[Intestinal parasitosis: physiopathologic aspects].

The variety of pathological changes that occur during parasitic infections is diverse. Recently there has been done considerable progress in understanding the physiopathologic aspects of the direct parasitic action of the products that they release, that affect the digestive function. The pathogenetic effects of intestinal parasites are important, not only for the adult forms that occupy the lumen or the intestinal mucosa, but for the migratory forms. The most important clinical manifestations are in direct relation with the host response to the aggression or trauma caused by the migratory larvae and/or adults parasites, the signs and symptoms depends on the infestation phase and on the severity or in the agent "virulence". The objective of this article is to present a brief revision on the physiopathologic effects of infestations by intestinal parasites.

[Gastroenterologic manifestations of acquired immunodeficiency syndrome].

The gastrointestinal system is a target organ for many infection in the immunosuppressed. Alterations in mucosal immunity may explain the increased incidence of enteric infections, abnormalities are most marked by a decrease in total T cells, CD4 T cells and IgA producing cells. Such involvement of the gastrointestinal tract contributes to nutritional problems and may provide a portal entry for systemic infection. Gastrointestinal manifestation of patients with AIDS has been found in 78% of patients in Perú; chronic debilitating diarrhea is commonly. These immune deficits predispone such patients to rare tumors; malignant neoplasia complicate AIDS in 12% of the cases in USA, most common was Kaposi sarcoma followed by Lymphoma. Liver abnormalities are common, those related to the opportunistic infections, chronic debilitating illness and concident exposure to hepatotropic viruses. Disinfection of the endoscopic instruments is very important; glutaraldehydes can be used in fiberoptic instruments and their accessories. This report reviews the clinical problems, diagnosis and treatment in patients with AIDS and gastrointestinal manifestation.

[Comparative study between midazolam and diazepam in upper gastrointestinal endoscopy premedication].

A prospective study evaluated and compared midazolam versus diazepam in the upper gastrointestinal endoscopy. Patients were excluded if they had experienced any of the following: intolerance to benzodiazepines, pregnancy, alcohol or drug addiction, and weakness chronic disease. Seventy one patients were assigned to two groups: 36 received diazepam (0,15 Mg/Kg. intravenous) and 35 midazolam (0,07 mg/kg.). Patients in the two study group were similar in regard to demographic data (sex, age, and weight). Midazolam resulted in a marked improvement of conscious sedation and the anxiolysis as compared with the group receiving diazepam (p<0.001). Amnesia were significantly more frequent in the midazolam group (total 77,14% and partial 17,14%) compared with diazepam group (total 11,11% and partial 2,77%). The present study show that midazolam improves patient tolerance lo endoscopy compared with diazepam (p<0.001).The selection of patients is very important, no clinically adverse events related to cardiopulmonary changes ocurred in any group.
