HpSA fecoprevalence in patients suspected to have Helicobacter pylori infection in Istanbul, Turkey.

OBJECTIVE: This study aims to investigate the Helicobacter pylori antigen in the stool samples of patients suspected to have H. pylori infection. It also aims to determine the fecoprevalence in different age groups and to look for the relationship between the clinical symptoms seen in patients and the existence of H. pylori. Current information in Turkey is insufficient. METHODS: The Helicobacter pylori antigen was investigated in the stool samples of 445 patients of whom 148 were in the 2-15 year age group, 96 in the 16-30 year age group, 85 in the 31-40 year age group and 116 in the 41-48 year age group. The main clinical complaints of the patients (stomach pain, heart burn, indigestion, gas, nausea, vomiting, diarrhea, abdominal pain) were recorded. RESULTS: The Helicobacter pylori antigen was found in 36.6% (163/445) of the patients and in the statistical evaluation made for different age groups, a significant linear relationship was found between age and infection (chi2=14.77, p=0.002). CONCLUSION: It was found that stomach pain was seen at a higher rate in patients with H. pylori antigen compared to those without it. The difference was highly statistically significant (chi2=117.70, p<0.001, OR=20.36, 95% CI=10.56-39.27).

PER-1- and OXA-10-like beta-lactamases in ceftazidime-resistant Pseudomonas aeruginosa isolates from intensive care unit patients in Istanbul, Turkey.

The presence of PER-1- and OXA-10-like beta-lactamases was investigated by PCR in 49 ceftazidime-resistant Pseudomonas aeruginosa isolates from patients hospitalised in the 24-bed general intensive care unit of the Istanbul Faculty of Medicine during a 12-month period between February 1999 and February 2000. The clonal relatedness of the isolates was investigated by random amplified polymorphic DNA (RAPD) analysis, and the sequences of the PER-1 and OXA genes from all isolates were determined. The rates of resistance of the isolates to imipenem, aztreonam and cefepime were 98%, 92% and 96%, respectively, and to piperacillin and piperacillin-tazobactam were 41% and 37%, respectively. Using the double-disk synergy test, 37% (18/49) of the isolates were identified as extended-spectrum beta-lactamase producers. The PER-1 gene was identified in 86% (42/49) and the OXA-10 gene in 55% (27/49) of the ceftazidime-resistant isolates. Of isolates carrying the PER-1 gene, 48% (20/42) also carried the OXA-10 gene. The respective nucleotide sequences were identical for each isolate. Sixteen RAPD patterns were detected among the PER-1-positive isolates, but 60% (25/42) of the PER-1-positive isolates belonged to two distinct patterns, while the remainder exhibited a wide clonal diversity. The results indicated that the prevalence of PER-1- and OXA-10-like beta-lactamases remains high among ceftazidime-resistant P. aeruginosa isolates in Turkey.

Relationship between ciprofloxacin resistance and extended-spectrum beta-lactamase production in Escherichia coli and Klebsiella pneumoniae strains.

Resistance to fluoroquinolones has increased markedly since their introduction. Mechanisms of resistance to any antibiotic class might play a role in resistance to an unrelated antibiotic class. This study evaluated the relationship between extended-spectrum beta-lactamase (ESBL) production and ciprofloxacin resistance in Escherichia coli and Klebsiella pneumoniae strains isolated as causative agents of urinary tract infection. ESBL-producing strains were significantly more frequent among ciprofloxacin-resistant E. coli strains than among ciprofloxacin-susceptible E. coli strains (p = 0.015), but the difference was not significant among K. pneumoniae strains (p = 0.276).

Investigation of cross-resistance between rifampin and rifabutin in Mycobacterium tuberculosis complex strains.

In order to indicate the cross-resistance between rifampin (RMP) and rifabutin (RBU), minimal inhibitory concentrations (MICs) of RBU were investigated in 50 Mycobacterium tuberculosis strains. The MIC values of 25 RMP-susceptible (to 2 microg/ml) and 25 RMP-resistant (to 2 microg/ml) M. tuberculosis strains against RBU were determined by the Bactec TB 460 system. All of the RMP-susceptible strains were also susceptible to RBU (MIC < or = 1 microg/ml). Three out of 25 (12%) RMP-resistant strains were determined as susceptible to RBU. The high level cross-resistance (88%) obtained in this study highlights the importance of testing susceptibility to RBU prior to its inclusion in the tuberculosis treatment regimens at the Istanbul Faculty of Medicine.

Effect of some antibiotics on pigmentation in Serratia marcescens.

Serratia marcescens is characterized by its ability to produce a red pigment called prodigiosin. It is well known that there are some substances affecting pigmentation in bacteria. Cefoxitin, erythromycin, tobramycin, co-trimoxazole, imipenem and nitrofurantoin were found to have an inhibitory effect on pigmentation in a S. marcescens strain isolated from urine. It was also shown that the LD50 dose determined by inoculation of eight-week-old BALB/c mice with serial dilutions of overnight cultures of pigmented and nonpigmented variants was lower (LD50 = 300 x 10(3.94)) in the nonpigmented variant than in the pigmented one (LD50 = 300 x 10(5.58)). In addition, the Sereny test showed that in contrast to the pigmented variant, the nonpigmented variant caused keratitis in guinea pig eye.

Phenotypic variation and antifungal susceptibility patterns of Candida albicans strains isolated from neutropenic patients.

The aim of this study was to investigate the relationship between phenotypes of Candida albicans strains isolated from clinical specimens and the susceptibility of the strains to three antifungal agents, fluconazole, amphotericin B and flucytosine. Oropharyngeal, gastrointestinal and urogenital tract specimens were collected from 122 neutropenic patients who had received no previous prophylactic treatment. Each of 122 C. albicans strains recovered was found to express one of the six phenotypes: smooth, fuzzy, irregular, star, ring and stipple. The mean minimum inhibitory concentrations (MICs) of fluconazole was consistently higher for C. albicans strains expressing the stipple phenotype. The mean MICs for the six phenotypes of C. albicans strains ranged between 1.22 and 7.94 micrograms ml-1 for fluconazole, 0.99 and 2.55 micrograms ml-1 for amphotericin B and 1.23 and 1.83 micrograms ml-1 for flucytosine. The antifungal susceptibility of the stipple phenotype requires attention, especially in patients who are clinically unresponsive to fluconazole chemotherapy or in cases of life-threatening C. albicans infections of immunocompromised hosts. Long-term use of fluconazole may explain the outcome of the resistant stipple phenotype.

Phage types, antibiotic susceptibilities and plasmid profiles of Salmonella typhimurium and Salmonella enteritidis strains isolated in Istanbul, Turkey.

OBJECTIVE: To examine 13 Salmonella typhimurium and 22 S. enteritidis strains isolated from individual cases of gastroenteritis for their phage types, antibiotic susceptibilities and plasmid profiles. METHODS: The phage typing of S. typhimurium strains was done according to the method of Anderson et al, and the phage typing scheme of Ward et al was used for phage typing of S. enteritidis strains. Antibiotic susceptibility testing was performed by the Kirby-Bauer disk diffusion method. Extended-spectrum beta-lactamase production of the strains was determined by the three-dimensional method. Plasmid profiles of the strains were examined using the method described by Kado and Liu with some modification by Graeber et al. RESULTS: Two S. typhimurium strains were DT 193 and one was DT 22, whereas 10 strains were untypable. PT 4 was the predominant phage type among S. enteritidis strains. Four S. enteritidis strains were DT 6a, three strains were PT 1 and one strain was PT 8, whereas only one strain was untypable. Eleven of 13 S. typhimurium and three of 22 S. enteritidis strains were found to be multiresistant. Ten different resistance patterns among S. typhimurium and four different resistance patterns among S. enteritidis strains were detected. Extended-spectrum beta-lactamase production was detected in 10 of 13 S. typhimurium and in three of 22 S. enteritidis strains. All S. typhimurium strains but one were found to contain at least one plasmid, with molecular masses varying between 4 and 107 MDa, and 11 different plasmid patterns were determined. Plasmid pattern analysis permitted further differentiation of the S. enteritidis strains into nine groups. A serovar-specific virulence plasmid of 36 MDa was detected in 13 of 22 S. enteritidis strains. CONCLUSIONS: The results suggest that the majority of S. typhimurium strains were closely related.

The epidemiology of onychomycoses in Istanbul, Turkey.

Between September 1995 and September 1997, 1840 patients attending our mycology department were suspected as having onychomycoses. Of the patients 985 were females and 855 were males. A total of 426 finger nail and 1414 toe nail scrapings were examined. Both microscopic and culture tests were carried out for each specimen. In 759 cases positive fungal cultures were found and 1081 cases were determined as negative. One fungal species was isolated from 715 specimens, in 44 of the cases two different species were found.

Characterization of beta-lactamase gene blaPER-2, which encodes an extended-spectrum class A beta-lactamase.

Plasmidic extended-spectrum beta-lactamases of Ambler class A are mostly inactive against ceftibuten. Salmonella typhimurium JMC isolated in Argentina harbors a bla gene located on a plasmid (pMVP-5) which confers transferable resistance to oxyiminocephalosporins, aztreonam, and ceftibuten. The beta-lactamase PER-2 (formerly ceftibutenase-1; CTI-1) is highly susceptible to inhibition by clavulanate and is located at a pI of 5.4 after isoelectric focusing. The blaPER-2 gene was cloned and sequenced. The nucleotide sequence of a 2.2-kb insert in vector pBluescript includes an open reading frame of 927 bp. Comparison of the deduced amino acid sequence of PER-2 with those of other beta-lactamases indicates that PER-2 is not closely related to TEM or SHV enzymes (25 to 26% homology). PER-2 is most closely related to PER-1 (86.4% homology), an Ambler class A enzyme first detected in Pseudomonas aeruginosa. An enzyme with an amino acid sequence identical to that of PER-1, meanwhile, was found in various members of the family Enterobacteriaceae isolated from patients in Turkey. Our data indicate that PER-2 and PER-1 represent a new group of Ambler class A extended-spectrum beta-lactamases. PER-2 so far has been detected only in pathogens (S. typhimurium, Escherichia coli, Klebsiella pneumoniae, Proteus mirabilis) isolated from patients in South America, while the incidence of PER-1-producing strains so far has been restricted to Turkey, where it occurs both in members of the family Enterobacteriaceae and in P. aeruginosa.

The presence of black-pigmented gram-negative anaerobes in the oral cavity of edentulous subjects.

The aim of this study was to investigate whether black-pigmented Gram-negative anaerobes are part of the indigenous oral flora of edentulous subjects with or without dentures. Group I consisted of 11 subjects with dentures (mean age 63.6 years, range 52-75) and Group II consisted of 39 subjects with complete dentures in both jaws (mean age 59.3 years, range 37-80). Two microbial samples for microbiological examination were taken from each subject. One of them was from the dorsum of the tongue and the other was from saliva. Black-pigmented Gram-negative anaerobes > 10(7) cfu/ml were found in both samples. In 50 edentulous subjects, they were found more commonly from tongue (40%) than from saliva (26%). None of the subjects had Porphyromonas gingivalis. There was no statistically significant difference (P > 0.05) between the presence of black-pigmented Gram-negative anaerobes on tongue and in saliva in the two groups. Our results suggested that high levels of black-pigmented Gram-negative anaerobes may belong to the indigenous oral flora in edentulous mouths with or without dentures.

The effect of parenteral iron administration on the development of Staphylococcus aureus-induced experimental pyelonephritis in rats.

The first of the three groups of rats was taken as a control and the other two groups were injected with high (15 mg/kg) and low (5 mg/kg) doses of ferric ammonium citrate given intramuscularly twice daily for 5 days. Pyelonephritis was produced in all groups by intravenous inoculation with Staphylococcus aureus. Serum and urine of each rat was collected periodically and their iron content was determined. The severity of pyelonephritis was evaluated by determination of bacterial growth and pathological lesions in kidneys after 10 days of bacterial inoculation. The results showed that parenteral iron administration markedly aggravated pyelonephritis development in rats. But there was no significant difference in the severity of pyelonephritis between rats treated with high or low iron doses.

Aggravation by morphine and D-aspartic acid of pyelonephritis induced by i.v. inoculation of Staphylococcus aureus in rats.

Experimental pyelonephritis was induced by intravenous inoculation of Staphylococcus aureus in homozygous Brattleboro diabetes insipidus (Hom Brattleboro DI), heterozygous Brattleboro (Het Brattleboro) and Wistar rats. One group of rats from each strain was implanted with morphine-containing pellet three days before inoculation. Another series of groups received D-aspartic acid (D-ASP) intraperitoneally, starting three days before inoculation throughout the experiments. Owing to the inhibition by morphine or D-ASP of food intake, another control group from each strain was subjected to food restriction. Pyelonephritis development on the tenth day of inoculation was evaluated by the determination of viable bacteria in urine and total kidney tissue, and pathomorphological lesions in kidney. Hom Brattleboro DI rats appeared more resistant. Morphine or D-ASP significantly increased the findings in three strains of rat.

The inhibition by morphine and D-aspartic acid of antibody production against Salmonella typhimurium antigen in rats: its antagonism by L-aspartic acid.

The changes in the production of antibody against Salmonella typhimurium antigen were investigated in rats by means of the agglutination test after chronic oral administration of the L-asparaginase inhibitors morphine (M) or D-aspartic acid (D-Asp) alone or together with L-aspartic acid (L-Asp) and food restriction, all of which had been started five days before the injections of antigen. The statistical evaluation, carried out after the titers had been defined as -log2 of the highest dilution giving a positive agglutination reaction, showed that M or D-Asp significantly decreased antibody production in comparison with the immunized control or food restricted group. The concomitant administration of L-Asp appeared to significantly antagonize the inhibitory effect of both M and D-Asp. Therefore, the results were considered as further supporting evidence for the fact that the deleterious effect of M on the immune system and its functions might be related to the inhibitory effect of M on L-asparaginase activity.

An electronmicroscopic study of experimental hematogenous pyelonephritis.

This paper consists of an electronmicroscopic study of hematogenous experimental pyelonephritis. In the first 5 days cell degenerations mitochondrial swellings, slight lysosomal increase, rupture of the apical cell membranes, deterioration of the basal labyrinth and a leucocyte-predominant cell infiltration into the interstitial tissue were observed. On the 10th day, beside these common findings, the complete necrosis of the tubular cells was also observed and was supposed to be the result of an increase of lysosomes directly proportional with elapsed time and of the destruction and extreme contraction of the basal membrane. The cause of these destructions was supposed to be primary schaemia and probably the antigen-antibody-complement complex.
