[Vermillionectomy].

Several external factors such as sunlight and tobacco smoking may cause changes in the epithelium of the vermillion border, especially of the lower lip, resulting in a dry and crusty epithelium often accompanied by painful ulcerations. Some of these patients will eventually develop lip cancer in the vermillion border. We would like to present a consecutive series of 25 patients treated with vermillionectomy. It is a very good and efficient treatment which may be the solution to a long standing problem. The functional and cosmetic results are very good.

[Thyroid surgery performed at a head and neck oncologic department].

During a period of ten years, 1977-1986, 778 patients were treated for diseases of the thyroid gland. Of these, 678 were benign and 100 malignant. The reasons for referral were most frequently a lump in the thyroid gland, possibly with suspected malignant changes. In the group with benign conditions, there were five times as many women as men whereas the proportion in the malignant group was 1.5 to 1. Technetium-scintigraphy and fine-needle aspiration were undertaken routinely preoperatively. As a rule, none of the preoperative investigations could reveal with certainty whether the condition was benign or malignant but, in certain cases, the investigations may provide indication and guidance for the continued treatment or control of the patients concerned. Treatment with particular attention to the recurrent nerve and the complications, particularly recurrent paresis, is mentioned.

[Bilateral paralysis of the vocal cords treated with endolaryngeal arytenoidectomy].

Bilateral paralysis of the vocal cords is a serious condition in which even slight oedema of the mucosa may endanger the patient's life. Bilateral paralysis of the vocal cords is most frequently due to thyroid operations but intubation, neuritis etc are among the other causes. Tracheostomy is nearly always necessary. In an attempt to render removal of the tracheal cannula possible with retention of vocal function, the authors have performed endolaryngeal arytenoidectomy. This intervention was carried out in 28 patients. In 23 of these the cannula could be removed while various conditions prevented removal of the cannula in the remaining five patients. The authors consider that arytenoidectomy is a good form of treatment of bilateral paralysis of the vocal cords involving only a slight risk. Thereafter, there are good chances for removal of the cannula with simultaneous retention of vocal function.

Mediastinoscopy in bronchogenic carcinoma.

Mediastinoscopy was performed in 470 patients with bronchogenic carcinoma in the ENT Department, Aalborg. The histological diagnoses were 195 squamous cell carcinomas, 149 adenocarcinomas, and 126 anaplastic carcinomas. Metastases in the mediastinal lymph nodes were found in 177 of the 470 patients (38%). Metastatic lymph nodes were found in approximately one fifth of the squamous cell carcinomas, in almost half of the adenocarcinomas, and in about half of the anaplastic carcinomas. The need is emphasized for lymph node biopsy from both right and left tracheobronchial angles and inferior to the bifurcation irrespective of the site of the carcinoma. Contralateral metastasis was found in approximately one third of the cases, and a positive biopsy from the nodes inferior to the bifurcation was found in about 40% of the patients in whom metastasis in the mediastinum were detected. In 10% of the patients, cytological and histological examinations of specimens obtained by bronchoscopy were negative, and the diagnosis of lung cancer was made by mediastinoscopy exclusively. The value of mediastinoscopy was particularly high in the diagnosis of adenocarcinomas.

Adenocarcinoma

[Bronchoscopy in monosymptomatic hemoptysis--hemoptysis in bronchial carcinoma].

Blood in sputum, haemoptysis, is a common symptom in mucous membrane disorders of the respiratory tract--it is often seen in conjunction with inflammation but may also be associated with malignant tumours of the respiratory tract. In a survey of 422 patients with monosymptomatic haemoptysis, 15 patients with bronchogenic carcinoma were identified. A cytological examination of the sputum of these 15 patients was carried out, and in 9 of these cases suspected malignant cells were observed. From these and other observations it is concluded that with patients of over 40 years of age who are smokers with haemoptysis, but who display a normal X-ray examination of the thorax, it would be prudent to carry out a cytological examination of the sputum before bronchoscopy.

Ductal carcinoma of the parotid gland.

A case of ductal carcinoma of the parotid gland is described. The medical literature contains only 13 previous reports on this kind of adenocarcinoma of the parotid gland. The tumour is characterized by its histologic resemblance to ductal carcinomas of the breast and prostate. The course of previously described cases suggests that this tumour has a highly aggressive biological behaviour.

Inflamed adenolymphoma with cholesterol granuloma.

Twelve cases of adenolymphoma with inflammation are described. Pre-operatively, three of these patients presented clinical signs of inflammation in the parotid gland. In ten cases purulent exudate could be aspirated from the tumour. Rupture of the cyst may cause an aseptic inflammation in the surrounding tissue. It is concluded that adult patients with clinical or cytological signs of inflammation in the parotid gland must be supervised until the gland is normal in order not to miss an underlying tumour.

Adenolymphoma

Orbital floor fractures: early repair and results.

In the 5-year period 1978-1982, 34 patients with facial fractures underwent repair of the orbital floor. Reconstruction of the orbital floor by means of an implant was used in 28 of 34 patients (82%). Nineteen patients (56%) had a balloon inflated in the maxillary sinus to stabilize the orbital floor. Radiographic contrast solution was used in the balloon to facilitate the postoperative radiographic control of the balloon and the orbital floor. It was possible to make a follow-up study of 30 of the 34 patients. Nine of the patients had persisting reduced sensitivity in the infraorbital nerve region and 7 patients had slight enophthalmos. Out of the 30 patients, only 2 patients had persisting diplopia of a slight to moderate degree and none found it disturbing enough to need further surgery. This series shows that good results can be expected after early surgery and active management with open reposition, reconstruction and placing of an intral balloon are recommended.

Chondrosarcoma of the larynx.

Chondrosarcoma of the larynx is a rare neoplasm. The characteristic features of this tumor are described, and a case in a 69-year-old woman is reported. This case demonstrates the difficulties of histological classification. On the first biopsy specimen a benign chondroma was diagnosed. However, the microscopical features of the excised tumor were those of a chondrosarcoma. It is generally accepted that radiotherapy is of no value; surgical treatment by laryngectomy is advised. Some authors recommend a conservative attitude towards these tumors, which are of low malignancy, the growth slow and the risk of local or distant metastases slight.

Aged

Inverted papillomas of the nose and the paranasal sinuses.

A series of 21 patients with inverted papillomas of the nose and/or the paranasal sinuses is presented. In 19 patients the tumour arose from the lateral nasal wall or paranasal sinuses, in 2 it arose from the nasal septum. Nine patients had been previously treated by nasal polypectomy. None of the patients with recurrences after primary surgical treatment showed any evidence of a progression from benign to malignant lesions. Only 14% of the patients suffered from recurrence, and it seems likely that this is because extensive surgical extirpation was recommended as primary treatment in most cases. We feel justified in recommending early and radical surgery.

Vocal cord sulcus.

Vocal cord sulcus is a congenital condition consisting of a furrow on the medial edge of the vocal cord. It is most often bilateral but may be unilateral. The symptoms are a hoarse and breathy voice due to incomplete closure of the vocal cords. The present series comprises 15 patients found among 1,400 patients with voice and speech disorders. The condition is often overlooked and regarded as part of a primary functional hyperkinetic voice disorder. By close inspection the furrow can often be seen by indirect laryngoscopy. In many cases, however, microlaryngoscopy under general anaesthesia should be performed, but the furrow is only detected if a search is made for it. The condition is often found in younger patients, probably due to the fact that the sulcus is difficult to detect in older patients who have developed severe organic changes in a effort to overcome the incomplete closure of the glottis.
