Influence of alveolar support on stress in periodontal structures.

The influence of alveolar bone support on the functional capability of a tooth remains unclear. It was hypothesized that a reduction in alveolar support causes an increase of maximum stress in the periodontal structures. Mathematical models of the maxillary incisor to simulate in vivo tooth movement were constructed with periodontium of normal or reduced bone height, and normal or widened periodontal ligament (PDL) space. Under simulated bite force, the maximum tensile stress at the lingual cervical region in the PDL increased with bone height reduction, but decreased with PDL widening. The compressive stress at the cervical region in the cortical bone was no more than 22% of the yield strength of bone, and did not increase by the height reduction with widened PDL. The result suggests that the height reduction potentially causes mechanical damage to the PDL, but, of itself, is not likely to have a negative effect on the bone.

Finite element contact stress analysis of the RPD abutment tooth and periodontal ligament.

OBJECTIVES: The purpose of this study was to determine the influence of the occlusal rest position in removable partial dentures on the displacement of the abutment tooth and the stress distribution in the periodontal ligament (PL). METHODS: We constructed three-dimensional finite element models of the mandibular first and second premolars. A layer of the PL and a mesial or distal occlusal rest were produced on the second premolar as an abutment. A zero displacement was prescribed on the outer surface of the PL and the first premolar. In each simulation, the rest was moved 0.05 mm vertically to the apical direction, with or without restriction of horizontal movements. We simulated the contact phenomena on the abutment surfaces, and calculated the movements of the abutment and stress distributions in the PL. RESULTS: We observed a maximum distal displacement of 42 microm at the buccal cusp of the abutment and a principal compressive stress of 0.35 MPa in the PL when the abutment was vertically loaded by a distal rest that was allowed to move horizontally. However, the displacements and stresses were relatively small, and were all within the physiological limitations of the tissues. The restriction of the horizontal movement of the rests was effective in reducing the horizontal displacements of the abutment, regardless of the rest position. CONCLUSIONS: The single vertical load exerted from either the mesial or distal rest on the abutment was unlikely to cause any mechanical damage to its supporting tissues.

Ileal mucosa-associated lymphoid tissue lymphoma showing several ulcer scars detected using double-balloon endoscopy.

A 72-year-old man was admitted to our hospital to undergo a novel small-intestinal endoscopic procedure. He had had occasional episodes of hematochezia over a 2-year period, during which he had been hospitalized twice previously. However, numerous investigations, including hematological and biochemical studies, gastroscopy, colonoscopy, computed tomography, scintigraphy, and angiography had failed to detect the source of bleeding in the gastrointestinal tract. On this admission, double-balloon enteroscopy was performed and revealed several ulcer scars with localized dilation of the ileum. Histopathological examination of the biopsy specimens revealed no abnormal findings. Partial resection of the ileum was performed to prevent further gastrointestinal bleeding, and histopathological examination of the resected specimen revealed aggregation of atypical lymphocytes, predominantly in the muscularis propria layer. Immunohistochemical examination demonstrated that the tumor cells were positive for CD20 and BCL2, but negative for UCHL1. Based on these findings, the lesion was diagnosed as a marginal-zone B-cell lymphoma of mucosa-associated lymphoid tissue. Eighteen months after surgery, the patient was still in complete remission.

Effect of metal strengthener length on stress distribution in acrylic denture bases: a finite element study.

This study investigated the influence of metal strengthener length on stress created in acrylic denture bases in relation to location of the vertical support to the dentures. Finite element analysis was conducted to calculate stress generated in the straps of 2 mm-thickness and 18 mm-width that were reinforced with metal strengtheners of five different lengths. A vertical biting force of 60 N was directed on one end of each strap, while the other end was fixed. Vertical movements were restricted at one of the three support locations between the centre of the strap and the loading site. When the straps were vertically supported near the loading site, greater maximum tensile stresses were seen in the straps with relatively short strengtheners than those shown in the straps with longer strengtheners. The metal strengtheners with sufficient length may provide a preventive denture design against the acrylic fractures.

A randomised, double blind, placebo controlled study of celecoxib, a selective cyclooxygenase 2 inhibitor, on duodenal polyposis in familial adenomatous polyposis.

BACKGROUND: Non-selective cyclooxygenase (COX) inhibitors (non-steroidal anti-inflammatory drugs) inhibit large bowel carcinogenesis in patients with familial adenomatous polyposis (FAP). Their role in the duodenum of these patients is less certain. The disease modifying activity of specific COX-2 inhibitors has not been explored in humans. PATIENTS AND METHODS: This was a randomised, double blind, placebo controlled study of celecoxib (100 mg twice daily (n=34) or 400 mg twice daily (n=32)) versus placebo (n=17), given orally twice daily for six months to patients with FAP. Efficacy was assessed qualitatively by blinded review of shuffled endoscopy videotapes comparing the extent of duodenal polyposis at entry and at six months and quantitatively by measurement of the percentage change in duodenal area covered by discrete and plaque-like adenomas from photographs of high and low density polyposis. RESULTS: Shuffled and blinded video review showed a statistically significant effect of 400 mg twice daily celecoxib compared with placebo treatment (p=0.033) with all five independent observers scoring a beneficial effect. Overall, patients taking celecoxib 400 mg twice daily showed a 14.5% reduction in involved areas compared with a 1.4% for placebo (p=0.436). However, patients with clinically significant disease at baseline (greater than 5% covered by polyps) showed a 31% reduction in involved areas with celecoxib 400 mg twice daily compared with 8% on placebo (p=0.049). CONCLUSIONS: A panel of five endoscopists found a significant reduction in duodenal polyposis after six months of treatment with celecoxib 400 mg twice daily. COX-2 inhibition may help this otherwise untreatable condition.

Dynamic retentive force of a mandibular unilateral removable partial denture framework with a back-action clasp.

PURPOSE: The purpose of this study was to investigate the dynamic retentive force of a mandibular unilateral distal-extension partial denture framework with a back-action clasp that was designed in a buccally tilted cast on a dental surveyor. The retention mechanism of this framework was analyzed in comparison with other typical unilateral and bilateral frameworks for the mandibular Kennedy class II case. MATERIALS AND METHODS: Experimental gold alloy frameworks of three designs were repositioned to the master cast, and lifted upward to the vertical direction to the occlusal plane. The load required to dislodge each framework was recorded. The retentive force of each framework was determined by the maximum load in a dislodgment cycle. The bending strength of a plain clasp pattern of the same length as the clasp arm used in each framework was also measured by cantilever beam test. RESULTS: The mean retentive force of the unilateral framework with a back-action clasp of relatively low bending strength was significantly higher (P < 0.01) than that of the unilateral framework with two Akers clasps of relatively high bending strength, and 70 to 80% of the bilateral framework with two Akers clasps on the edentulous side and a double Akers clasp on the contralateral side. The unilateral framework with a back-action clasp showed the greatest early load resistance in the dislodgment cycle among the three designs. CONCLUSION: The reasonable retention mechanism was demonstrated by the unilateral framework with a back-action clasp.

Short-range polaron correlations in the ferromagnetic La1-xCaxMnO3

We use neutron scattering to demonstrate the presence of lattice polarons and their short-range correlations for several samples of La1-xCaxMnO3 in the Ca doping range 0.15</=x</=0.3. We establish the doping dependence of the orientation, commensuration, and coherence length of the polaron correlations and show that the populations of correlated and uncorrelated polarons are intimately related to the transport properties of the materials.

The effect of celecoxib, a cyclooxygenase-2 inhibitor, in familial adenomatous polyposis.

BACKGROUND: Patients with familial adenomatous polyposis have a nearly 100 percent risk of colorectal cancer. In this disease, the chemopreventive effects of nonsteroidal antiinflammatory drugs may be related to their inhibition of cyclooxygenase-2. METHODS: We studied the effect of celecoxib, a selective cyclooxygenase-2 inhibitor, on colorectal polyps in patients with familial adenomatous polyposis. In a double-blind, placebo-controlled study, we randomly assigned 77 patients to treatment with celecoxib (100 or 400 mg twice daily) or placebo for six months. Patients underwent endoscopy at the beginning and end of the study. We determined the number and size of polyps from photographs and videotapes; the response to treatment was expressed as the mean percent change from base line. RESULTS: At base line, the mean (+/-SD) number of polyps in focal areas where polyps were counted was 15.5+/-13.4 in the 15 patients assigned to placebo, 11.5+/-8.5 in the 32 patients assigned to 100 mg of celecoxib twice a day, and 12.3+/-8.2 in the 30 patients assigned to 400 mg of celecoxib twice a day (P=0.66 for the comparison among groups). After six months, the patients receiving 400 mg of celecoxib twice a day had a 28.0 percent reduction in the mean number of colorectal polyps (P=0.003 for the comparison with placebo) and a 30.7 percent reduction in the polyp burden (the sum of polyp diameters) (P=0.001), as compared with reductions of 4.5 and 4.9 percent, respectively, in the placebo group. The improvement in the extent of colorectal polyposis in the group receiving 400 mg twice a day was confirmed by a panel of endoscopists who reviewed the videotapes. The reductions in the group receiving 100 mg of celecoxib twice a day were 11.9 percent (P=0.33 for the comparison with placebo) and 14.6 percent (P=0.09), respectively. The incidence of adverse events was similar among the groups. CONCLUSIONS: In patients with familial adenomatous polyposis, six months of twice-daily treatment with 400 mg of celecoxib, a cyclooxygenase-2 inhibitor, leads to a significant reduction in the number of colorectal polyps.

Endoscopic mucosal resection using a partial transparent hood for lesions located tangentially to the endoscope.

BACKGROUND: Numerous methods have been developed to resect early-stage gastric and esophageal cancers, but it is difficult to resect lesions viewed tangentially with the endoscope. METHODS: We have designed and developed an original method of endoscopic mucosal resection using a partial transparent hood to treat difficult cases in which the lesions are located tangentially to the endoscope. The hood was attached on the right side of the endoscope and, after insertion into the stomach or the esophagus, was lightly pressed on the orad side of the lesion. Then the lesion was resected using grasping forceps and electrosurgical current snare. RESULTS: The average diameter of specimens was 26 +/- 8 mm in gastric lesions and 20 +/- 3 mm in esophageal lesions, both 6 mm larger than those obtained by previous methods. CONCLUSION: This device and technique were extremely useful for mucosal resection of lesions located tangentially to the endoscope.

A novel cis-acting element regulates HES-1 gene expression in P19 embryonal carcinoma cells treated with retinoic acid.

The regulatory mechanisms of mammalian hairy and Enhancer of split homologue-1 (HES-1) genes were examined in mouse P19 embryonic carcinoma cells (P19 cells). Undifferentiated P19 stem cells expressed a basal level of the HES-1 gene, whereas the expression of this gene was increased upon induction of the cells to a neural cell lineage using retinoic acid (RA). Reporter co-transfection analysis identified an activating region within the upstream promoter region of HES-1 from nucleotides -201 to -172. This activating region, called activating region X (ARX), shows a high GC content and contains both an AP-2 binding motif and a CCAAT box. An electrophoretic mobility shift assay using nuclear proteins extracted from P19 cells showed that ARX forms a specific DNA-protein complex. Importantly, ARX-dependent transcription, as well as ARX-binding activity, was significantly increased in P19 cells treated with RA. These results indicate that ARX transduces signals that up-regulate HES-1 gene expression in response to RA-treatment. Thus, a novel cis-acting element involved in HES-1 gene regulation that plays a role in RA-induced neural differentiation of P19 cells has been identified.

Crack initiation modes in bilayered alumina/porcelain disks as a function of core/veneer thickness ratio and supporting substrate stiffness.

We hypothesize that the fracture resistance of alumina core/porcelain veneer disks increases and that crack initiation shifts from veneer to core as the core/veneer thickness ratio (t(C)/t(V)) increases from 0.5/1.0 to 1.3/0.2, or as the elastic modulus of the supporting substrate (E(S)) to which it is resin-bonded increases from 5.1 to 226 GPa. When supported by a low-modulus substrate, disks with low t(C)/t(V) ratios exhibited cracks in the veneer and within the core, while those with high t(C)/t(V) ratios demonstrated core cracks, but not veneer cracks. None of the disks supported by Ni-Cr alloy (E = 226 GPa) exhibited core cracks. These results support the hypothesis that the crack initiation site shifts as the t(C)/t(V) ratio increases, but the increase in E(S) did not affect the crack initiation site. This study suggests that the t(C)/t(V) ratio is the dominant factor that controls the failure initiation site in bilayered ceramic disks.

The cDNA cloning and transient expression of an ovary-specific 17beta-hydroxysteroid dehydrogenase of chickens.

A cDNA clone, pc17bHSD, was obtained from the chicken ovarian cDNA library by its partial homology to the cDNA sequence of the rat 17beta-hydroxysteroid dehydrogenase (17beta-HSD). The cDNA insert of pc17bHSD is 979bp long and contains an open reading frame (ORF) of 906bp. The deduced amino acid sequence of the ORF shows 48 and 50% overall identity with those of the rat and the human type-1 17beta-HSD, respectively. Five sequence regions common to the short-chain alcohol dehydrogenase superfamily are well conserved, including the YxxxK sequence motif at the active site. Northern blot hybridization detected a transcript of about 1kb only in ovaries of both sexually immature and mature female chickens. The 17beta-HSD activity, which was highly specific to the interconversion between estrone and estradiol-17beta, was detected in the cytoplasmic fraction of human 293 cells transfected transiently with an expression vector carrying the c17bHSD cDNA sequence, pcDNAI/c17bHSD. From these results, it is concluded that the pc17bHSD is the cDNA clone for the ovary-specific molecular species of 17beta-HSD in chickens.

Keap1 represses nuclear activation of antioxidant responsive elements by Nrf2 through binding to the amino-terminal Neh2 domain.

Transcription factor Nrf2 is essential for the antioxidant responsive element (ARE)-mediated induction of phase II detoxifying and oxidative stress enzyme genes. Detailed analysis of differential Nrf2 activity displayed in transfected cell lines ultimately led to the identification of a new protein, which we named Keap1, that suppresses Nrf2 transcriptional activity by specific binding to its evolutionarily conserved amino-terminal regulatory domain. The closest homolog of Keap1 is a Drosophila actin-binding protein called Kelch, implying that Keap1 might be a Nrf2 cytoplasmic effector. We then showed that electrophilic agents antagonize Keap1 inhibition of Nrf2 activity in vivo, allowing Nrf2 to traverse from the cytoplasm to the nucleus and potentiate the ARE response. We postulate that Keap1 and Nrf2 constitute a crucial cellular sensor for oxidative stress, and together mediate a key step in the signaling pathway that leads to transcriptional activation by this novel Nrf2 nuclear shuttling mechanism. The activation of Nrf2 leads in turn to the induction of phase II enzyme and antioxidative stress genes in response to electrophiles and reactive oxygen species.

Regulatory mechanisms of cellular response to oxidative stress.

An antioxidant responsive element (ARE) or electrophile responsive element (EpRE) mediates the transcriptional activation of genes encoding phase II drug metabolizing enzymes. The ARE consensus sequence shows high similarity to an erythroid gene regulatory element, and based on the observation, we have recently found that transcription factor Nrf2 is essential for the coordinate induction of phase II detoxifying enzymes. The expression of anti-oxidative stress enzyme genes is also regulated by Nrf2. Detailed analysis of the regulatory mechanisms of Nrf2 activity has ultimately led us to the identification of a new protein, which we have named Keap1, that suppresses Nrf2 activity by specific binding to its evolutionarily-conserved N-terminal Neh2 regulatory domain.

Infrared gold alloy brazing on titanium and Ti-6Al-4V alloy surfaces and its application to removable prosthodontics.

PURPOSE: This study investigated the area size of the flow of a gold braze alloy on commercially pure titanium and Ti-6Al-4V alloy plates, and elemental composition at the interface was determined. In the second part of this study, the tensile strengths of titanium plates brazed using a gold alloy were investigated. MATERIALS AND METHODS: Chips of Type IV gold alloy and silver braze alloys were melted onto commercially pure titanium and Ti-6Al-4V surfaces in a dental infrared radiation unit. Flow area of each braze alloy was measured using a digital image analyzer. Tensile specimens (n = 5) were also prepared by infrared brazing using the braze alloys. Five specimens for each combination of the two titanium plates and the two braze alloys were subjected to tensile loading using a Universal testing machine. Electronprobe microanalysis of x-rays at cross-section of the brazed joints to determine elemental composition across the interface, as well as scanning electron microscopic observation at the fracture surfaces, were also conducted. RESULTS: The braze alloys flowed well and spread over the Ti and Ti-6Al-4V plates. Braze alloy type significantly influenced flow, and the gold alloy flowed less on the titanium materials. The mean tensile strengths of Ti and Ti-6Al-4V plates brazed using the gold braze alloy were 219 MPa and 417 MPa, respectively. The fracture surfaces of Ti-6Al-4V specimens with the gold braze alloy exhibited typical ductile behavior. Ti with the same braze alloy showed brittle surfaces. A greater concentration of Cu was found at the Ti with gold braze interface. CONCLUSION: The flow and the tensile strength of the gold alloy coating on titanium surface by means of an infrared brazing is adequate for dental use.

Molecular cloning and characterization of meichroacidin (male meiotic metaphase chromosome-associated acidic protein).

We have isolated a cDNA clone encoding a germ cell specific protein from an expression cDNA library prepared from the mouse testis, using testis-specific polyclonal antibodies. Sequence analysis of the cDNA revealed that the deduced amino acid sequence consisted of 284 residues, including a nominal repeat structure in the N-terminal region. Northern blot analysis revealed the presence of a transcript of 1.3 kb exclusively expressed in the testis and ovary, but at relatively low levels in the ovary. In contrast, no other tissues and organs expressed significant levels of the transcript. Expression of the mRNA in the testis was first detected on day 14 in postnatal development. Western blot analysis showed the presence of the protein with a molecular weight of approximately 40 kDa and an isoelectric point of 4.9. The protein was exclusively found in the testis and ovary, but in a far lesser amount in the ovary as was the case with the transcript. Immunohistochemical examination revealed that the protein was predominantly present in the cytoplasm in pachytene spermatocytes through to round spermatids. However, during the disappearance of the nuclear envelope at both the first and second meiotic divisions, the protein was localized around the metaphase chromosomes and spindles. Because of this, the name meichroacidin which stands for male meiotic metaphase chromosome-associated acidic protein is proposed for this antigen. The highly regulated stage-specific expression of meichroacidin and its specific association with the metaphase chromosomes and spindles suggest that the protein plays important roles in male meiosis.

The influence of some demographic and clinical variables on psychosomatic traits of patients requesting replacement removable partial dentures.

The purpose of this study was to investigate the influence of some demographic and clinical variables on psychosomatic traits of the patients who visited the prosthodontic section of the Dental Hospital at Tokyo Medical and Dental University for replacement of their removable partial dentures (RPDs). Sixty-six patients with 50 maxillary and 44 mandibular RPDs completed the psychological measurements concerning their dentures using visual analogue scales (VAS). Female patients rated significantly lower satisfaction with the comfort of their dentures than males. Younger patients expressed less satisfaction with the aesthetics of their dentures than the older patients. The period of wearing the present denture significantly correlated with psychological measures of pain, comfort, and general satisfaction, suggesting that this clinical variable may be an important factor. There was no significant correlation between the patients' general satisfaction with dentures and the retention/stability scores assessed by the examiner.

The cDNA cloning and transient expression of a chicken gene encoding a follicle-stimulating hormone receptor.

A clone, pcFSHR, containing a 3.1-kb insert was isolated from a cDNA library of chicken ovarian follicles by screening with an RT-PCR-generated cDNA probe for a putative N-terminal half-region of chicken FSH-R. The deduced amino acid sequence of pcFSHR exhibits about 84% identity to those of mammalian FSH-R and about 70% to those of mammalian LH-CG-R. Northern blot analysis for total RNA preparations from several chicken tissues revealed that transcripts detected with pcFSHR were present exclusively in ovary and testis. In a chicken ovary, the transcripts were detected in granulosa but not in theca cells. A radioligand receptor assay for the human 293 cells transfected with an expression vector containing the insert of pcFSHR demonstrated the production of a receptor which showed about 10-fold higher affinity for chicken FSH than for chicken LH. The affinity for chicken FSH was significantly higher than for human FSH. In consistency with the ligand specificity of the receptor, a significantly higher level of intracellular accumulation of cAMP was detected when the transfected cells were treated with chicken FSH than with chicken LH. From these results, we conclude that pcFSHR is a cDNA clone for the chicken FSH-R.
