Anti-CD4 monoclonal antibody treatment in acute and early chronic antigen-induced arthritis: influence on T helper cell activation.

To examine the effects of anti-CD4 mAb treatment in acute and chronic antigen-induced arthritis (AIA), C57BL/6 mice were treated intraperitoneally either with the depleting anti-CD4 mAb GK1.5 or with rat-IgG (control) on Days -1, 0, 1, 3, 5, and 7. Arthritis was monitored by assessment of joint swelling and histological evaluation in the acute (Day 3) and the chronic phase (Day 21) of AIA. To determine the effects on cellular immune responses, in vivo T-cell reactivity (delayed type hypersensitivity; DTH) was measured, as well as protein levels of TH1- (IL-2, IFN-gamma) and TH2-cytokines (IL-4, IL-10) in joint extracts and supernatants of ex vivo stimulated spleen and lymph node cells. The humoral immune response was analysed by measuring serum antibodies against methylated bovine serum albumine (mBSA) and extracellular matrix proteins. Treatment with GK1.5 reduced swelling, inflammation, and destruction of the arthritic joint. Unexpectedly, the effects were even more pronounced in the acute than in the chronic phase. The anti-inflammatory effect was accompanied by a diminished DTH against the arthritogen mBSA and a decrease of TH1-cytokine production in spleen and pooled body lymph nodes, whereas the TH2-cytokine production in these organs was unchanged and the humoral immune response was only moderately reduced. There was a failure of depleting CD4+ T-cells in the joint, reflected also by unchanged local cytokine levels. Therefore, systemic rather than local effects on the TH1/TH2 balance appear to underlie the therapeutic efficacy of anti-CD4 treatment in AIA.

Creating a reliable bond. An all-in-one system.

This paper explains the reasons why a new adhesion concept was developed, how it was conceived and what properties and test results were achieved with the product Prompt L-Pop that follows this new concept. Therefore, a short historical introduction into the development of adhesive materials is given followed by a summary of today's state-of-the-art techniques. The requirements for the new adhesion concept are pointed out and the outcome of the effort in the development is explained in more detail. In vitro and in vivo test results of Prompt L-Pop support the new concept by revealing excellent bond strength values in combination with good marginal adaptation and a significantly reduced post operative sensitivity. Finally, further developments regarding the new concept are discussed, such as the use of Prompt L-Pop in combination with plasma curing lights or indirect restorations.

Ambulatory blood pressure monitoring in healthy schoolchildren.

Ambulatory blood pressure monitoring (ABPM) was performed in 564 healthy schoolchildren during normal circadian activities. The data of two cohorts (155 boys and 139 girls aged 9-13 years and 184 boys and 168 girls with a body height between 120 and 155 cm) are presented. From the age of 9 to 13 years the mean 24-h systolic/diastolic blood pressure (SBP/DBP) increases from 107 +/- 9/66 +/- 7 mmHg to 115 +/- 13/68 +/- 9 mmHg in boys and from 104 +/- 5/64 +/- 6 mmHg to 109 +/- 8/65 +/- 9 mmHg in girls. When related to body height the values rise from 105 +/- 6/64 +/- 6 mmHg at 120 cm to 113 +/- 8/67 +/- 7 mmHg at 155 cm in boys and from 100 +/- 7/65 +/- 7 mmHg to 112 +/- 9/66 +/- 9 mmHg in girls. In comparison with the causal blood pressure data obtained from European studies, the presented ABPM values (daytime BP) are higher throughout, which may be explained by the increased activity during daytime with ABPM. There is a mean difference of 4.4 mmHg in boys and of 3.0 mmHg in girls for SBP and of 10.8 mmHg in boys and of 9.0 mmHg in girls for DBP when related to age. In relation to body height, there is a mean difference of 4.4 mmHg in boys and of 3.5 mmHg in girls for SBP and of 10.9 mmHg in boys and of 10.5 mmHg in girls for DBP. We conclude that standards derived from causal blood pressure measurements should not be used for the evaluation of ABPM data.

[Assorted aluminium-oxide ceramic heads for prostheses of the hip-joint. A preliminary communication (author's transl)].

In the search for even more hard-wearing prostheses of the hip, a rotating total- and femoral head prosthesis and a total- and femoral head prosthesis with fixed detachable heads are described, the head of which is made from highly polished aluminium oxide ceramic, Biolox. For total prostheses the excellent gliding features between ceramic head and polyethylen acetabulum are being used. They will also permit return to proved metal-polyethylen prostheses, should the ceramic head fracture. The fracture prosthesis provides both such an exchange and conversion into a total prosthesis. Extending the present programme of rotation-prostheses to ceramic heads will require a wide assortment of heads. This will mean not only wide adaptability but also the need for detailed knowledge of this assortment, the use of which demand thorough preparation of the surgeon. In order to collect material of the use of these prostheses, the authors are very keen to hear of drawbacks- and without reticence-as soon as possible. Publication of our own experience will follow this preliminary communication in due course.
