Are pathologists biased by clinical information?: A blinded cross-over study of the histopathological diagnosis of mesothelial tumours versus pulmonary adenocarcinoma.

In a blinded cross-over design, we studied whether three pathologists were biased by clinical information when making histopathological diagnoses of adenocarcinoma of the lung and benign and malignant mesothelial tumours. Furthermore, the interobserver variation of these diagnoses was assessed. Forty-one cases of adenocarcinoma of the lung and mesothelial tumours were assessed by three pathologists in four rounds. In the first two rounds, slides stained by H&E and clinical information were available. Slides and information were matched so that a specific slide in one round was given clinical information suggesting adenocarcinoma and in the other round, the clinical information suggested mesothelial tumour. In the third and fourth rounds, a panel of immunohistochemical stains was added. The clinical information was matched in the same way as in the first and second rounds. Bias by clinical information was observed when the diagnoses were made on slides stained by H&E, while no bias could be demonstrated when immunohistochemical reactions were included. The reproducibility also improved significantly when these slides were available.

Adenocarcinoma

Melanotic neuroectodermal tumour as a predominant component of an immature testicular teratoma. Case report with immunohistochemical investigations.

A case of melanotic neuroectodermal tumour (MNT), or so-called retinal anlage tumour, as a predominant component of an immature testicular teratoma is presented. The patient was a 17-year-old man who furthermore had a mature mediastinal teratoma. The MNT was composed mainly of two cell types: small immature neuroblast-like cells and large columnar or cuboidal epithelial-like cells with or without melanin granules. The tumour cells were arranged in solid formations, nests, cords, alveolar and pseudoglandular structures with cleft-like or glomeruloid-like spaces. Myogenic differentiation was found in minor foci. Immunohistochemistry showed both neuroepithelial and mesenchymal features with positive staining reaction for neuron-specific enolase (NSE), S-100 protein (S-100), melanoma antigen (HMB45), cytokeratin and vimentin. Vimentin, desmin and actin were present in the myoid cells. To the best of our knowledge this is the first reported case of MNT originating in the testis. As this tumour component occurred in an immature teratoma, neuroectodermal differentiation of germ cell origin is considered most likely.

The muscularis mucosae of the human urinary bladder. Implications for tumor staging on biopsies.

The human urinary bladder wall is traditionally described as being without a muscularis mucosae. A consecutive one year material of 772 bladder biopsies from 171 patients were examined for the proposed presence of lamina muscularis mucosae (MM). MM was observed in 15% of the biopsies and in 35% of the patients and graded into three patterns according to its continuity. Biopsies with transitional cell carcinomas were reviewed in order to find out whether MM-positive biopsies had been staged correctly. The data are discussed in relation to earlier studies on the subject.

Bilateral mixed germ cell sex-cord stroma tumour in a young adult woman. Case report.

The first Scandinavian case of mixed germ cell-sex cord stroma tumour is presented. The tumour occurred in an anatomically and genetically normal young adult woman. The tumour exhibited mainly two histopathological patterns. Larger parts consisted of abundant connective tissue containing cellular nests with numerous germ cells and a few sex cord derivatives. Minor parts showed a tubular pattern composed of tubules of sex cord elements containing scattered germ cells. Immunohistochemical stains for cytokeratin, vimentin, placental alkaline phosphatase and various other substances were all negative. This case is in addition the first reported case where the tumour was bilateral. The second tumour occurred in the contralateral ovary three years after excision of the first one and showed the same histopathological pattern as in the predominant part of the first.

Influence of drainage conditions on mucosal bladder damage by indwelling catheters. II. Histological study.

A previous study demonstrated a strong but short-lasting suction through the catheter eyes by a hydro-dynamically generated negative pressure fluctuation terminating bladder evacuation in some frequently occurring circumstances of indwelling catheter drainage. This report regards the biological effect. Fifteen successive evacuations on such drainage conditions in each of 4 anesthetized pigs were followed by cystectomy and histological examination. All bladders presented small swollen areas, histologically showing localized mucosal elevations dominated by edema of lamina propria and submucosa, occasionally with urothelial thinning or defects. Drainage with suction prevented in 3 animals caused normal bladders. The changes were similar to those following hydro-statical suction and much like those of the "polypoid cystitis" so commonly occurring with indwelling catheters. This suggests both types of suction by ordinary drainage as a major pathogenetic factor in the latter condition. The clinical significance and the occurrence during regimes of straight drainage or intermittent clamping are discussed.

The effect of oral aluminium salts on the bone of non-dialysed uremic patients.

12 patients with conservatively treated uremia were investigated using bone histomorphometry, bone aluminium concentration determination and total body bone mineral content (TBBM). The bone aluminum was raised in 10 patients and was significantly related to oral aluminium salt consumption (p less than 0.01). Two of four patients who had not received aluminium also had raised levels but the difference was not significant from nonuremic patients. The two patients with the highest levels had a mineralisation defect despite normal levels of 1,25-dihydroxyvitamin D. Three patients had significant bone loss of whom one had osteomalacia (OM) while two had secondary hyperparathyroidism (2HP). It is concluded that 1) aluminium salt consumption results in bone aluminium accumulation, and may contribute to the mineralisation defect; 2) uremic patients not treated ith aluminium salts may have slightly raised levels, but this seems not to be clinically important; 3) secondary hyperparathyroidism causes greater destruction of bone mass than other uremic bone diseases; 4) atomic absorption spectrometry is a more sensitive method for detecting aluminium bone deposition than histochemical methods.

Iliac crest biopsy: representativity for the amount of mineralized bone.

The aim of the study was to evaluate the representativity of iliac crest biopsy for the amount of mineralized trabecular and cortical bone in the skeleton. The following data were obtained on bone from 14 necropsies: right sided iliac crest biopsy, lumbar spine biopsy, dry fat free weight of lumbar spine, bone mineral density (BMD) in the lumbar spine and dry fat free weight of cortical and trabecular bone from the left distal forearm. The amount of mineralized cortical and trabecular bone from various sites was compared by linear regression analysis. The results confirm iliac crest biopsy as a good predictor of the amount of trabecular bone, but not of cortical bone. Furthermore, iliac crest biopsy is a better estimate of the amount of trabecular bone in the lumbar spine than spinal BMD.

Iliac crest biopsy: an investigation on certain aspects of precision and accuracy.

We investigated the intraobserver and section to section variation in static and dynamic histomorphometric variables in ten iliac crest biopsies. We also did a comparative study of iliac crest and lumbar spine specimens from 14 cadavers. The ten patients had osteoporosis (spinal crush fractures) and the 14 specimens taken at the necropsies were either normal or osteopenic, without malignant bone disease, bone metastases or metabolic bone disease. The majority of the coefficients of variation (CV%) were between 5 and 15% in the intraobserver analysis and 10 and 30% in the section to section analysis. These results suggest that more sections from each biopsy should be investigated to increase the precision. The correlation of variables from the iliac crest and spine showed poor agreement between resorption and osteoid variables. This fact, combined with a rather low precision of bone dynamic variables in the section to section investigation, raises the question of whether iliac crest biopsy is representative for trabecular bone turnover in the skeleton.

Carcinoma recti. The predictive value of diagnostic biopsies for histologic grading.

The differentiation of 39 diagnostic biopsies, as determined by histologic grading, and that of the corresponding 39 rectal carcinomas were compared. About 60 percent of grade I tumors were diagnosed by grade I biopsies, while about 90 percent of grade II and only about 40 percent of grade III tumors were diagnosed by grade II and grade III biopsies, respectively. The results indicate caution when biopsy grading is used as a guidance to therapy, especially of poorly differentiated tumors.

Necrotizing vasculitis in athymic rats with infarct kidney hypertension.

Infarct kidney hypertension was induced in congenital athymic nude rats and in their haired normal littermates. In both groups a significant and similar elevation of blood pressure was observed. The mesenteric vessels were studied histologically five, 12 and 20 days after operation. Necrotizing vasculitis with and without perivascular inflammatory reactions was found in mesenteric arteries and arterioles in six out of six athymic and in six out of 11 control rats. In sham operated athymic rats and in haired littermates neither hypertension nor vasculitis was observed. These observations indicate that the thymus play no role in the pathogenesis of acute hypertensive vascular disease.

Effect of diazoxide-induced hypotension on cerebral blood flow in hypertensive rats.

The effect on cerebral blood flow of acute diazoxide-induced hypotension was studied in rats with renal and spontaneous hypertension. Diazoxide (5 mg/kg, i.v. bolus), caused arterial pressure to fall rapidly to that of normotensive rats, i.e. c. 75 mmHg. There was a concomitant fall in cerebral blood flow of about 35% (P less than 0.01) in renal hypertensive rats and 25% (P less than 0.05) in spontaneously hypertensive rats; the greater fall in flow in the former corresponded to a greater drop in pressure. Flow remained at these reduced levels during a 2 h observation period. Histological examination revealed small areas of ischaemic damage in the brains of five of the twelve animals. In control hypertensive rats not given diazoxide, cerebral blood flow and blood pressure were stable during a 2 1/2 h period and there was no evidence of ischaemic damage to the brains. The diazoxide-induced reduction in cerebral blood flow was interpreted as being secondary to a blood pressure fall to below the lower limit of cerebral blood flow autoregulation. No evidence was found of direct effects on the cerebral circulation such as seen with ganglionic blockers, alpha-blockers and cerebral vasodilators.

Infarct-kidney hypertension in the rat mutant nude.

Infarct-kidney hypertension was induced in congenital athymic nude rats, and in their immunologically normal haired littermates. In both groups a significant initial increase in blood pressure was seen in the course of the first 30 days. In the remainder of the observation period of 120 days the mean blood pressure in the nude rats decreased to a significantly lower level where-as in the haired rats the mean blood pressure remained unchanged at the high level. Although a high mortality weakened the results, it is hypothesized that the failing ability to maintain the elevated blood pressure into the late phase in the nude rats could be due to impaired thymus function. Nine of 14 haired rats had increased numbers of lymphocytes around intrarenal arteries, in contrast to only 1 of 10 nude rats. A periarteritis nodosa like picture was observed around mesenterial arteries of 3 nude and 2 haired rats. The level of plasma renin was similar preoperatively in nude and haired rats. Infarction of the kidney was followed by a significant decrease in the plasma renin level in both nude and haired rats, which at 10 days was significantly lower in haired than in nude rats, despite a higher blood pressure in the latter.

Cerebral blood flow in rats with renal and spontaneous hypertension: resetting of the lower limit of autoregulation.

The effect of chronic hypertension on cerebral blood flow (CBF) was studied in anaesthetised rats. CBF was measured with the intracarotid 133Xe injection method. Rats with spontaneous and renal hypertension were compared with normotensive controls. The lower limit of autoregulation was determined during controlled haemorrhage. In the normotensive rats, CBF remained constant until mean arterial pressure (MAP) had decreased to the range of 50-69 mm Hg. Thereafter, CBF decreased with each further decrease in MAP. In both types of hypertensive rats, CBF remained constant until MAP had decreased to the range of 70-89 mm Hg. Thus, a 20-mm Hg shift of the lower limit of CBF autoregulation was found in both spontaneous and renal hypertensive rats. A neuropathological study revealed ischaemic brains lesions in half of the hypertensive rats following hypotension, whereas only a single lesion was found in one of six normotensive rats. No ischaemic brain lesions were found in a control study in which CBF was shown to be stable over a 21/2-h period. In conclusion, hypertensive rats showed a shift of the lower limit of CBF autoregulation as well as an increased susceptibility to ischaemic brain damage during hypotension. These findings presumably reflect hypertensive structural changes in the cerebral circulation.

Malignant non-Hodgkin lymphoma of the gastrointestinal tract. A histopathological review of 34 cases.

Thirty-four cases of malignant non-Hodgkin lymphomas of gastro-intestinal origin were histopathologically reviewed employing Rapaport's (18) classification. The majority of the tumours, 25, were localized to the stomach. The rest of the lesions occurred with decreasing frequency throughout the gastro-intestinal tract, with the exception of 3 rectal cases. Thirty lymphomas displayed a purely diffuse pattern, 1 was purely nodular, and 3 were nodular with diffuse components. "Histiocytic" lymphoma was the commonest type, including 16 cases; poorly differentiated lymphocytic type next, with 10 cases. The histopathology of this series does not differ from those of comparable studies.
