Breast-feeding during infancy in patients who later develop Crohn's disease.

Breast milk is of importance for protection against infection and for normal development of the intestinal mucosa. This study is a case-control study comparing the length of the breast-feeding period of patients who later in life develop Crohn's disease with matched control individuals. In 308 matched pairs both patient and control were able to produce information concerning the length of their period of breast-feeding. The mean length of the breast-feeding period was 4.59 months among patients and 5.76 months among controls, a significant difference (p less than 0.01). Crohn's disease patients were particularly overrepresented among those with no or very short periods of breast-feeding.

Occurrence and outcome after primary treatment of anal fistulae in Crohn's disease.

During the period 1955 to 1974 inclusive, 826 patients developed Crohn's disease and were registered citizens of Stockholm County at the time of definite diagnosis. These patients were followed up and investigated with reference to the occurrence of and outcome after treatment for anal fistulae. These fistulae were recorded in 184 patients (23%). The incidence of anal fistulae increased the further distally the intestinal lesion was located. Healing followed local operation for anal fistula before curative intestinal resection in approximately 60% of the patients with small intestinal and combined ileocolic disease. However, patients with colonic Crohn's disease did not heal after such treatment. When curative intestinal resection was the primary procedure, 47% of the patients healed spontaneously. Small intestinal and combined ileocolic disease was associated with a good prognosis--90% were healed at follow-up, while patients with colonic disease had a poor prognosis. The combination of rectal Crohn's disease and anal fistula invariably led to proctocolectomy.

Malignant tumours of the colon and rectum in patients aged 30 and younger.

Hospital records of the 27 patients, aged 30 years or younger, with carcinoma of the colon or rectum diagnosed 1969--1970, were reviewed. Predisposing diseases were found in 12 cases, 10 of these did not survive 5 years. Of 15 patients without predisposing disease 9 survived 5 years. The material is small but the prognosis does not seem to be worse in young patients with colonic cancer compared to elderly, provided the tumour has not developed from a predisposing disease. In such cases an early prophylactic operation must always be considered.

Carcinoids of the colon and rectum in patients aged 30 and younger.

Carcinoid tumours of the colon and rectum were diagnosed in 26 patients, 30 years or younger, in Sweden during 1969--1970. Patho-anatomical description was adequate in 24 cases and of these 23 were localized to the appendix and one to the caecum. The treatment was appendicectomy and in the last case ceacal resection. None of the patients had carcinoid syndrom. One patient was lost at follow-up, the 5-year survival rate of the remaining 25 patients was 100 per cent.

The incidence of fecal fistula following exploratory laparotomy and appendicectomy in patients with ileocolic Crohn's disease.

The aim of this study was to investigate if appendicectomy en passant in connection with exploratory laparotomy for ileocolic Crohn's disease increases the risk for development of postoperative fecal fistula. This was studied among 105 patients, 65 of whom had an additional appendicectomy and 40 who had not. The postoperative frequency of fecal fistula was 18.5 versus 15% in the two groups. The difference is not significant. On balance, appendicectomy is advised under these circumstances.

Outcome following treatment of anal fistulae in Crohn's disease.

The results of treating 24 patients with anal fistulae and active intestinal Crohn's disease is reported. Four patients had multiple extended, seven high and ten low fistulae and three patients had sinus tracts. Seven patients with only minor discomfort were treated conservatively and five of them healed. Thirteen were treated by local surgery and ten subsequently healed. Four patients with rectal Crohn's disease and multiple fitulous systems were submitted to abdominoperineal excision. These results indicate that in patients without rectal involvement, conservative treatment and local surgery is indicated.

Anal tuberculosis. A report of two cases.

Two cases of anal tuberculosis are reported. One presented as an ulceration in the anal canal and the other as an anal abscess and fistula. Both were diagnostic problems, tuberculosis was not suspected at first. Both were cured, one by medical treatment, the other by combined surgical and medical treatment.

The incidence of carcinoma in the gastric remnant after resection for benign ulcer disease.

UNLABELLED: During the period 1950-1953, gastric resection was performed on 569 patients for benign ulcer disease at the Serafimer Hospital, Stockholm, Sweden. The purpose of this investigation uas to assess the incidence of cancer and precancerous lesions in the gastric remnant. There were 15 postoperative deaths. 223 patients when traced were dead, and in 130 of these follow-up was adequate. Two cases with carcinoma in the gastric remnant were found among these. 72 patients were lost from follow-up. 253 patients when traced were alive, and in 111 of these, endoscopy and biopsy have been performed. In one patient there was a severe dysplasia but no invasive carcinomas were found. CONCLUSION: 130 dead patients and 111 living patients have been adequately followed after gastric resection for benign ulcer disease. Two carcinomas were found which makes the incidence 0,8%.

Negative findings in search for a transmissible agent in Crohn's disease.

Affected lymph nodes from 3 patients with Crohn's disease were homogenised and inoculated intramurally into the distal ileum of five piglets. The homogenates were also inoculated intramurally in the distal ileum of 15 rats and also by percutaneous injection intraabdominally in 15 rats and compared with the same number of animals of each species inoculated in the same manner with homogenates prepared from 3 patients with caecal cancer. There was no difference in weight increase between the animals inoculated with Crohn's tissue homogenate or control tissue homogenate. Neither did we find any macroscopic or microscopic changes in the animals inoculated with Crohn's tissue homogenate. We found a rather high frequency of mammary tumors in the rats inoculated with Crohn's tissue homogenate, although compared to the rats inoculated with control tissue homogenate there was no statistically significant difference. The negative results may be due to the method of preparing the tissue homogenate or to the use of lymph nodes instead of the bowel in preparing the homogenate, but may also be taken as support against the suggestion that a transmissible agent is present in Crohn's disease.

Contamination of germfree animals with intestinal Crohn tissue. A preliminary report.

The preliminary results of one of a series of prospective studies in the investigation of a possible transmissible agent in Crohn's disease are presented. Filtered human Crohn tissue homogenates was inoculated into eight germfree Sprague-Dawley rats, to test the reproducibility of recent reports suggesting a transmissible agent in regional ileitis. No histological changes were evident after 150 days, and no changes were observed in animals inoculated with non-Crohn tissue homogenates.

Results of the treatment of rectal prolapse operated according to Ripstein.

59 patients have been submitted to abdominal rectopexi according to Ripstein. The operative mortality was 5%, which was, however, not wholly due to the operation. The mortality-rate shows, however, that patients older than 60 years of age must be carefully evaluated preoperatively. The recurrence-rate was 5,4% with a mean observation period of 5 years. The method appears to be satisfactory for treatment of rectal prolapse.

A retrospective study of carcinoma of the pancreas with special reference to the results of surgical treatment.

469 patients treated for carcinoma of the pancreas in the County of Stockholm in 1963--73 were analysed for mean survival time, mean hospital-stay, post-operative complications and mortality. The results of treatment with palliative biliary by-pass procedures and radical surgery were compared. The mean survival time after by-pass operation was 5,7 months and after radical surgery 12,8 months. The mean hospital-stay was 39 and 83 days respectively. The post-operative mortality after palliative biliary by-pass procedures was 16%, and the frequence of major post-operative complications 10%. The corresponding figures after radical surgery were 25% and 30% respectively. Despite the longer survival time, it is doubtful if patients benefit from radical surgery.

Temporary arterio-venous shunts to dilate saphenous crossover graft and maintain graft patency.

A modification of the Palma operation is described in a 25-year-old man with impaired venous outflow of the right leg. After a phlebitic occlusion of the right superficial femoral and external iliac veins he had been operated on twice for varicose veins. The result of these operations was a serious outflow stasis of the right leg during exercise. A saphenous cross-over graft to the right popliteal vein was constructed. Preoperatively a temporary arterio-venous shunt between the left posterior tibial artery and the great saphenous vein had been made in order to increase the diameter of the saphenous vein. Three months later the dilated saphenous vein was resected at the level of the sapheno-tibial artery shunt and anastomosed to the popliteal vein of the right leg. The cross-over graft occluded several times during this operation. A temporary popliteo-popliteal arterio-venous shunt was established distally to the sapheno-popliteal anastomosis to keep the vein graft patent. This second arterio-venous shunt was resected after three months. Venography one month later showed that the vein graft was patent. The patient's complaints had disappeared one month after the operation and a normalization of his venous outflow was recorded plethysmographically. The graft has remained patent during an observation time of eighteen months.
