[Cerebral ischemic strokes in young patients with neurosyphilis].

In split of a dramatic increase of syphilis incidence over the last time, neurosyphilis cases are reported relatively rare. The data on ischemic stroke of syphilis origin in 5 patients (3 males and 2 females, aged 35-43 years) are presented. A diagnosis was verified on the basis of clinical and MRT data and the results of blood and spinal fluid serological reactions. The patients had mild and moderate headache (5), movement (5) and sensitivity (3) disorders, membrane syndrome (2), hemianopsia (2), coma (1). Because of the absence of anamnesis data on syphilis survived, recognition of stroke origin was delayed and established only after positive serum and liquor tests. One patient died of multifocal brain lesion, 4 patients discharged from the hospital had differently pronounced movement disorders.

[Individual prognosis of the risk of primary and secondary female infertility].

The authors suggest methodologic approaches to individual prediction of the risk of primary and secondary female sterility. A total of 512 women suffering from infertility were examined, and the major biomedical and ++socio-hygienic risk factors were detected. ++Socio-hygienic factors were found conducive to the development of biomedical factors or augmenting their effects. The Bayes' probability method and Wald's consecutive analysis underlie the developed system for prediction. Use of the prediction tables with due consideration for the results of medical examination of the patients helps more precisely define the group for prophylactic follow-up and give individual recommendations for medicosocial counselling.

[Approaches to formation of a territorial logopedic service].

The incidence of speech disorders in children and adolescents of the Ryazan region in 1993-1998 is analyzed. Speech abnormalities became highly prevalent among children in recent years, particularly among preschool children, and hence specialized service for subjects with speech disorders is to be formed, which should pay the priority attention to children. Approaches to creation of logopedic service of a territory are outlined.

[Risk factors for sexually transmitted infections in adolescents].

The prevalence of sexually transmitted infections (STI) among young people in Russia is examined for the recent decade. Risk factors and organization of STI patients detection, the knowledge of young people about STI are analyzed. Approaches to prevention of these diseases in the society are outlined.

[Forecasting of prevalence of cardio-vascular pathology in children population].

Statistical modeling and forecasting of morbidity, disability and mortality of circulatory system diseases and congenital anomalies in children of Ryazan oblast was organized. Procedures of expotential smoothing and determination of statistical significance power of predicted values were applied. Application of expotential smoothing models with linear trend enables reliable forecasting of morbidity of circulatory system diseases and congenital anomalies in children. In Ryazan oblast, disability and mortality of children has higher variance which prevent to apply reliable forecasting. Increase in morbidity and disability rate of circulatory system diseases and congenital anomalies and widening of dispensary group is detected. Predicted increasing of morbidity rate of cardiovascular pathology in children and widening of corresponding dispensary group requires enhanced capacity of medical care supported by according financing.

[Cost availability and patient satisfaction of hospital care].

The results of sociological questionnaire of 1496 patients of oblast and region hospitals related to cost availability and patient satisfaction of hospital care. About 75% of respondents considered their treatment spendings as significant. It is established that degree of patients' satisfaction of hospital care depends on length of stay at the in-patient department. The longer it is the fewer patients satisfied with medical care are. It is proposed to organize in hospitals among socially unprotected patients evaluation of individual spendings for medical care to protect constitutional rights of the population and to reveal factual cost of medical care in the framework of the territorial program of public guarantees.

[Medico-social aspects of sexual and reproductive behavior of young people].

Discusses the negative aftereffects of sexual and reproductive behavior of modern young people. Special attention is paid to medicosocial aspects, such as increased incidence of sexually-transmitted diseases, undesired pregnancies, induced abortions and deliveries in adolescents. The effect of natural birth on the health status and development of a child is demonstrated. A conclusion is made on the necessity of more intensive work aimed at preparing young people to marriage and childbirth.

[Social-hygienic and medico-biological risk factors in spontaneous abortion and infertility].

Statistically significant socio-hygienic and biomedical factors have a far greater impact on miscarriages than on primary and secondary infertility. A conclusion is made that during gestation the organism reacts to a variety of exposures, whereas infertility is a grave disorder of reproductive health which develops as a result of combined exposure to the most potent endogenous and exogenous risk factors. Miscarriages in the workers occupationally exposed to hazardous factors later often transform into stable infertility.

[Characterization of medical activities of urban families in raising a healthy child].

Ten statistically significant factors of risk of frequent diseases of children have been distinguished; four of them directly reflect medical activity of parents: late application for medical care when a child falls ill, neglect of prophylactic visits to doctors, neglect of child's health fortification, and refusal to take a sick leave when the child is ill. Poor medical activity of families as far as it concerns the upbringing of a healthy child results in a higher insurance risk, which should be taken into consideration when developing legal and economic aspects of medical insurance.

[Priority trends in the prevention of reproductive health disorders].

Common biomedical and sociohygienic risk factors leading to disorders of the reproductive health indicate that universal approaches to their prevention are to be developed. The author discusses primary (mainly social), secondary (sociomedical), and tertiary (mainly medical) prevention of the above conditions. Realization of these measures aimed at preservation and restoration of the reproductive health of a family is intended to solve the problem of proper reproduction of healthy generations.
