Foreign body giant cell reactions and ossification associated with benign melanocytic naevi.

AIMS: To assess the incidence of foreign body giant cell reactions and ossification in benign/melanocytic naevi; and to examine their pathological features to gain an insight into their pathogenesis. METHODS: Intradermal (n = 185) and compound naevi (n = 110) from a routine histology service, together with 60 naevi submitted to an ophthalmic pathologist, were examined for foreign body reactions and ossification. Additional cases were identified prospectively in the course of routine reporting. The clinical and pathological features of positive cases were assessed. RESULTS: Foreign body reactions were identified in nine (4.9%) intradermal and four (3.6%) compound naevi, but in none of the naevi from around the eye. One intradermal naevus showed ossification. A further 11 naevi showing foreign body reaction and five showing ossification alone were identified prospectively. The 24 naevi showing a foreign body reaction had a similar age and sex distribution to controls but were more likely to occur on the head and neck. The reaction usually occurred deep to the naevus, sometimes in relation to a hair follicle, and fragments of hair or keratin were identified in most. Osteoid or bone was present within the reaction in five. In six other naevi, all from the head and neck of women, osteoid or mature bone was present deep to the naevus in the absence of a giant cell reaction. CONCLUSIONS: Foreign body giant cell reactions occur not uncommonly in relation to benign naevi, as a result of follicular damage, possibly due to trauma. The similar siting of foci of bone suggests that ossification occurs as a secondary phenomenon in these cases.

Intradural bronchogenic cysts.

The pathological findings of an intradural and extramedullary cyst in the mid cervical spinal canal are described in a 55 year old woman who presented with a short history of pain and paraesthesia of the right arm. Intradural well defined solitary cystic lesions in the spinal canal are uncommon, their pathogenesis is poorly understood, and their nomenclature is confusing. In this case the cyst was a bronchogenic cyst; these are a rare form of such cysts and they are thought to be a malformation arising from a split notochordal syndrome and not a teratoma.

alpha-Tocopherol concentrations in serum and tissues of sheep fed different sources of vitamin E.

Thirty-five crossbred wethers were used to determine the concentrations of alpha-tocopherol in serum and tissues after oral supplementation of six different vitamin E product forms. Five wethers were assigned to each of the following treatments: 1) control, no supplemental vitamin E (C), 2) emulsifiable DL-alpha-tocopheryl acetate-dry (Rovimix E-50% SD), 3) nonemulsifiable DL-alpha-tocopheryl acetate-dry (Rovimix E-50% Ads), 4) emulsifiable DL-alpha-tocopheryl acetate-liquid (Rovimix E-40% Dispersible Liquid Concentrate [DLC]); 5) emulsifiable DL-alpha-tocopherol-liquid (Hoffmann-La Roche, E-40% DLC alcohol), 6) micellized DL-alpha-tocopheryl acetate-liquid (Bioglan, Inc., E-20%); and 7) micellized DL-alpha-tocopherol-liquid (Bioglan, Inc., E-20%). Animals were supplemented daily with 1,000 IU of their respective vitamin E sources for 56 d. Blood samples were collected daily from d 0 to 7 and weekly until d 56. Animals were subsequently killed by exsanguination after stunning and eight different tissues were collected for alpha-tocopherol analysis. There were effects of day, treatment, and day x treatment interaction on serum alpha-tocopherol. All supplemented groups were higher in serum alpha-tocopherol concentration than were the C wethers. The emulsifiable vitamin E alcohol liquid product form (Treatment 5) yielded higher (P less than .01) serum alpha-tocopherol concentration than the emulsifiable acetate liquid product (Treatment 4). Sheep on Treatment 5 reached maximum concentration on d 1, sheep on Treatment 6 on d 2, and the sheep on the remaining Treatments by d 3. Blood sera alpha-tocopherol concentrations stabilized by d 6 in all supplemented groups.(ABSTRACT TRUNCATED AT 250 WORDS)

Performance of skin biopsies by general practitioners.

OBJECTIVE: To evaluate and appraise skin biopsies performed by general practitioners and compare their performance with that of hospital doctors. DESIGN: Retrospective analysis of histology records. SETTING: University hospital. SUBJECTS: Records of 292 skin biopsy specimens obtained by general practitioners and 324 specimens obtained by general and plastic surgeons. MAIN OUTCOME MEASURES: Clinical and pathological diagnoses and completeness of excision. RESULTS: The number of specimens received from hospital surgeons and general practitioners increased over the study period; the proportion of specimens from general practitioners rose from 17/1268 (1.3%) in 1984 to 201/2387 (8.7%) in 1990. The range of diagnoses was similar among hospital and general practitioner cases, although malignancy was commoner in hospital cases (63/324 (19%) v 14/292 (5%) in general practitioner cases; chi 2 = 28, p less than 0.00001). Completeness of excision was less common among general practitioners than hospital surgeons (150/233 (3/15 malignant) v 195/232 (57/63); chi 2 = 22, p less than 0.00001). CONCLUSIONS: The increase in minor surgery has implications for the staffing and finance of histopathology departments. General practitioners must be given proper training in performing skin biopsies, and all specimens should be sent for examination.

Lack of C-erbB-2 protein expression in pulmonary carcinoid tumours.

To determine if amplification of the C-erb-B2 proto-oncogene could be correlated with prognosis in carcinoid tumours, 49 pulmonary carcinoid tumours (26 typical, 23 atypical) were examined using a polyclonal antibody to the C-terminal peptide of the C-erb-B2 protein sequence. No C-erb-B2 gene product could be shown: the demonstration of C-erb-B2 does not seem to help, therefore, in determining diagnosis or prognosis in pulmonary carcinoid tumours.

Nucleolar organiser regions in normal, hyperplastic, and neoplastic endometria.

Nucleolar organiser regions (NORs) can be visualised by a silver stain that shows NOR-associated proteins as black intranuclear dots (AgNORs). NORs are loops of DNA that transcribe to ribosomal RNA, and the number of AgNORs is thought to reflect cellular proliferative activity. A study of AgNOR counts in normal, hyperplastic, and neoplastic endometria is reported. The highest AgNOR counts were found in normal proliferative endometrium and in well-differentiated endometrioid adenocarcinomas that were invading the myometrium. A mean AgNOR count of greater than 9 in curettage material from an atypical proliferative lesion of the endometrium is highly suggestive of an invasive neoplasm.

Effects of alpha and beta adrenoceptor blocking drugs and ACE inhibitors on long term glucose and lipid control in hypertensive non-insulin dependent diabetics.

Long-term glycaemic control measured by glycosylated haemoglobin concentration was compared in hypertensive NIDD's taking placebo, alpha-blockers, beta-blockers (alone and in combination) and an ACE inhibitor (enalapril) each for 4 months. Alpha-blockade caused no deterioration in long-term glycaemic control in NIDD's. Beta-blockers caused a deterioration in glycaemic control in NIDD's (with some adverse effects on lipid profile). This was especially apparent with the lipophilic beta-blocker P. Concurrent use of an alpha-blocker with a beta-blocker prevents the deterioration due to beta-blockade. The ACE inhibitor, enalapril caused an improvement in long-term glycaemic control without alteration of lipid control in NIDD's.

p21 ras protein expression in benign and malignant human breast.

The ras oncogenes encode for GTP binding and GTPase active proteins of relative molecular mass 21,000 (p21ras) which are involved in the transduction of stimuli for cell proliferation. There have been conflicting reports about the detection and significance of expression of p21ras protein in human breast disease as determined by immunohistochemistry. The antibody Y13-259, which detects a single protein of Mr 21,000, has been applied immunohistochemically to frozen sections of normal, benign proliferative breast, fibroadenomas, and carcinomas. Uniform staining of normal breast epithelium and myoepithelium was found, with occasional stronger staining in areas of epithelial hyperplasia in benign breast disease. Contrary to previous reports, decreased expression, usually heterogeneous, was found in half of the carcinomas examined. Thirty per cent of the carcinomas exhibited heterogeneous staining stronger than that of normal breast, interpreted as increased expression of p21ras protein. This did not relate to tumour grade or node status but showed a significant correlation with proliferation rate as determined by the monoclonal antibody Ki-67. This study confirms previous reports that p21ras protein expression is a feature of normal cells, and has identified increased expression in 30 per cent of tumours associated with higher proliferation rates, which is a lower incidence than previously claimed when a different antibody was employed.

Evaluation of personal protection methods against phlebotomine sand flies including vectors of leishmaniasis in Panama.

Three personal protection methods were evaluated against phlebotomine sand flies in Panama. Skin applications of five selected repellents including deet (N,N-diethyl-m-toluamide) provided a mean coefficient of protection (CP) of 99.2% against the attack of at least three sand fly species. Deet-treated net jackets also provided good protection, but it was concluded that an additional application of repellent to the unprotected face was necessary for maximum protection. Permethrin-treated clothing did not provide the protection expected. Apparently sand fly behavior and resistance to quick knock-down were responsible for the numbers of bites recorded, and therefore maximum protection from bites would require application of deet or another suitable repellent to the exposed skin when wearing permethrin-treated clothing.

Image-guided biopsy in women with breast cancer presenting with peritoneal carcinomatosis.

When women with a history of breast cancer present with peritoneal carcinomatosis, the differential diagnosis lies between recurrent breast cancer or a new primary tumor. This scenario is of particular relevance to women with a BRCA gene mutation, who have a genetic predisposition to develop second primary tumors of the ovary, fallopian tube, and peritoneum. We describe the use of image-guided core biopsy as an alternative to laparoscopy or exploratory laparotomy in providing minimally invasive diagnosis in this increasingly common clinical dilemma.
