Schizophrenia - A disturbance of signal interaction between the entorhinal cortex and the dentate gyrus? The contribution of experimental dibenamine psychosis to the pathogenesis of schizophrenia: A hypothesis.

In addition to the existence of complex memory (similar to the implicit nondeclarative memory of Squire), the existence of a phylogenetically old apparatus of a memory of situations (SMA) is supposed, which is to some extent comparable with the declarative memory of Squire. During actual sensory information the SMA generates a general frame and forms a general 'mark', indicating whether a given information has its origin inside or outside the body, and whether it is new or known. The procedure of this marking process can be explained as the time-depending arrest of a copy of the actual original information-transporting signal 'shower'; this copy must last until the feedback from thalamocortical centers indicates the termination of the processing of the original signal showers. The arrest of the shower copies is the performance of neuronal networks of the entorhinal cortex (EC) and the gyrus dentatus (GD). The psychopathological and biochemical analyses of experimental dibenamine psychosis show a different effect of dibenamine on the noradrenaline (NA) receptors of the EC and GD, respectively: these effects are responsible for the repeated perception cycles of a single situation. N,N-Dibencylamine blocks the postsynaptic alpha(1)-receptors of the EC without influencing the beta-receptors of the GD. Thus the interaction between EC and GD is changed: instead of new scenes, perceptions that have just been experienced get repeated presence and the quality of familiarity. The prolonged arrest of shower copies simultaneously blocks the entrance of new signal showers from the EC to the GD. No information-transporting signal showers can come in as long as the arrest lasts. In case of a disturbance in NA-dependent actions within the EC and the GD, the duration of arrest of information-transporting signal showers is shortened. Thus the formal frame of experience receives the quality of novelty instead of familiarity, and in addition the qualities of uncertainty, vagueness, and alienity. These very changes in perception and experience represent the basic disturbance of schizophrenia. All the symptoms of schizophrenia may be explained by this basic disturbance. The analysis of biochemical aspects turns attention to the energetic situation of NA and N-methyl-D-aspartate systems. These considerations suggest a genetic background of the basic disturbance of schizophrenia: transmitter effects on membranes of neurons and possibly also on glial cells, and energy supply of these effects may be predetermined genetically. It may be assumed that the compensation of such membrane-dependent disturbances will be possible within wide areas of the neural network, except for the 'bottleneck' of the overlapping region of the iso- and allocortex.

Schizophrenic thought disorders: comparison between units of symptoms and axial syndrome.

The schizophrenic axial syndrome postulates the presence of thought disorder and/or cryptic neologisms. The theory of symptom units postulates three groups of symptoms within the schizophrenic pattern. Each group arises through a special basic disturbance within the performance of experience close to the biological substrate. 465 schizophrenics have been examined personally at least twice within 8 years. We investigated signs (A) of subjectively perturbed experience of thoughts, such as transitoriness, disconnection, pressing, movielike thinking, double-sense thinking: withdrawal, broadcasting, telepathy, insertion, audible thinking, postresonance, echo; lack of impressivenes, veiled thinking, hazy thinking, thoughts getting stuck; thoughts getting muddled and crossed, condensation, contamination; (B) of perturbed performance of thinking, such as loosening of association, thought gaps, derailment, substitution, blocking, desultory thinking, tangentiality, performed word contaminations, word approximation, dissolution of terms, private use of words, disturbed formulation of the meaning of facts, vague thinking, paraphrasia, neologism, driveling. The series of general frequency shows: driveling 67.9%, desultory thinking 57.3%, withdrawal, broadcasting, insertion 32.7%, loosening of association, gaps, derailment 28.9%, blocking 16.5%, transitoriness, movielike thinking, double-sense thinking 12.0%. From the practical point of view, the time of the first manifestation of the symptoms is important. This will de demonstrated. The study confirms the presence of thought disorders in each case but at different times within the course depending on the type of course (phases, attacks, attack processes, process). With regard to these results the theory of symptom units can assist the practicability of the schizophrenic axis syndrome.

Levels of interaction between schizophrenia and manic-depressive disease.

Interaction between schizophrenia and manic-depressive disease (MDI) has to be examined with respect to type, intensity and time points within the courses. Interactions are possible at the level of genetics, constitution, biological (e.g. biorhythmical) mechanisms, neurophysiological actions, biochemical complexes and psychopathological interferences including defense mechanisms of personality. The common denominator seems to be the energetic situation into the satellite-glia-neuron cooperation. Activity of G6PD and AK 2-1 could be a hint of a factor of bipolarity, existing independently from schizophrenia and MDI. Interactions of different psychoses should be designated as interference psychoses. Within this field, the term schizoaffective characterizes the interference between schizophrenia and MDI, mixed psychoses mean stages of rapidly alternating or stable combinations of symptoms within the MDI. 'Legierungspsychosen' should be reserved for interferences at the genetic level.

Fluvoxamine-a new serotonin re-uptake inhibitor: first clinical and psychometric experiences in depressed patients.

The clinical efficacy of fluvoxamine (DU 23000)--the first selective serotonin re-uptake inhibitor of the new class of 2-aminoethyloximethers of aralkylketones was investigated in endormorphous depressed patients during 5 weeks treatment with mean daily dosages of approximately 150 mg DU 23000. A marked and statistically significant improvement of the overall and detailed psychopathology was noted by means of the Global Clinical Impression and Hamilton Rating Scale as early as in the first week of drug administration. Bipolar patients tended to improve more than unipolar ones, retarded depressions more than agitated ones. An additional anxiolytic medication was occasionally required. The drug was well tolerated. Psychometric and laboratory findings did not show any significant changes. The findings are discussed in the light of the indolamine hypothesis of depression.

[Adenylat-kinase-Systeme, ein Beitrag zum manisch-depressiven Kranksein (MDK)].

Adenylate-kinase (AK) interacts in the balance of phosphate acceptors and donators. With respect to the polymorphism of enzyms of erythrocytes there can be assumed a system of two co-dominate allels determined by the genotype of AK 1 and AK 2 with the phenotypes of AK 1-1, AK 2-1 and AK 2-2. The distribution of AK within 108 cases of MDS and 90 controls was examined. AK 2-1 was found in about 30% of bipolar MDS, but in no case of unipolar MDS or controls. The possible relations were discussed demonstrating a synopsis of the pathogenesis of MDS.

[N,N-dimethyltryptamine: first comparative results. Preliminary communication].

On 7 male subjects (VP) the effect of 1.0--1.2 mg/kg N,N-dimethyltryptamine (DMT) s.c. on the acoustic evoked potentials (AEP) was studied; in 3 subjects simultaneously with the scalp EEG. The AEP were reduced 10 min after the application of DMT and returned at the 55th min. The on-off effect was diminished at the same time. Nevertheless, the VP recognized every click and tone and understood every word. d-LSD 25 (100 mug p.o.) showed no effect on AEP. The maximum of the psychopathological phenomena of DMT was shown about the 15th min. The maximum of the AEP suppressing effect was observed at the 30th min and coincided with the extrapyramidal signs of DMT effect. In cats (2 mg/kg DMT i.v.) AEP reacted in the same way. 2 mg/kg serotonin (5-HT) i.v. did not change the AEP in cats. I.v. DMT application 23 min later reduced the AEP only for a short period. It is discussed whether the antagonistic mechanism on peripheral substrates of 5-HT and DMT might be possible also on central synapses. In that case DMT might suppress inhibitory effects of 5-HT on receptors of the area postrema and---via feed-back---stimulate neurons of the ascending reticular system (hallucinogenic effects). The extinction of AEP and the reduction of on-off effect might be interpreted as a restoration of 5-HT effect with overcompensation and, therefore, with strong inhibitory interaction.
