Xanthogranulomatous cholecystitis: clinical, sonographic, and CT findings in 26 patients.

OBJECTIVE: The purpose of our study was to evaluate the sonographic and CT features of xanthogranulomatous cholecystitis, correlating the pathologic and surgical findings. MATERIALS AND METHODS: Xanthogranulomatous cholecystitis was pathologically diagnosed in 26 patients from January 1996 to August 1998. The patients were 15 women and 11 men with a mean age of 63 years. All patients had preoperative sonography and nine also underwent CT In five patients, sonography was performed on the surgical specimen. Clinical indications for imaging included cholecystitis (14 patients), biliary colic (six patients), stone-induced pancreatitis (three patients), tumor (two patients), and gallstone ileus (one patient). RESULTS: The most characteristic sonographic finding, confirmed by sonographic study of the surgical specimens, was the presence of hypoechoic nodules or bands in the gallbladder wall, which were seen in 35% of the patients. Cholelithiasis and a thickened gallbladder wall were frequent findings. The most characteristic (specific) CT finding was a hypodense band in the gallbladder wall, seen in 33% of the patients. Two of twelve patients who underwent laparoscopic cholecystectomy required conversion to open surgery. CONCLUSION: Although the preoperative imaging diagnosis of xanthoganulomatous cholecystitis is difficult, the presence of hypoechoic nodules or bands in the gallbladder wall on sonography or of a hypodense band around the gallbladder on CT, is highly suggestive of this disease.

[Benign cervical mesenchymoma].

Mesenchymoma are growths made up for two or more mesenchyme derivatives that normally not joint in the same tumor, with the exception of the connective tissue which appears in all tumors of mesenchymal lineage. Benign mesenchymoma is a rare tumor more frequent in extremities and peri-renal area or, very rarely in head and neck territories, being the oropharynx the most encountered sitting in the latter place. The paper reports a clinical case of benign cervical mesenchymoma of a long course, more than 20 years, and big size, asymptomatic and diagnosed through fine needle biopsy. After total removal the histological examination showed a composite growth of extended areas of fibrous tissue associated to vessels and other zones of ripe fat tissue.

An unusual form of pneumatosis intestinalis associated with appendicitis.

A rare case of pneumatosis intestinalis is described in a patient with appendicitis. Gas in the colonic wall, epiploic appendices and mesentery caused a new appearance not previously reported on plain radiography or CT. The relevance of an associated redundant right colon is also discussed.

[Painful lymphocytic subacute thyroiditis].

Subacute granulomatous thyroiditis (SGT) and subacute lymphocytic thyroiditis (SLT) present a similar evolution during the first year, however, posteriorly, except on rare occasions, SGT maintains normofunction while SLT may present relapse with persistent goiter or permanent hypothyroidism requiring periodic follow up. The presence of spontaneous pain and very elevated VSG have been described accompanying SGT but not SLT histologically proven to be used for differentiating these entities. Two cases with clinical criteria and cytological diagnosis of SLT consulted for spontaneous thyroid pain are presented. VSG greater than 50 mm/1st hour was suggestive of SGT. The importance of cytology for the correct management of subacute thyroiditis is emphasized.

Normal pregnancy and delivery two years after adjuvant chemotherapy for grade III immature ovarian teratoma.

A 27-year-old woman with grade 3 immature teratoma of the ovary was treated with unilateral salpingo-oophorectomy and VAC combination chemotherapy. She received 6 postoperative courses, although initially 12 were planned. Chemotherapy had to be discontinued because of severe bone marrow toxicity. Eighteen months later the patient became pregnant and was ultimately delivered of a normal child. This is the second report of a successful pregnancy following VAC chemotherapy for immature teratoma.

[Biopsy of the temporal artery. Experience with the use of a protocol at an ENT department].

INTRODUCTION: Temporal artery (TA) biopsy is required for the evaluation of several clinical scenarios because of the therapeutic implications of a positive result. We describe the experience of our department, which is the center for this technique in our hospital. PATIENTS AND METHODS: A retrospective study was made of all TA biopsies performed in the Department of Otolaryngology, Hospital Sierrallana, from January 1995 to April 1997. RESULTS: Thirty-four biopsies of the TA artery were performed in 33 patients for possible temporal arteritis. Mean age was 76 years (range: 61-87 years). Twelve were male and 21 female. Twenty-two patients (66.6%) had recent onset cephalalgia, 11 (33.3%) polymyalgia rheumatica, 5 (15.1%) mandibular claudication, and 8 (24.2%) ocular symptoms. In 75.8% ESR was elevated. An abnormal TA exam was observed in 9 cases (27.3%), 6 of which had a positive biopsy. Only 1 of the 15 patients without local findings at the physical examination had a positive TA biopsy. No information on the physical exam was recorded for the other 2 patients with a positive biopsy. CONCLUSIONS: TA biopsy is a relatively easy surgical procedure with a very low rate of complications. An abnormal physical TA examination correlates with a positive histopathological study.

[Nasal sinus adenocarcinoma in patients exposed to wood dust in the Community of Cantabria, Spain].

OBJECTIVE: To study the incidence and clinical and histological features of sawdust-related nasosinusal adenocarcinoma (SRNA) in the Community of Cantabria. MATERIAL AND METHODS: A retrospective study was made of all patients diagnosed as SRNA in 9 years. RESULTS: Ten male patients, ranging in age from 56 to 64 years, were diagnosed as SRNA in this period. The most common location was the ethmoid. Histologically, six tumors were papillary and four were mucinous. Five patients received combined treatment (surgery and postoperative radiotherapy), one surgery alone, two radiotherapy, and one chemotherapy. The 5-year survival rate was 28.5%. The most frequent cause of death was local recurrence. CONCLUSIONS: The incidence of SRNA in the Community of Cantabria is less than 0.2 cases per 100,000 inhabitants/year. SRNA occurs almost exclusively in men and has an occupational origin. Papillary adenocarcinoma is the most frequent histological type. The treatment of choice is surgery associated with postoperative radiotherapy. The long-term prognosis is poor, so preventive measures to reduce exposure to sawdust are fundamental in risk groups.
