[Value of sonomorphologic criteria in the identification of regional metastases of squamous cell cancers of the ENT area].

We compared the preoperative ultrasound findings with the histological results of 127 neck dissections for squamous cell carcinoma of the head and neck. We checked several sonomorphological criteria (size, shape, boundary, echo structure, arrangement, mobility) to assess their value in identification of metastatic disease. If all nodes found on ultrasound were classed as metastases, the specificity was 30%, because many lymph nodes showed nonspecific reaction only. Lymph nodes with a rupture of the capsule or central necrosis or being larger than 3 cm, proved to be metastatic in all cases. Round or oval nodes with a size of more than 2 cm were found to be metastatic with an accuracy of 89%. The almost certain (97%) identification of necks with no metastatic lymph nodes allows elective neck dissection to be avoided.

Detection of antigen-specific antibodies on lung tissue in a patient with hypersensitivity pneumonitis.

A patient with hypersensitivity pneumonitis showed positive Ouchterlony's immunodiffusion tests against pigeon faecal extract, Cephalosporium and Pullularia antigens. Deposits of immunoglobulins-IgG and IgM antibodies-were detected in a subendothelial position in arterial and venous vessels and on alveolar macrophages in the lung tissue. The IgG deposits in blood vessels belonged to IgG1, IgG2 and IgG3 subclasses and the absorbed IgG on alveolar macrophages to all IgG subclasses. The detection of allergen specific antibodies in lung tissue was made by indirect immunofluorescent staining with FITC conjugated antigen extracts from pigeon faeces and demonstrated the aetiology of this hypersensitivity pneumonitis.

Medullary thyroid carcinomas express chromogranin A and a novel neuroendocrine protein recognized by monoclonal antibody HISL-19.

PURPOSE AND METHODS: A number of endocrine peptides and proteins are expressed by medullary thyroid carcinoma (MTC). The expression of two newly appreciated neuroendocrine tumor markers, chromogranin A (CgA) and the endocrine antigen defined by monoclonal antibody HISL-19, was determined in 14 MTCs by immunohistology to evaluate the clinical utility of these markers in the diagnosis of MTC. Papillary, follicular, and undifferentiated thyroid tumors were also evaluated along with an MTC cell line. The same tissues were evaluated with antibodies to human calcitonin. RESULTS: All human calcitonin antibodies were found to react with the MTCs. In addition, all MTCs were reactive for CgA and the antigen detected by antibody HISL-19. CgA was generally present in the human calcitonin-containing cells, whereas the HISL-19 antigen had a more distinctive distribution. The other thyroid tumors failed to show reactivity with any of the three antibodies. CONCLUSION: Our results demonstrate that, in addition to human calcitonin, MTCs commonly express CgA and the antigen defined by antibody HISL-19. Our observations thus add to the repertoire of endocrine substances produced by MTC. These studies also demonstrate the clinical value of immunohistologic procedures for two novel antigens in distinguishing MTCs from other thyroid tumors.

Pulmonary veno-occlusive disease in a patient with unilateral absence of right pulmonary artery.

A 25-year-old patient had unilateral absence of the right pulmonary artery (UARPA) and severe left pulmonary artery hypertension. After death from congestive right heart failure, autopsy revealed histologic signs of pulmonary veno-occlusive disease (PVOD) and pulmonary hypertension (PH). An accessory arterial vessel that was thrombotically occluded was found connecting the ascending aorta and the right pulmonary hilum. There was also histologic evidence of arterial thrombi within the right lung arterial vascular bed. The PH in UARPA usually occurs very early during the course of disease. From histologic findings and medical history, it is likely that in this case, late-onset elevation of pulmonary pressures was triggered by the occurrence of PVOD. This is the first case of UARPA and PVOD--a congenital unilateral arterial malformation in the presence of bilateral involvement in a possibly acquired venous obliterative disease.

Immunochemical characterization of a novel secretory protein (defined by monoclonal antibody HISL-19) of peptide hormone producing cells which is distinct from chromogranin A, B, and C.

In this study we have investigated the biochemical properties as well as the subcellular localization of a 67 kD (35/32 kD dimer) polypeptide detected by a monoclonal antibody (HISL-19), which was generated after immunization of BALB/c mice with human islet cell preparations. This protein is expressed by neuronal and peptide hormone producing cells and shares many biochemical and molecular key features with the chromogranin proteins. As demonstrated by one-dimensional and two-dimensional gel electrophoresis, immunoblotting, monoclonal antibody HISL-19 immunoaffinity chromatography, and immunoelectron microscopy, it is a water-soluble, acidic protein stored within secretory granules of peptide hormone-producing cells, and it is released in detectable amounts into the serum of patients bearing neuroendocrine carcinomas. Differences of the HISL-19 protein and chromogranins A, B, and C are indicated by their different tissue distribution, the discrepancy of their apparent molecular weights in sodium dodecyl sulfate gels and isoelectric points, and by the lack of cross-reactivity of their specific antibodies. The protein detected by monoclonal antibody HISL-19 represents therefore a novel component of the soluble compartments of neurosecretory granules, which is distinct from chromogranin A, B, and C.

[Preoperative radiochemotherapy in advanced oral and oropharyngeal carcinoma].

In a prospective study, 16 patients with advanced carcinoma of oral cavity and oropharynx were submitted to a combined preoperative radio-chemotherapy. The radiosensitizers mitomycin and 5 fluorouracil were given simultaneously with the beginning of radio-therapy. The primary tumor as well as the lymph node regions were exposed to a total dose of 50 Gy administered over five weeks. Eight out of 16 pretreated patients had a complete histological remission, 4/16 a partial remission, and 4/16 showed a tumor reduction of less than 50%. A progression was found in no case. The treatment of lymph node metastases had a slightly poorer effect: CR 7/16, PR 3/16, NC 5/16, PD 0. Therapy effect and side effects as well as the effect on late results of simultaneous radio-chemotherapy are discussed.

[Scalpel and CO2 laser in the animal experiment. A comparative study of inoculation tumors of the mouse].

The study hereby submitted has been designed to show whether there are any differences between using a conventional knife or a CO2-laser when excising and grafting tumors in animal experiments. The animals were C57Bl/6 mice, the tumor was the Lewis lung carcinoma. Eight days after tumor inoculation into the subcutaneous layer of the back, 213 mice were treated by tumor excision either with a conventional knife or a CO2-laser. 86 excised tumors were cut and the tumor surface was swabbed into the subcutaneous layer on the nape of 86 tumor-free mice. Survival times of the laser-operated animals were insignificantly longer. However, small tumors showed markedly longer survival times. The interval without recurrence was longer for all tumor sizes when the laser was used (p less than 0.005). The laser method yielded lower growth rates when the tumor surface was swabbed into tumor-free mice (p less than 0.0001). Histological and cytological tests of the laser-excised and -swabbed specimen demonstrated a high rate of cell destruction. Therefore, the CO2-laser seems to have some importance in cases where the incision is made close to the tumor or where the tumor surface may be lesioned.

Efficacy of gastric emptying: gastric lavage versus emesis induced with ipecac.

A prospective study was done to compare the efficacy of gastric emptying achieved by gastric lavage (GL) with that of ipecac-induced emesis (IE) in victims of drug overdose. Thiamine was used as a marker of recovery in gastric samples, as measured by ion exchange/ion pair high-performance liquid chromatography. There were 51 patients in the IE group and 37 in the GL group. GL produced a higher mean per cent recovery of thiamine than did IE (90% +/- 34% compared with 50% +/- 35%). There was a significant difference between the two groups (P less than .001). Recovery of thiamine was more than 70% in 28% of the IE patients, and exceeded 70% in 75% of GL patients. When gastric emptying is desired for management of the adult overdose victim, the use of GL maximizes the chance of recovering unabsorbed liquid drugs from the stomach.

[Radiotherapy of carcinoma of the paranasal sinuses].

Carcinomas of the paranasal sinuses are usually advanced when diagnosed and present a therapeutic challenge. During the period between February 1970 and June 1981 44 patients were treated. 22 received postoperative irradiation, seven in combination with chemotherapy. 18 patients were treated with radiation alone, eleven with concomitant chemotherapy. Four patients received preoperative irradiation, three in combination with chemotherapy. The three-year survival is 43% and the five-year survival 33%. For those 26 patients who were irradiated pre- or postoperatively with or without concomitant chemotherapy the five-year survival is 45%. We believe the patient will be afforded the greatest opportunity for cure with the combined efforts of the radiotherapist and the surgeon. The combination of chemotherapy and radiotherapy did not provide better results but increased acute and chronic toxicity of the therapy.

Report of two cases of male breast cancer after prolonged estrogen treatment for prostatic carcinoma.

Reviewing our experience with 19 cases of male breast cancer, we found two patients with a 12-year history of estrogen therapy for prostatic carcinoma. One initially received diethylstilbene followed by estradiol, 80 mg monthly, and finally estramustin, 140 mg twice daily. The other patient had dienestrol, 2.5 mg daily, and later estradiol, 40 mg every 6 weeks. The possible causal relationship between prolonged estrogen administration and breast cancer in males is in contrast to available epidemiologic data. Therefore, the possibility of an association by chance cannot be ruled out.

[Value and incidence rate of the inflammation of the bowels in a general children's autopsy material during twenty years (author's transl)].

The acute enteritis in the postneonatal period with or without dyspepsia appeared with an incidence ranging from 18,9% to 27,6% during twenty years period (1956--1975) observations in a general children's autopsy material. Bacteriological and serological analysis may be of success and complete the pathologic-anatomical diagnoses. Infective enteritis often is accompanied by pneumonia and/or otitis media. The acute enteritis can always be detected as an important feature in regard to the cause of death. Most children die within less than twenty-four hours from hospital admission. The hemorrhagic, ulcerous and necrotic form of the enterocolitis predominate the perinatal and neonatal period, frequently combined with peritonitis and bowel perforation. Bacteriological examinations should be obtained. This form of the acute enteritis and enterocolitis of the newborns must be taken into consideration of the ileus symptoms differential diagnosis.
