Selenium status of idiopathic infertile Nigerian males.

Selenium concentration in the sera and seminal plasma of 60 infertile males (40 oligospermia and 20 azoospermia) and 40 males with proven evidence of fertility (normospermia; control group) were estimated using atomic absorption spectrophotometry. Results were correlated with spermatogram and hormonal levels in order to determine their relationship and significance in male infertility. The mean serum concentrations of selenium was found to be significantly increased in oligospermic compared to azoospermic subjects and controls (p < 0.01), whereas the seminal plasma level was significantly higher in azoospermic compared to oligospermic subjects and controls (p < 0.001). Thus, the ratio of serum selenium to seminal plasma selenium was 1: 1 in controls, 4: 1 in oligospermia, and 1: 2 in azoospermic subject.A significant inverse correlation was observed between serum selenium level and sperm count (p < 0.01). Similarly, seminal plasma selenium correlated with spermatozoa motility, viability, and morphology. Serum selenium level shows positive correlation with the serum testosterone level (p < 0.01). In conclusion, there appears to be a physiological balance in the distribution of selenium in serum and seminal plasma compartment of control males. A disturbance in this balance has a significant influence on spermatogenesis. Selenium appears to have a positive influence on Leydig cells, thus influencing the secretion of testosterone.

Serum and seminal plasma prolactin levels in male attenders of an infertility clinic in Ibadan.

Serum and seminal plasma prolactin and testosterone were studied in 51 healthy-looking men who presented at the infertility clinic. They included 20 men with normospermia, 20 with oligospermia and 11 with azoospermia. The mean serum prolactin and gonadotrophin levels of the oligospermics and azoospermics were similar but significantly higher than those of the normospermics. The mean seminal plasma prolactin levels in the three groups were similar. The mean serum testosterone of the azoospermics was significantly higher than those of the oligospermics and normospermics who had similar levels. The seminal plasma testosterone of the oligospermics was similar to that of the azoospermics but significantly higher than that of the normospermics. A significant correlation occurred between serum prolactin and sperm motility. No correlation occurred between the serum and seminal plasma prolactin levels, and seminal plasma testosterone or between serum and seminal plasma testosterone. Seminal prolactin has no correlation with any biophysical parameters. It was concluded that poor spermatogenesis is associated with high serum prolactin in patients with infertility. Serum but not seminal plasma prolactin assay is useful in assessing male infertility.

Sildenafil citrate (Viagra) for the treatment of erectile dysfunction in Nigerian men.

Fifty-eight Nigerian outpatients with documented erectile dysfunction (ED) received open-label sildenafil citrate (Viagra) for 8 weeks. The 50-mg starting dose could be adjusted to 100 or 25 mg based on response and tolerability. The International Index of Erectile Function (IIEF) Questionnaire, a global efficacy question, and intercourse data recorded in a patient event log were used to assess efficacy. Frequency of penetration and maintained erection were both significantly enhanced (P<0.0001); 95% of patients reported improved erections and 81% of all attempts at intercourse were successful. Orgasmic function, sexual desire, intercourse satisfaction, and overall satisfaction also improved significantly (P&<0.0001). The most frequent adverse events (all-cause) were headache (17%) and myalgia (3%); only one patient discontinued treatment because of headache, which was considered unrelated to sildenafil. Oral sildenafil significantly improved erectile function and was well tolerated in this trial of Nigerian men suffering from ED. Our results are consistent with reports from other countries.

Delayed primary realignment of posterior urethral rupture.

The treatment of acute posterior urethral rupture is controversial. Twelve patients who presented with acute posterior urethral rupture over a five-year period were treated by delayed primary realignment of the injury. The technique of this procedure and the outcome are the subject of this presentation. Eight patients had successful realignment without strictures. Two patients with strictures responded to simple dilatations with bougies and the other two had formal urethroplasty.

Orbital metastases of prostatic carcinoma in a tropical African population.

We have reviewed the cases of orbital metastases from carcinoma of the prostate gland seen in the University College Hospital, Ibadan over an 11 year period, 1990-2000. During the period, seven cases of orbital metastases that presumably arose from carcinoma of the prostate were seen. Four of the patients died of progression of the primary disease over a variable period ranging between 2 weeks and 30 months of diagnosis and treatment, two were lost to follow up and one is alive 46 months after initial diagnosis of orbital metastases from carcinoma of the prostate and treatment.

Further characterization of storage-related alterations in immunoreactivity of archival tissue sections and its implications for collaborative multicenter immunohistochemical studies.

Storage of unstained paraffin slides may lead to the deterioration of specimens and failure to detect cellular proteins immunohistochemically. Although the implication of age-induced alterations on multicenter immunohistochemical studies would be considerable, they have not been investigated previously. The current study was undertaken to examine the effect of this factor further and to explore new ways of overcoming the resultant shortcomings. The authors now report on the immunodetection of a host of antigens in similarly preserved unstained serial paraffin slides obtained from three centers using a panel of eight antibodies. Staining of recently prepared sections from the authors' centers resulted in similar strong patterns in seven of eight antibodies, with one antibody demonstrating variable immunoreactivity. However, storage of unstained paraffin sections at room temperature resulted in a variable but progressive decrease in expression of several tissue antigens. Although the loss in antigenicity was proportional to the length of storage, the effect was reversible if super antibody concentrations were used. The authors conclude that recently prepared paraffin sections from centers with similar fixation protocols have similar immunoreactivity and are suitable for use in comparative multicenter studies. However, in view of the delays that may attend tissue transportation during these projects, the authors suggest that test systems should be checked for age-induced antigen degradation by incubating sections with higher antibody concentrations.

Fracture of the penis--diagnosis and management.

Fracture of the penis is an uncommon injury, but it occurs most commonly during over enthusiastic sexual intercourse. Diagnosis is not difficult but serious complications such as urethral rupture and corporo-urethral fistula may occur. Management should be by early surgical exploration, with evacuation of the haematoma and repair of the tear in the tunica albuginea in order to minimize the length of patient's stay in hospital and avoid complications such as penile deformity and painful penile erections.

Prostate specific antigen in the Nigerian African.

The biological characteristics of prostate-specific antigen were studied in the Nigerian African. Two hundred and fourteen persons were selected for the study. The group was made up of 59 apparently healthy men (age range 22-76 years), 58 men (age range 40-91 years) who had biopsy proven diagnosis of cancer of the prostate gland, 81 men (age range 46-87 years) who had biopsy proven benign prostatic hypertrophy (BPH) and 16 women (age range 23-47 years). Their median ages were 53 years, 66 years, 66 years and 27 years, respectively. The median PSA value among the control population was 0.7 ug/L (range 0.1-4.3 ug/L) and 98.3% of them had PSA of less than 4 ug/L. No person below 50 years of age had PSA value greater than 2 ug/L. There was a significant correlation between age and serum PSA value over the entire age range (r = 0.523; P = 0.001). In the female study group, 4(25%) had detectable values the cause of which could not be determined. The median PSA value among the BPH patients was 8.5 ug/L (range 0.2-350 ug/L). In this group 37% of them had values below 4 ug/L and 54.3% of them had values less than 10 ug/L. Among the patients who had cancer of the prostate gland the median PSA value was 92.6 ug/L and the mode was greater than 350 ug/L (32% of the patients); 10% and 20% of them had values below 4 ug/L and 10 ug/L respectively. It is concluded that the trend is the same as in the studies and there is the need for prostate gland volume studies and evaluation of women who have detectable PSA values.

Increased incidence of prostate cancer in Nigerians.

An increased incidence of prostate cancer among African-American men (now the second most common cause of cancer death) has been attributed mainly to the introduction of screening techniques, which have enabled earlier diagnosis of patients. This study reviewed male cancer patients recorded in a Nigerian cancer registry to assess the current trends in prostate cancer in Nigeria. For comparison, data were broken into two groups: 1980-1988 and 1989-1996. Only the top 10 cancers occurring in both periods were considered initially in this report. For emphasis, an analysis of adult male cancers was done per decade since 1960. Results show that prostate cancer has become the number one cancer in Nigerian men and constitutes 11% of all male cancers. The median age of patients was 67.5 years (variance 5.6), and the mean age was 71.4 years (variance 14.3). These results indicate that despite the absence of screening programs in Nigeria, the number of prostate cancer cases has increased. The known risk factors probably contribute to a varying degree among Nigerians, who are generally of average build or in the low-normal range for body mass index. Moreover, the role of genetics cannot be underplayed. Given its biological characteristics, more cases of prostate cancer probably would be recorded among this population if screening were undertaken.

Seminoma arising in cryptorchid testes in Nigerian males.

Histological slides of seventy four patients with testicular malignancy found in the records of the cancer registry, University College Hospital, Ibadan over a thirty-five year period (1960-1994) were reviewed. Three (4%) cases occurred in undescended testes. Overall, eighteen (24.3%) patients were found to have seminomas. All the tumours seen in undescended testes (100%) were seminomas while about seventeen percent of seminomas were seen in undescended testes. There was a statistically significant difference between the mean ages of those with seminomas (37.6 years) and those with non-seminomatous testicular cancers (30.3 years); t = 1.69; p < 0.05. The numbers seen corroborate the fact that testicular cancers are not very common in this environment. The incidence of undescended testes is not established in this environment either. However, it seems that 100% of tumours arising in undescended testes are seminomas. It is recommended that abdominal testes seen after puberty should be removed and older patients presenting with abdominal masses should have a differential diagnosis of testicular cancer entertained.

Long-term outcome of posterior urethral valves ablation using the Mohan's urethral valvotome.

CONTEXT: Posterior urethral valves are the most common cause of mechanical infravesical obstruction in children. The disorder has a broad spectrum of severity and the patients could develop complications on the long term, even after valves ablation. OBJECTIVE: To report the result of the follow-up study of 26 children who had posterior urethral valves ablated with the Mohan's urethral valvotome at the University college hospital, Ibadan over a period of five years. STUDY DESIGN SETTING AND SUBJECTS: Children who had posterior urethral valves ablated with the Mohan's urethral valvotome at the UCH, Ibadan between January 1996 and December 2000 were followed up in the out patient clinic on a regular basis. RESULTS: A total of 26 patients with posterior urethral valves had the valves ablated with the Mohan's urethral valvotome during the period of the study. The period of follow-up varied between 18 months and five years. Problems noticed on follow-up include recurrent urinary tract infections in 35% of the cases, acute and chronic renal failure in 5% and 15% respectively, rickets, anaemia, malnutrition and urethral stricture. CONCLUSION: Children who had posterior urethral valves ablated should be followed up much beyond the immediate post valve ablation period as complications could arise on the long term.

Plasma and semen ascorbic levels in spermatogenesis.

BACKGROUND: Conflicting reports on the mechanism of action of ascorbic acid level in male reproductive system exist and very little is known about the ascorbic acid status in Nigerian males with weak fertility. METHOD: Ascorbate that accumulates preferentially in the testis, the lipid and lipoprotein levels were determined in the plasma of Nigerian males. Twenty-seven (27) male with inadequate spermatogenesis (36+/-1.0) years, with mean value of 15.6+/-6.90 million/cm3 sperm count and fourteen (14) controls (34+/-0.6) years, with mean value of 108.0+/-25.42 million/cm3 sperm count were selected for this study. The anthropometric indices were also determined. RESULTS: There were highly significant decreases in sperm cell count, percentage motility and percentage vitality (p<0.001) in each case, while percentage morphologically abnormal sperm cells was significantly elevated (p<0.001) compared with the control values. There were significant decreases in the seminal and plasma ascorbic acid concentrations (p<0.001) in the males who had inadequate spermatogenesis compared with the control values. The plasma total cholesterol (TC) and body mass index (BMI) were not significantly different from the corresponding control values, but the plasma low density lipoprotein (LDLC) (p<0.001) and triglyceride (TG)(p<0.01) concentrations were significantly increased in all the patients. While the plasma high density lipoprotein cholesterol (HDLC)(p<0.001) was significantly decreased compared with the controls. The plasma lipid and lipoprotein levels did not demonstrate any definite pattern with the sperm characteristics. CONCLUSION: The decreased semen ascorbate level may play a significant role in the reduced sperm characteristics in these patients.

Management of ectopia vesica in Ibadan: an 8-year review.

CONTEXT: The management of children with ectopia vesica is intricate and complex. Repair of the bladder soon after delivery is desirable, particularly in our environment as the social stigma associated with such an anomaly can lead to child abandonment or infanticide. OBJECTIVE: To report our experience in the management of children with ectopia vesica at the University College Hospital (UCH), Ibadan, over an 8-year period. STUDY DESIGN, SETTING AND SUBJECTS: All children who presented at the UCH, Ibadan between January 1995 and December 2002 with ectopia vesica had the bladder closed primarily. The children that presented between 1995 and 1998 had the symphysial diastasis approximated with no. 1 nylon suture, while those that presented subsequently had the muscle and fascial layer closed with a darning suture of no. 1 nylon. RESULT: A total of 13 children presented with ectopia vesica during the period of the study. The bladder was closed in all cases. Abdominal wound dehiscence occurred less frequently in patients who had darning suture approximation of the muscle and fascial layer, compared with those who had simple symphysial approximation with nylon suture. CONCLUSION: The result of immediate bladder closure in children with ectopia vesica can be satisfactory in this environment. There is a need for continuing experience in the management of these children to improve outcome.

An unusual cause of bladder rupture in a paraplegic.

Over distention of the bladder or a bladder substitute may lead to rupture. The chances of rapture are more in a previously scarred bladder, particularly in a patient with poor sense of bladder fullness such as paraplegic.

Intravesical Lippes loop following insertion for the treatment of Asherman's syndrome: a case report.

A case report of a 36-year-old Para 6+0 (1 alive) civil servant who developed Asherman's syndrome following repair of ruptured uterus is presented. She had adhesiolysis and insertion of Lippes loop. She defaulted 3 months after presentation and was seen 1 year after with intravesical translocation of the IUCD. This was successfully removed using a forward biting bladder biopsy forceps under direct cystoscopic view.

Combined general and epidural anaesthesia for excision of phaeochromocytoma--a unique and safe technique.

Surgical excision of phaeochromocytoma is usually associated with elevation of arterial blood pressure above 200 mmHg for some time irrespective of the use or not of preoperative alpha-adrenergic blockade. Attempts at controlling the elevated blood pressure usually involve the use of many drugs through induction, maintenance, termination and post operative period. We recently used a combined general and epidural anaesthesia for excision of a huge phaeochromocytoma in a 24 yr old Nigerian lady with a satisfactory outcome. The combined technique seemed to have modified the anaesthetic course considerably, resulting in the use of fewer drugs and a steady haemodynamic state which is very unusual in anaesthesising patients with phaeochromocytoma (see haemodynamic record Illustration II). Post-operative analgesia was provided with morphine given through the epidural catheter.

Priapism: an appraisal of surgical treatment.

The surgical outcome in 66 patients with priapism who presented at the University College Hospital, Ibadan, over a ten-year period was evaluated. Operative procedures carried out included bilateral cavernostomies in 23 patients, caverno-glandular shunts in 11, caverno spongiosal shunt in 18 and caverno-saphenous shunt in 1. Complete detumescence was achieved immediately postoperatively in all patients, however, this was not maintained. Some turgidity recurred after twenty-four hours in all patients. In 12 patients with recurrence of turgidity, 8 had flaccid penis after. Long-term results and follow up in these patients are scanty due to default, but of the 12 that were followed up for a period ranging from 2 months to 2 years, 6 are still able to achieve and maintain an erection, while 6 had no erection at all. Two of five patients who had conservative treatment are able to achieve and maintain an erection. The outcome of surgical treatment appears to be superior to conservative treatment. Bilaterial cavernostomies appear to be effective, but when this fails a shunt procedure should be carried out.
