Intrauterine exposure to the beta-adrenergic receptor-blocking agent metoprolol and allergy.

Cord blood IgE levels and the development of allergy were studied in 29 children born by mothers who had been treated with the beta-adrenergic receptor blocking agent, metoprolol, during the pregnancy and in 23 children of placebo-treated mothers. In 13 (45%) of the former and 3 (15%) of the placebo group elevated cord blood IgE levels, i.e. more than 0.9 kU/l and/or obvious or probable allergy developed (p = 0.03). The findings support previous experimental observations that beta-blocking agents may enhance IgE antibody formation. It also adds to the number of environmental factors that may increase the risk for atopic disease in children.

CKBB-enzyme activity in serum in neonates born after vaginal delivery and cesarean section.

Creatine kinaseBB has been analysed in blood from the umbilical cord and from venous blood within 24 hours after birth in infants without any signs of perinatal asphyxia. S-CKBB activity was found to be much higher than in adults and older children. 25% of the children born after normal vaginal delivery showed very high S-CKBB activities, higher than any of the children born by elective cesarean section. There were, however, no signs of neurological disturbances to be found in any of these children with high neonatal CKBB activities at follow-up after four years.

Neonatal reference values for ionized calcium, phosphate and magnesium. Selection of reference population by optimality criteria.

Thirty-one full-term newborn babies were investigated in order to establish reference values for ionized calcium. Only children fulfilling certain optimality criteria (with best possible maternal and infant conditions and uncomplicated pregnancy and delivery) were included. All infants were breast fed. Capillary blood for analysis of ionized calcium was collected by heel puncture on day 1 (6-36 h post partum, p.p.), day 3 (60-84 h p.p.) and day 5 (108-132 h p.p.). Ionized calcium was measured with a semi-automatic electrode system ICA 1 (Radiometer A/S, Copenhagen, Denmark). The reference ranges (mean +/- 2 SD) for days 1, 3 and 5 were 1.05-1.37, 1.10-1.42 and 1.20-1.48 mmol/l, respectively. The mean ionized calcium concentration on day 1 was significantly lower than on days 3 and 5. Reference values are also given for total calcium, magnesium and phosphate. We emphasize that it is impossible to calculate ionized calcium from total calcium or vice versa.

Neonatal hyperexcitability in relation to plasma ionized calcium, magnesium, phosphate and glucose.

Twenty-two full-term babies were examined with a quantitative neurological assessment on days 1, 2 and 5 in order to confirm the clinical impression of hyperexcitability observed at the routine examination on day 1. Ultrasound examination of the head was normal in all cases. Plasma ionized calcium, phosphate and glucose in the hyperexcitability group fell within the neonatal reference ranges, as described previously. However, serum magnesium was significantly lower on day 1, but spontaneously normalized on day 5. Routine check-ups at the well-baby-clinics did not reveal any major illnesses or developmental abnormalities.

Forceps or vacuum extraction? A comparison of effects on the newborn infant.

Sixteen women delivered by forceps and 20 women delivered by vacuum extraction (VE) owing to secondary uterine inertia were compared with a control group of 11 women who gave birth spontaneously. The cord arterial pH was lower in the VE group than in the forceps and control groups. Base deficit in both arterial and venous cord blood was greater in the VE group than in the forceps group, probably owing to the longer application and extraction times in VE than in forceps delivery. The incidence of retinal hemorrhage did not differ between the forceps and VE groups. The incidence of cephalhematomata was greater among VE infants than in the forceps and control groups. All infants were examined on the 1st and 5th day by standardized neurological and behavioural examination. There were no significant differences in neurological status between the forceps and VE groups. Thus, in low extraction with no signs of fetal asphyxia, either method can be used with safety if the obstetrician is familiar with both methods of operative vaginal delivery.

Metabolic effects of a human milk adapted formula on sulfur amino acid degradation in full-term infants.

The metabolic effects of formula feeding on sulfur amino acid degradation were studied in 6 full-term infants on the 6th day of life, and compared to corresponding data from breast-milk fed neonates. A normal excretion of total sulfur and inorganic sulfate was found, indicating an adequate intake of sulfur amino acids and a normal ability to oxidized these compounds to inorganic sulfate. A similar transaminative degradation of cysteine was observed in both groups. Formula-fed infants showed a normal output of methionine, whereas increased cystathionine and decreased taurine excretions were registered. The results were probably a combined effect of a limited intake of taurine, and a multiple immature enzymatic activity.

Neonatal effects of beta-blocking drugs in pregnancy.

A review is presented as regards the neonatal effects of beta-blocking agents in hypertension during pregnancy. Experience points to low fetal and neonatal risks, but firmly based data concerning the neonates are very scanty. An on-going double-blind study is presented in which pregnant women with hypertension are randomly allocated to either metoprolol or placebo treatment. One of the aims is to study the postnatal adaptation of the infants. The following measurements, recordings and examinations are performed: anthropometric measurements, Apgar score, blood gases and acid-base status in umbilical (venous and arterial) blood, lactate and hypoxanthine in umbilical venous blood, catecholamines in umbilical arterial blood, pulse rate, blood glucose and a quantitative neurobehavioral assessment in the neonatal period.

The excretion of sulfur compounds in the urine of newborn infants.

The excretion of sulfur-containing compounds was determined on the third and sixth day of life in male infants and the results were compared with those previously obtained on fed and fasting men. The output of total sulfur and inorganic sulfate was very low on the third day of life but had increased by the sixth day to levels found in the fasting men, whereas the excretion of mercaptolactate in the newborns decreased from the third to the sixth day of life. These results may be explained by the initial fasting state of neonates followed by an anabolic phase. Neonates excreted acid-labile ester sulfate and mercaptoacetate at levels similar to those found in adults, but the neonatal urine also contained sulfate esters (probably steroid sulfates) that required more drastic acid conditions for hydrolysis. Raised concentrations of sulfur-containing amino acids (methionine, cystathionine, cyst(e)ine and taurine) were found in neonatal urine in confirmation of earlier reports. The excretion of thiosulfate could only be determined in newborns on the sixth day and was low in comparison with that of adults. High urinary thiocyanate concentrations were found in newborns fed by tobacco-smoking mothers, whereas the excretion of thiocyanate was low in other newborns. The possible medical hazards from the exposure of neonates to thiocyanate are emphasized.

Triplet pregnancies in Linköping, Sweden, 1973-1981.

A survey of triplet pregnancies in Linköping, Sweden, 1973-1981, showed a low perinatal mortality (4%) and low morbidity possibly due to prophylactic bed rest and tocolytic treatment. All children were followed at child health clinics. In one child, a persistent growth retardation was recorded at 4 years of age. No psychomotor deviations were found in the group. An interview concerning social and economic factors after delivery was carried out. The answers revealed considerable strain in daily life for all families.

Evaluation of radiometer ICA1 as a routine instrument for serum ionized calcium and its application for whole blood capillary samples from newborn infants.

The Radiometer ICA1 was evaluated with four sets of samples: a) 289 serum samples sent for routine analysis of ionized calcium, b) a reference set of serum samples from 40 healthy laboratory workers, c) 42 capillary whole blood samples from healthy full-term and d) six capillary whole blood samples from pre-term infants. For comparison, all sets except the capillary samples were also run on the Orion SS-20. The ICA1 gave higher readings than the Orion SS-20 for normo-(mean difference 0.011 mmol/l) and hypocalcaemic (mean difference 0.037 mmol/l), but not for hypercalcaemic serum samples. The mean +/- 2 SD range for the serum reference set was 1.15-1.31 mmol/l for ICA1 and 1.14-1.27 mmol/l for Orion SS-20. The within-assay and between-assay precision were 0.014 and 0.032 mmol/l (SD) respectively for ICA1 and 0.010 and 0.022 mmol/l (SD) for Orion SS-20. The 42 healthy full-term infants had a mean +/- 2 SD range of 1.18-1.47 mmol/l. The six pre-term infants had whole blood ionized calcium 0.82-1.07 mmol/l. Three of them had ionized calcium less than 0.90 mmol/l and a symptomatic hypocalcemia.

Correlation between neurological examination and behavioural assessment of the newborn infant.

A modified Prechtl neurological examination and the Brazelton Neonatal Behavioural Assessment Scale (BNBAS) were used on days 1 and 5 in 78 healthy newborn infants. Correlations between variables in the neurological examination and the behavioural items and subscales in the BNBAS were calculated. Especially the total neurological score and optimality score were significantly correlated to motor performance (days 1 and 5) and to orienting responses to animate and inanimate stimuli (day 1) in the BNBAS. The neurological variables showed poor or no correlation to the habituation items and to certain items concerned with changes of behavioural state in the BNBAS.

Studies on the Brazelton neonatal behavioural assessment scale.

The Brazelton neonatal behavioural assessment scale was applied in 120 healthy full-time newborn infants. Interobserver reliability on day 5 (n = 20), test-retest reliability on day 4-5 (n = 20), and correlations between examinations on day 1 and day 5 (n = 80) were studied with regard to individual and grouped items (subscale) in the test. The over-all interobserver reliability generally seemed to be satisfactory, but low estimates of test-retest reliability were obtained for many items. Items assessing orientation capacity and regulation of state showed better test-retest stability when the predominate states were identical on both occasions. Some subscales were significantly related to parity, sex, birthweight, and age in hours at examination.

The immotile-cilia syndrome in a newborn infant.

A newborn infant was suspected to have the immotile-cilia syndrome. He had situs inversus, atelectasis and other pulmonary changes. A biopsy from the nasal mucosa at the age of 21 months confirmed the diagnosis and showed a lack of ciliary orientation and absent dynein arms. Chymotrypsin values, sweat test and most immunoglobulins were normal. The boy is now three years old. So far he has had rather mild respiratory symptoms. The importance of early diagnosis for management and prognosis is stressed.

Neurology and behaviour of growth-retarded neonates. Relation to biochemical placental function tests in late pregnancy.

Serial estimations of maternal urinary oestriol, serum cystine aminopeptidase (S-CAP), and human chorionic somatomammotrophin (S-HCS) were done in a prospective study on 29 pregnancies in which intrauterine growth retardation was diagnosed in the third trimester by the gravidogram method and/or serial ultrasound measurements of the fetal biparietal diameter. The series was divided into 2 growth-retarded groups: (i) severe growth retardation with birth weight less than -2 SD from the mean for gestational age (N = 14); (ii) moderate growth retardation with birth weight between -1 and -2 SD from the mean for gestational age (N = 15). These were compared with a control group of 18 normal pregnancies and infants. A modified Prechtl neurological examination and the Brazelton Neonatal Behavioural Assessment Scale (NBAS) were done in the neonatal period at full-term age. Both categories of growth-retarded infants showed lower muscle tonus than the controls. The severely growth-retarded infants showed fewer optimal items in the neurological examination; they also showed poorer capacity for orientation to external stimuli, inferior motor function, and less physiological stability in NBAS than the controls. The abnormal biochemical placental tests were significantly correlated to low Apgar scores (urinary oestriol), to low excitability (S-CAP) and to poor motor function (S-HCS). The neurological and behavioural condition of the neonate seemed to be more closely associated to the extent of growth retardation than to the occurrence of abnormal biochemical placental tests.

Spontaneous labor and elective induction--a prospective randomized study. II. Bilirubin levels in the neonatal period.

The effect of induction of labor on the neonatal bilirubin levels was investigated in a prospective randomized study. Forty-three infants born after elective induction with oxytocin using the Cardiff infusion system and 38 infants born after spontaneous delivery were studied. Several factors other than oxytocin administration which could influence the bilirubin levels such as gestational age, placental transfusion and feeding routines did not differ between the two groups. There were no differences with regard to pH in umbilical vein. Apgar scores, haemoglobin and erythrocyte volume fractions of bilirubin levels neonatally. Thus there was no evidence of an association between induction of labor with oxytocin and jaundice during the neonatal period.
