Large bowel volvulus in children. Review of a case material and the literature.

Four pediatric patients with volvulus of the right colon, and three others with twisting of the sigmoid colon are reported. The clinical manifestations, mainly pain, were vague. Radiological plain film findings were occasionally characteristic, but were in the majority of cases equivocal. An active radiological approach including a supplementary barium enema in any patient with inconclusive plain abdominal roentgenograms will, in addition to other unexpected diagnoses, reveal rare cases of large bowel volvulus. Elective surgical procedures, carried out on viable bowel do not necessarily provide advantages over a non-operative approach. In addition spontaneous reduction of the volvulus may result in permanent cure.

Localized intra- and retroperitoneal gas collections following intraoperative use of surgical gelatine sponge.

Following recent intra- or retroperitoneal surgery, four children in whom hemostasis was obtained by means of abundant use of surgical gelatine sponge, displayed unusual bubbly air collections in the region of the operation. Three of them had vague inflammatory symptoms and signs, while in the fourth the gas was discovered incidentally. The connection between the radiological finding and the hemostatic agent is presented. In the typical case manifesting a mild clinical course, knowledge of this association helps to guard against the diagnosis of abscess, and subsequent unnecessary reoperation can be avoided.

Segmental vascular occlusion of the colon. A tentative diagnosis in two pediatric cases.

Segmental vascular occlusion of the colon is a rare, quite commonly reversible condition, so far recorded only in adults. The present report deals with two pediatric patients displaying clinical and radiological findings consistent with this entity. A benign course of disease with rapidly subsiding manifestation support our interpretation. The etiology remains obscure.

Malignant versus benign paravertebral widening in children.

Dorso-medial pleural reflection line anomalies occur in malignant tumours, neuroblastoma being the commonest one, in benign tumours, spinal inflammatory disorders and in cases of pseudotumours of different origin. The latter group comprises normal structure variants, malformations and acquired disease but also a small number of transitory or permanent lesions of uncertain or obscure origin. The radiological manifestations encountered are displacement, obliteration or blurring of the paraspinal contour, sometimes a frank paravertebral mass. Based on a case material consisting of 65 patients the importance of recognizing this feature is stressed. An approach to clinical and radiological diagnosis is discussed.

Reliability of the abdominal plain film diagnosis in pediatric patients with suspected intussusception.

In order to reassess the diagnostic reliability of the abdominal plain film examination in suspected intussusception, the findings in 100 consecutive cases of this disorder were analysed. Then, these were compared with the same number of cases in which the diagnosis had been rejected by means of barium enema. It was found that a positive plain film diagnosis of intussusception was possible in 89 patients. In 11 cases inconclusive plain film findings called for supplementary barium enemas to establish the diagnosis. In the reference group intussusception was excluded on the basis of plain film findings alone in 74% of cases. In the remaining 26% of patients a barium enema proved necessary to reject the diagnosis of suspected intussusception.

Ring chromosome 14 in a mentally retarded girl.

A case of ring chromosome 14 in a 12-year-old girl, showing mental retardation, epilepsy and minor somatic abnormalities, is described and compared with five previously reported cases with the same chromosome aberration.

Micturition cysto-urethrography. Potential diagnostic pitfalls in the male infant.

Some technical aspects of micturition cysto-urethrography, particularly important in the male infant, are emphasized. The catheter used for installation of the contrast medium must be removed before the examination. Unless films are exposed in the true lateral projection and the patient voids with a constant stream, abnormalities of the urethra may easily become overlooked.

Ectopic ureterocele in the male infant.

An account is given of a series of ectopic ureterocele present in 14 male infants. The malformation is found to be more complex than in the female. The ipsilateral renal function is severely impaired or abolished and obstruction to the bladder outflow common. Associated dilatation and elongation of the posterior urethra during micturition may result in a valvelike appearance. Eversion of the male ureterocele is common and possible mechanisms to account for this event are discussed.

Scanty intestinal gas in infants and children. Clinical significance and radiologic diagnosis.

A material of 23 boys and 18 girls attending as abdominal emergencies and covering the age interval of neonates to 15 years of age had only scanty amounts of intestinal gas on films of the abdomen. A wide variety of underlying abnormalities were revealed, some of them calling for immediate operation. Supplementary contrast examinations were frequently needed in order to establish the diagnosis.

Recurrent intussusception. Analysis of a series treated with hydrostatic reduction.

The appearances at conventional radiography and barium enema were compared in a series of 35 patients with recurrent intussusception and in the same number of cases with an uncomplicated course. Only minor disparities between the groups could be established. The significance of a sparse amount of intestinal gas pointing to intussusception and the implication of a complete evacuation of the bowel following barium enema are stressed. The latter phenomenon, occasionally combined with persistent mild small bowel meteorism, occurred in approximately one-third of all early relapses. Patients with one manifest relapse run a significantly increased risk of further recurrences although in the majority of cases no significant etiologic factors are encountered.

Radiologic aspects of BCG-osteomyelitis in infants and children.

An account is given of results of the radiologic examination of 29 infants and children with bone tuberculosis caused by the Bacilli Calmette-Guérin (BCG) as a complication to intradermal vaccination. The diagnosis is based on radiologic appearances, microscopy and bacteriologic examination of specimens obtained from the bone lesions. The radiologic appearance of the bone lesions is in most cases characteristic and differs decisively from that of pyogenic osteomyelitis and malignant disease.

Kidney size in children. A method of assessment.

A method for assessment of kidney size in pediatric practice is presented. Length of the kidney is correlated to length of the lumbar segment L1 to L3 including the intervertebral spaces comprised by these vertebrae. The result may either be expressed in standard deviations or as a kidney ratio. The method makes comparison with films from previous examinations or other hospitals feasible without access to any other data than those available in the film. The method also serves the purpose of properly assessing the growth of the kidneys.
