Papillary squamous cell carcinoma of the uterine cervix: report of three cases and a review of its classification.

Papillary squamous cell carcinoma (SCC) of the uterine cervix has been defined as a malignant squamous cell lesion characterized by a papillary architecture with fibrovascular cores and moderate to severe dysplasia devoid of frank keratinization and koilocytic change. Papillary SCC should be histopathologically delineated from other rare variants of SCC with papillary features including verrucous and condylomatous carcinoma and the recently recognized (squamo-)transitional cell carcinoma of the uterine cervix. We report three cases of papillary SCC (FIGO stages IB, IV, and IVB) in postmenopausal women. Each tumor tested was positive for human papillomavirus (HPV) 16 and negative for HPV 6, 11 and 18 by general primer mediated polymerase chain reaction and subsequent enzyme-linked immunosorbent assay (PCR-ELISA). These findings 1) support the hypothesis that papillary SCCs (unlike verrucous carcinoma) are similar with regard to risk factors to (squamo-)transitional and condylomatous carcinoma; 2) suggest that HPV may play an etiologic role in at least some of these tumors; and 3) suggest that papillary SCC is the only subtype among squamous/(squamo-)transitional carcinomas that is associated with high-risk HPV infection in the absence of HPV-related histopathologic alterations.

[Restrictive dermopathy].

Restrictive dermopathy is a rare, fatal, autosomal recessive, congenital skin disease. Rigidity of translucent thin skin, which is thus highly vulnerable and tears, spontaneously causes intra-uterine fetal akinesia or hypokinesia deformation sequence (FADS), characteristic dysmorphic facies with fixed open mouth in O position, and generalized joint contractures (arthrogryposis). Polyhydramnios and pulmonary hypoplasia are distinctive manifestations, leading to respiratory insufficiency and premature delivery at about 31 weeks of gestation. We report on a case of a prematurely born infant who presented with the typical morphological features and describe the light- and electron-microscopical findings as described in the literature.

[Rare malignant tumors of the ovaries in adolescents--clinical aspects in deciding therapeutic aggressiveness].

4 patients below the age of 20 years have been treated for a malignant tumor of the ovary during the period November 1, 1984 until April 30, 1988. Dysgerminoma was the diagnosis in two cases, as the third patient suffered from a bilateral malignant teratoma. Burkitt's Lymphoma involved both ovaries primarily in an 17-year-old girl. Retrospectively we analyzed diagnosis, therapy and clinical course of these young patients. Regarding the different histological types of the tumors that have been found we discuss critically current recommendations in therapeutic managements referring chemotherapy and/or radiotherapy. Defined conditions provided surgical treatment, that preserves fertility in early stages of malignant germ cell tumors of adolescent women, may be justified, especially for dysgerminomas. A real benefit relate to survival and quality of life by using chemotherapeutic agents can only be expected, if all prognostic factors are regarded.

Inverse ratio of hCG in peritoneal fluid to that in serum in normal and tubal pregnancies.

The ratio of hCG in peritoneal fluid (PF) to that in serum (S) was studied in 60 patients with normal pregnancies (5th-15th wk of gestation = control group) and in 12 tubal pregnancies, 7 tubal abortions (5th-10th wk of amenorrhea) and one case of an early interstitial pregnancy (8th wk). The PF level in the control group was in every patient lower than in S independent of the gestational age. The ratio PF to S ranged from 0.24 to 0.87 (mean +/- S.E.: 0.51 +/- 0.02). In contrast to this, the patients with tubal pregnancy and tubal abortion showed in each case higher PF levels than in S. The ratio in the group with tubal pregnancy ranged from 1.1 to 374 (54 +/- 30) and in the group with tubal abortion from 1.2 to 162 (33 +/- 23). The difference in the ratio between the control group and both tubal pregnancy and tubal abortion was highly significant (P less than 0.001). The ratio of PF to S in the patient with interstitial pregnancy (0.73) did not differ from the control group. At the time of investigation, the S levels in all but 2 patients with ectopic pregnancy were below the range for normal pregnancy of the same gestational age. These findings indicate that the hCG ratio of PF to S may be dependent on the location of the gestational product.
