, blind to the date of redo surgery. RESULTS: In 39 patients of group I (52%) vs. 8 (10.7%) in group II, abnormalities in the sternal wire and/or a midsternal stripe could be found (p < 0.0001): rotated wires (p = 0.003), shifted wires (p = 0.043), and ruptured wires (p = 0.312). Seven patients presented with combined wire abnormalities in group I vs. 0 in group II. Midsternal stripe sign could be detected in 26 patients of group I vs. 3 in group II (p < 0.0001). Sternal dehiscence was suspected based on the above mentioned abnormalities as early as three days postoperatively (Q1 = 2; Q3 = 8 days) in 39 patients, whereas clinical diagnosis of sternal dehiscence was delayed up to ten days postoperatively (Q1 = 7; Q3 = 13 days). CONCLUSIONS: Radiographic signs of sternal dehiscence could be detected before the clinical diagnosis was apparent and predicted sternal dehiscence in more than half of the patients.

[Prevention of thromboembolism in trauma surgery by dose adjustment of low molecular weight heparin depending on levels of TAT and D-dimer].

Deep vein thrombosis (DVT) is reported in up to 40% of trauma patients. The individual risk is nearly unpredictable. A daily single dose of a low molecular weight heparin (LMWH) was administered as prophylaxis to 518 trauma patients who were examined preoperatively and up to 10 days postoperatively in a prospective study. They were divided into two groups: group I comprised surgery of the hip and femur as well as total knee replacement and group II knee and lower leg surgery. Thrombin-antithrombin complex and D-dimer were analyzed. A second daily dose of LMWH was added if D-dimer exceeded the cutoff. If ultrasound was suspicious for DVT venography was added. Deep vein thrombosis was seen in five cases (group I=4, group II=1), without pulmonary embolism. TAT and D-dimer were significantly higher in group I than in group II ( p<0.005). One hundred patients, 79 of them belonging to group I, were treated with a second dose of LMWH. The daily cutoff had the highest sensitivity and specificity for day 4. Due to individual monitoring of coagulation markers, the risk for thromboembolism compared to actual data in the literature seems to be reduced.

[Artefact reducing in diagnosis of lung embolism using spiral CT with saline bolus].

PURPOSE: To improve the diagnostic efficacy of bolus-enhanced spiral CT (SCT) in the detection of pulmonary embolism using a saline push immediately after bolus injection of the contrast medium. PATIENTS AND METHODS: The study included 90 patients with suspected acute or chronic pulmonary embolism. The CT scan was performed in a caudocephaled direction. In Group I (n = 60) we applied a bolus contrast injection (120 ml, 3 ml/s, 300 mg J/ml), after a median delay of 25 s. Group II (n = 30) had the same contrast injection which was immediately followed by an additional saline push (60 ml, 2 ml/s). Streak artifacts originating from high contrast concentrations in the superior vena cava were rated on a 4-point scale for different locations: right pulmonary artery, pars basalis, truncus anterior, and the segmental upper lobe arteries. RESULTS: The incidence of artifacts in group I was nearly twice as high as in group II. The difference was significant (p < 0.05) for the upper and anterior superior lobe artery, the right pulmonary artery and the pars basalis. CONCLUSION: The presented protocol significantly reduces artifacts mainly by a washout of contrast medium in the superior vena cava.

Screening for gestational diabetes: variation in guidelines.

OBJECTIVE: To compare published guidelines concerning screening for gestational diabetes. STUDY DESIGN: Systematic search and comparative analysis of published guidelines. Appraisal of guidelines quality. Simulation analysis. RESULTS: Ten published guidelines proposed either universal screening (5), selective screening (3) or screening when clinically indicated (2). Variations of testing schedules and blood glucose thresholds were observed. The quality of the published guidelines was low, on average 22 (8-51) percentage points on the assessment scale. These differences would have led to large variations in the number of patients to be screened. CONCLUSIONS: Large variations between guidelines have been observed which would translate in large practice variations, if the guidelines were systematically applied. These variations are partially explained by the absence of definite evidence that universal or selective screening for gestational diabetes do more good than harm on infant and maternal health. The methodology of developing guidelines should be more evidence based, systematic and explicit.

Computed tomography-based tracheobronchial image reconstruction allows selection of the individually appropriate double-lumen tube size.

OBJECTIVES: To determine whether individualized selection of double-lumen tubes or alternatives based on three-dimensional reconstruction of the tracheobronchial image from routine preoperative computed tomography (CT) scans leads to clinically appropriate choices. DESIGN: Prospective observational study; comparison to historic controls. SETTING: Anesthesia and radiology facilities of a university medical center. PARTICIPANTS: Forty-nine patients undergoing thoracic surgery requiring one-lung ventilation. INTERVENTIONS: Three-dimensional image reconstruction of individual tracheobronchial anatomy was performed from routine preoperative spiral CT scans as well as from scans of five left-sided and four right-sided double-lumen tubes. Results of image-based tube size selection were compared with literature recommendations. Prospectively, individualized tube selection was performed by superimposition of printed transparencies of tubes over the tracheobronchial system and was validated using bronchoscopic and clinical criteria (n = 24). MEASUREMENTS AND MAIN RESULTS: Three-dimensional reconstruction visualized individual anatomy with good accuracy and resolution. Correlations between patient morphology and tracheobronchial dimensions were weak (height versus mainstem bronchial diameters: r < 0.50). In 11 of 48 patients (23%). CT-fitted double-lumen tube sizes would have differed from a conventional height-based and gender-based selection. Individual, prospective, CT-based double-lumen tube selection was associated with (1) good fit and positioning confirmed by fiberoptic bronchoscopy, (2) adequate bronchial cuff seal volumes, (3) complete lung separation, and (4) oxygenation and ventilation parameters during one-lung ventilation similar to those with conventional size selection. In one patient, three-dimensional CT study allowed noninvasive evaluation of a tracheal stenosis precluding double-lumen tube placement. CONCLUSION: Individualized selection of double-lumen tube size using CT-based reconstructions of tracheobronchial anatomy leads to clinically appropriate choices. Risks resulting from variations in tracheobronchial morphology are recognized in advance.

Paclitaxel arrests growth of intracellular Toxoplasma gondii.

Addition of paclitaxel (Taxol) at a concentration of 1 microM to Toxoplasma gondii-infected human foreskin fibroblasts arrested parasite multiplication. Division of the T. gondii tachyzoite nucleus was inhibited, leading to syncytium-like parasite structures within the fibroblasts by 24 h after infection and treatment of the cultures. By 4 days after infection and treatment of the cultures with paclitaxel, this inhibition was irreversible, since the arrested intracellular form was incapable of leaving the host cell, infecting new cells, and initiating the growth of tachyzoites with normal morphology. Specifically, when paclitaxel was added to infected cells for 4 days and then removed by washing and the infected, paclitaxel-treated cells were cultured for 4 more days, there were no remaining T. gondii organisms with normal morphology. Syncytium-like structures in the cultures that were infected and treated with paclitaxel for 8 days were similar in appearance to those in preparations of infected paclitaxel-treated fibroblasts that had been cultured for 24 to 48 h. Pretreatment of the tachyzoites for 1 h with paclitaxel followed by the removal of the paclitaxel by repeatedly centrifuging and resuspending the parasites in fresh medium without paclitaxel and then adding fresh medium prior to culture of the parasites with fibroblasts did not prevent their invasion of fibroblasts but did affect their subsequent ability to replicate within fibroblasts. Pretreatment of the fibroblasts with paclitaxel also diminished subsequent replication of T. gondii in such host cells after 8 days. Thus, paclitaxel alters the ability of T. gondii to replicate in host cells. Inhibition of parasite microtubules by such compounds at concentrations which do not interfere with the function of host cell microtubules may be useful for development of novel medicines to treat T. gondii infections in the future.

[Spiral CT and 3-D reconstruction of the tracheobronchial tree for the selection of the proper double-lumen tubes before individual lung ventilation].

PURPOSE: To assess the potential of 3-D spiral CT in the selection of adequate double-lumen tubes (DLT) for one-lung-ventilation. MATERIALS AND METHODS: 3-D spiral CT of eight different DLTs was performed with standardized acquisition parameters: section thickness 4 mm, table speed 6 mm/s, increment 3 mm and standardised thresholds, window settings and magnification zoom. The accuracy was confirmed by measuring 3-D objects on screen and original DLTs by vernier caliper. 3-D spiral CT was performed in 20 patients preoperatively with slice thickness 4-8 mm, pitch 1-1.5, increment 4-8 mm. To select the adequate DLT size the 3-D reconstructions of the patient's tracheobronchial tree and of the DLT were superimposed by templates or computer animation. The accuracy was controlled by endoscopic and clinical measurements. RESULTS: The accuracy of the 3 D spiral CT was nearly 0.1 mm. The superimposition with templates was easy and reliable. The 3-D spiral CT determined DLTs were shown to fit perfectly by clinical and endoscopic measurements in all cases. CONCLUSION: 3-D reconstructions of the tracheobronchial tree obtained from routine preoperative spiral CT scans allow for an easy and accurate individual selection of double-lumen tubes.

Congenital toxoplasmosis transmitted from an immunologically competent mother infected before conception.

Congenital transmission of Toxoplasma gondii from a mother who was apparently immunologically competent and who had toxoplasmic lymphadenitis 2 months before conception is described. Since no T. gondii-specific serological data were available for this mother from the time her lymph node biopsy specimen was obtained, the specimen was studied by polymerase chain reaction (PCR) to determine whether the T. gondii B1 gene was present. The predictive diagnostic value of histologic findings previously considered to be classic signs of T. gondii lymphadenitis also was studied. This was done by correlation of serological tests diagnostic of acute acquired T. gondii infection and presence of characteristic findings in biopsy specimens from persons without known immunocompromise. Both PCR and review of the characteristic features of her lymph node biopsy specimen confirmed the diagnosis of preconceptual infection in the mother. We also discuss two other cases in which apparently immunologically competent mothers with preconceptually acquired infection transmitted this parasite to their fetuses.

[Detection of a bronchopleural fistula with spiral CT and 3D reconstruction].

A patient with a carcinoma of the oral cavity, pleural empyema, persisting seropneumothorax, and suspected bronchopleural fistula underwent computed tomography for further evaluation. Volume acquisition using the spiral technique was the prerequisite for three-dimensional reconstruction, which was done by minimum intensity-projection and seed-based "volume of interest" segmentation. The diagnosis "bronchopleural fistula", which was displayed in its entire course, was confirmed.

Desipramine and testicular swelling in two patients.

Two patients are described who developed testicular swelling and pain during treatment with desipramine. The symptoms disappeared when desipramine was discontinued and in one case returned when desipramine treatment was reinitiated. The observations suggest that testicular swelling and pain are side effects of desipramine.
