A comparative study of Norinyl 1/35 versus Lo-Ovral in Ile-Ife Nigeria.

A comparative analysis of those women on Norinyl 1/35 versus those on Lo-Ovral showed those on Norinyl 1/35 were significantly (P less than or equal to 0.05) older than women in the Lo-Ovral group. There were no significant differences (P greater than or equal to 0.05) between the groups for other characteristics at admission. There were no significant differences (P greater than or equal to 0.05) between the groups at follow-up in the number of women reporting serious complications, menstrual complaints or other pill-related problems. The continuation rates at 12 months were 79.4 for the Norinyl 1/35 group and 76.7 for the Lo-Ovral groups. There were no pregnancies reported during the study period.

Cervical cytology in an urban Nigeria population.

The prevalence of cytologic abnormality was investigated among two population groups in an urban setting: those responding to a mass cytologic screening campaign and are without symptoms and those referred on account of symptoms or signs suggestive of cervical pathology. Among a total of 1550 clients so screened a 6% incidence of cytologic abnormality was found among the asymptomatic group contrasting with an overall 23% prevalence in the whole group. Even though a prevalence of 90% was found among the early symptomatic group, only 2 cases of CIS was found giving a prevalence of 13/10,000. This suggests cervical screening could prove highly effective in identifying cervical pathology at a significantly highly treatable state in this population.

Characteristics of contraceptive acceptors in an urban Nigerian setting.

Intensive efforts in promoting family planning concepts and contraceptive delivery in the Third World over the past two- and one-half decades have yielded only token dividends . This has occurred in Nigeria, despite the favorable government attitude. A study of the characteristics of current contraceptive acceptors showed an overwhelming percentage of acceptors are uneducated , married and from the lower socioeconomic class, a striking departure from usual expectations. Most acceptors prefer oral contraceptives. The status of acceptance of modern contraception by the educated population is still undetermined. Contraceptives appeared to be used primarily by women aged 30 and older in our population.

Characteristics and significance of the latent phase in the outcome of labor among Nigerian parturients.

The characteristics and significance of the latent phase in labor among 716 West African parturients were studied. A wide variation in duration (1 to 70 hours) of this phase was noted. A consistent correlation was found between the duration of the latent phase and subsequent events, such as the duration of the active phase, duration of ruptured membranes, and the total modes of delivery. This correlation holds true to all parities even when latent phase duration is within normal limits. No difference was noted in the incidence of abnormal latent phase between the primigravida and the multigravida. Although these findings have potentially useful but limited application, the significant correlation with Apgar score at 1 minute and the association of larger breeches with prolonged latent phase are sufficient predictive indices of great value to the obstetrician in the prompt management of such labors.

The significance of cholera outbreak in the prognosis of pregnancy.

Five hundred sixty-one persons were treated in a comprehensive cholera unit during the 1979-1980 cholera outbreak at Ile-Ife. Sixty-one pregnant cholera patients were identified and followed up. Compared to the general female population, all female cholera patients in the 15-29 year age group show significantly more resistance to the disease than those aged 30 years and above. The pregnant cases, as well as all reproductive-years age groups, showed significantly less mortality than both the non-pregnant patients and those at both extremes of age. Our findings show that pregnancy does not render the woman more susceptible and may, in fact, render her less susceptible after the first trimester, when prognosis brightens for both the mother and the fetus.

Antibiotic prophylaxis in high-risk obstetrics.

Unregistered patients, who subsequently had cesarian section for obstetric indications, were placed on prophylactic chloramphenicol in doses of 2-6 gm a day. These were matched with an equal number of randomly selected, similarly unbooked patients not receiving prophylactic antibiotics. These groups were given a fever work-up when sepsis manifested even in those previously on antibiotic therapy.Results showed little or no significant difference in the two groups when length of hospitalization and cost of care were compared. During the period of the study, a sample of booked patients, who subsequently had cesarian section, were also compared. Highly significant differences were noted. In Nigeria, a more rational approach to this group of significantly high-risk patients is advocated. A review of current practices in obstetrics is discussed.

Microadministration of ketamine during labor and delivery of Nigerian women.

Carefully controlled intravenous administration of ketamine produced excellent analgesia and dissociative sleep in all patients in the active phase of labor. The introduction to delivery interval was significantly shortened (3.6 vs 6 hours) in our general population. Of the 50 patients studied, 49 were unable to remember if contractions were painful and 80% experienced and narrated dreams. The auditory component was consistently intact in 82% of the cases; 78% described the total experience as pleasant, while the rest found it unpleasant or uncertain. The one-minute Apgar score of 6.8 was not significantly different from that of the general population.
