Some observations on the use of computerised tomography in Legg-Calve-Perthes' disease.

The use of computerised tomography in the assessment of hip pathologies in children is an uncommonly used modality. To date, we were unable to find any definite work on the use of CAT scan investigations in LCPD. Ten children with Legg-Calve-Perthes' disease were reassessed by two-plane AP and lateral radiographs and by computerised tomography. The findings strongly suggest that computerised axial tomography is an important modality capable of providing more detailed information on the distorted anatomy, magnitude and severity of the disruptive process in the femoral head. This modality demonstrated the presence of fragmentation and cyst formation in the femoral head even if two-plane radiographs appeared to be normal.

Diagnostic radiology in wartime.

During the Lebanon War, 1982, over 80% of the wounded were sent from the triage area of the hospital directly to the radiology department. This article reports changes in the working pattern and organization of the department that were instituted for emergency treatment in wartime, and describes radiological examination methods for different organs. Computerized tomography is emerging as the most important diagnostic tool in addition to conventional radiological examinations.

Computed tomography in the diagnosis of adrenal pathology.

Computed tomographic diagnosis of adrenal pathology, based on experience with 13 cases, is presented. These include five pheochromocytomas, two functioning cortical adenomas and one case each of nonfunctioning adenoma, adrenal cyst with adenoma, carcinoma, metastasis, and bilateral hyperplasia. One additional case of a small adrenal tumor of undetermined nature was probably a small nonfunctioning adenoma. The smallest tumor detected was 25 mm in diam and the largest was 150 mm. Other imaging modalities employed were mainly conventional urography with tomography and ultrasonography. By these means only the larger adrenal tumors were detected. The relative merits and drawbacks of other imaging procedures are discussed and compared to computed tomography. Computed tomography should be the initial imaging procedure of choice when adrenal pathology is suspected.

[Aortic regurgitation and aortic stenosis associated with gastrointestinal bleeding. Report of a case (author's transl)].

A 67-year-old woman with a double atherosclerotic aortic valvulopathy and rheumatoid arthritis presented recurrent episodes of gastrointestinal bleeding. Selective arteriography of the superior mesenteric artery demonstrated the existence of vascular malformations with extravasation of the contrast. The physiopathology of this lesion is unknown, but its incidence in adulthood suggests a degenerative process. A selective angiogram of the superior mesenteric artery is mandatory in patients with gastrointestinal bleeding and double aortic valvulopathy in order to establish the etiological diagnosis of the hemorrhage.

Superior vena cava syndrome: a new radiologic approach to diagnosis.

Superior vena cava syndrome, a cause of severe distress, requires careful assessment of its etiology before therapy can be instituted. The diagnostic work-up is varied depending on whether or not the superior vena cava syndrome is associated with a mediastinal mass. Fine needle aspiration biopsy has proved to be of considerable value in the evaluation of superior vena cava syndrome associated with a mediastinal mass.

Computed tomography in guiding of fine needle aspiration biopsy of the lung and mediastinum.

Computed tomography displays the image of the chest in a transverse plane; therefore localization and depth calculation of the lung and mediastinal lesions to be biopsied can be done easily and in a precise manner. Use of computed tomography for guiding the biopsy procedure itself ascertains control of the needle tip inside the lesion and assures a positive result of the biopsy. The procedure of using computed tomography for the planning and guiding of fine needle aspiration biopsy in lung and mediastinal masses is described. The results in 105 patients examined are discussed and the value of the method is assessed.

Combined treatment of concurrent laryngeal and tracheal stenosis.

This report describes a one-stage combined surgical procedure for laryngeal and tracheal stenosis presenting concurrently. The technique links together three new procedures. Autogenous hyoid bone transposition on a muscle pedicle for reconstruction of the larynx, together with implantation of a silicone T-tube in conjunction with a custom-carved silicone stent, for maintenance and resurfacing of both strictures. This technique has produced satisfactory results in four cases. In two of the patients there is sufficient follow-up to indicate the efficacy of this method.

Fine needle aspiration biopsy of osteolytic metastatic lesions.

The use of fine needle aspiration biopsy of solitary osteolytic-type bone metastases in 24 patients is described. A positive diagnosis of malignancy was obtained in 21 patients, as the softness of the osteolytic lesion yielded enough cell material for cytologic examination. No complications were encountered. The procedure can be performed with ease, even on an outpatient basis, without discomfort to the patient.

The heart in acute glomerulonephritis: an echocardiographic study.

Patients with acute glomerulonephritis often are seen with signs suggesting heart failure. Whether these signs are due to fluid overload secondary to kidney damage only, or whether there is associated myocardial damage has not been elucidated. Fourteen children with acute glomerulonephritis were studied by echocardiography during the edematous phase of the disease and five months later to evaluate cardiac function in this disease. Left ventricular size and function remained normal in all children throughout the study. The most consistent finding was enlargement of the left atrium during the edematous phase with a return toward normal values five months later. There was no correlation between blood pressure and the echocardiographic findings. This study suggests that signs of heart failure in acute glomerulonephritis are not due to myocardial damage but probably reflect fluid overload.

Invasive radiology in the diagnosis of mediastinal masses. Use of fine needle for aspiration biopsy.

A review is presented of the radiological and surgical procedures used for diagnosing mediastinal masses. The radiological examinations evaluate the size, shape and site of the mediastinal lesion, whereas the surgical methods provide tissue diagnosis. Fine needle aspiration biopsy which is an invasive radiological procedure has the advantage of obtaining cytological diagnosis. The method is described and the results of 56 biopsies in 38 patients are discussed in detail. The success rate is 76% and increasing steadily as further experience is gained. The complications are of a light nature and with the exception of one patient no treatment was necessary.

Fine needle aspiration biopsy in the diagnosis of mediastinal lesions.

In the presence of a mediastinal mass, fine needle aspiration biopsy is an alternative to other time-consuming and expensive diagnostic procedures. We have performed a total of 25 fine needle aspiration biopsies of mediastinal lesions in 18 patients. Although our material is too limited to reach definite conclusions as to risks, we have found the technique enables a high percentage of positive diagnoses and is well tolerated by the patient.

Radiology at war.

It can be assumed that the high percentage of wounded sent immediately from the triage area to the department of radiology was a reflection of the necessity for X-rays. The radiological examination was essential in reaching the correct diagnosis, and the radiologist was the central consultant, as by giving the diagnostic information and performing therapeutic angiographic intervention, he actively participated in the treatment of the patient. The radiologist and the surgeon together reached the final decision as to which injury prevailed and in which sequence treatment had to be planned. Hence, the department of radiology became the vital link in flow of patients from the sorting area to the operating theatre.

Radiologic aspects of Ask-Upmark kidney.

Radiologic work-up in a young girl suffering from severe hypertension revealed a small right kidney. In the pathological examination of the surgical removed right kidney segmental hypoplasia was found--Ask Upmark kidney. A detailed radiologic description of this entity is presented.
