Sympathoadrenal system under conditions of increased left and right ventricular afterload.

Adrenergic plexuses in the myocardium and adrenal medulla were studied histochemically under conditions of increased left or right ventricular afterload. Under conditions of high afterload not accompanied by heart failure the density of sympathetic myocardial innervation remained unchanged in the loaded ventricle, but increased in the intact ventricle. Comparison of the state of the sympathoadrenal system under conditions of increased afterload complicated or uncomplicated by heart failure revealed common prognostically unfavorable changes: sharp decrease in the density of adrenergic nerve plexuses in the ventricular myocardium and activation of adrenal chromaffin cells.

Comparative pathomorphological study of contractile myocardium under conditions of increased left and right ventricular afterload.

Structural changes in the myocardium under conditions of increased left and right ventricular afterload were studied using polarization microscopy and histological, histochemical, and stereological methods. Increased afterload not complicated by heart failure was characterized by low number of damaged cardiomyocytes (3.3-6.5%) and moderate structural changes in the ventricular myocardium (contractures of different severity). Increased afterload complicated by heart failure was characterized by high ratio of damaged cardiomyocytes (5.6-19.2%) and severe reversible (grade I and II contractures) and irreversible (grade III contractures and lump degradation of myofibrils) structural changes. Irreversible damage to most cardiomyocytes included plasmatic impregnation, which was most pronounced in the subendocardial layer of ventricles operating under conditions of increased afterload. Comparative study showed that increased left and right ventricular afterload induces similar pathomorphological changes in the contractile myocardium. Our results indicate that increased afterload to the right or left ventricle is accompanied by the development of stereotypical structural changes in the myocardium. Profound and severe disturbances can cause heart failure.

Structural and metabolic characteristics of liver acini in dogs with acute cholecystitis.

Structural and metabolic changes in liver tissue in dogs with acute cholecystitis were studied by histological, histoenzymatic, and biochemical methods. It was found that the development of destructive inflammatory processes in the gallbladder wall induces morphological damage to the liver tissue, which correlates with metabolic disorders in this organ and leads to liver dysfunction.

[The problem of postoperative venous thromboembolic complications in general surgery].

The paper reviews occurrence, diagnostic tools venous thromboembolic complications. Cost effectiveness of preventive measures for venous thrombosis and pulmonary embolism in general surgery is analysed. The study included 45 patients who underwent elective (21) and urgent (24) abdominal and pelvic interventions. Each patient was examined with ultrasonic with color Doppler imaging before and 8-10 days after surgery. Deep venous thrombosis of lower limbs was diagnosed in 11.1%, most of cases being asymptomatic. Ultrasonic angioscanning appeared to be an acute and reliable diagnostic tool for this pathology. Various approaches to the management of patients at thrombotic risk are reviewed. Cost effectiveness analysis of anticoagulation prevention for postoperative venous thromboembolic complications must address the problem: is prevention outcome worth those extra efforts? Surveillance of 500 patients at high thrombotic risk has shown that preventive use of low weight heparin (Clexane) is economically beneficial then conventional Heparin, the more in the absence of special preventive strategy. Targeted prevention in patients with different risk of postoperative venous thromboembolic complications can yield more then 3-fold reduction in occurrence of these complications.

[Morphofunctional characteristics of erythrocytes in chronic kidney failure and purulent intoxication].

The morphology of blood erythrocytes was examined under a scanning electron microscope and erythrocyte surface charge, lipid peroxidation on erythrocyte membranes, and blood viscosity were studied in 213 urological patients with chronic renal insufficiency (CRI) and purulent intoxication (PI) of different severity. CRI and PI were characterized by morphofunctional changes in erythrocytes, which depended on the type and severity of intoxication. Changes in erythrocyte morphology in CRI and PI were paralleled by a decrease in their negative surface charge and activation of lipid peroxidation. These parameters can serve as diagnostic tests for evaluation of the severity of intoxication.

[Morphofunctional characteristics of erythrocytes in extracorporeal efferent detoxication].

The erythrocyte morphology (scanning electronic microscope, the superficial erythrocyte charge, the lipid peroxidation rate and the blood viscosity were investigated in 78 patients, treated by use of hemodyalisis, hemosorbtion, plasmosorbtion and hyperbaric oxygenation to correct the chronic renal insufficiency and the purulent intoxication. The described erythrocyte changes reflect the features of definite kind of the efferent treatment and the program of its use, the individuality of the patient initial state. The efferent methods of treatment cause significant and mostly differently directed changes in the peripheral red blood. Morphologic and functional parameters of the erythrocytes appear to be important diagnostic criteria of the efficiency of the extracorporal methods of detoxication.

[Status of the myocardial ventricles in thromboembolism of the pulmonary arteries and their branches (based on early autopsy material)].

Ventricle myocardium of 16 patients who had died from acute thromboembolism (TE) of the pulmonary arteries was studied. The most important factor in the development of myocardial alterations is the degree of a vascular occlusion by the thrombus. Chronic lung and heart diseases as well as patient age may considerably aggravate the structural changes of the cardiomyocytes. Relatively low activity of catabolic enzymes in the myocardium of patients with TE correlates with a small size of embolic damage, the presence of chronic lung and heart diseases and with advanced age.
