Vaginal calculi in a young woman.

A case of a large vaginal stone in a 26-year-old woman is presented. Vaginal stones are relatively rare. Various causes can lead to vaginal stone formation. In our case, the calculi was due to vaginal outlet obstruction in childhood. The diagnosis is usually easy to be done. In our patient, X-ray films using frontal and lateral views of the urinary system permitted to establish the right diagnosis. Treatment can vary, depending on the cause of the stone formation and the consistency of the calculi. In this case, the treatment was surgical with simple evolution. It permitted a normal social and sexual life to this young patient.

Treatment of pharmacological priapism with phenylephrine.

Pharmacological prolonged erections or priapism due to anesthesia were treated in 36 patients with intracorporeal injections of phenylephrine. Detumescence was obtained rapidly in every case and no marked side effect was reported. Considering the possible toxicity of other adrenergic agents, phenylephrine is strongly recommended in the treatment of priapism.

[Acute urinary retention secondary to cavernous metastases from a prostatic tumor].

Prostatic carcinoma metastatic to the penis is a rare phenomenon. We report two cases of acute urinary retention due to penile metastases from a prostatic carcinoma. Patients may have a penile mass or diffuse swelling of the penis. Some patients develop priapism and urinary retention can be rarely observed. The most effective treatment seems to be local excision with wide resection and, if necessary, total penectomy. The prognosis remains poor regardless of the type of therapy employed.

[Inverted urothelial papilloma].

Three cases of inverted urothelial papilloma are reported. In one case this lesion, commonly considered as benign, was associated with a transitional cell carcinoma. In an other case inverted papilloma presented histological features of a malignant tumor. The possibility of common etiologic factors of inverted papilloma and other urothelial tumors, explaining their association, is discussed. Considering these results, patients with inverted papilloma must be carefully followed in the same way as patients with low grade urothelial tumors.

Clinical study of Rocephin, a 3d generation cephalosporin, in various septicaemias.

Rocephin (Ro 13-9904, ceftriaxone) is a new injectable 3d generation cephalosporin characterised by a high intrinsic activity, a high resistance to beta-lactamases and a half-life of 8 h which is 4-10 times as long as the half-lives of the other cephalosporins. We administered Rocephin intravenously at a dosage of 2 x 1 g/day in 23 cases of septicaemia confirmed by positive blood cultures. The results achieved were good in 21 cases (cure of the infection episode), fair in 1 case (need to supplement Rocephin with ampicillin) and poor in 1 case of septicaemia due to resistant enterococci. The clinical and biological tolerance was excellent, and administration was particularly easy.
