[1 year of multiple organ procurement using the Starzl technic of rapid cooling: personal results].

We did perform 20 multiorgan procurements in 1987, using the last technique described by STARZL. We were very favorably impressed by the security (all organs harvested achieved good long-term function), the rapidity (mean operative time 125 min) and the flexibility of the technique. Waiting lists for hearts and livers in Switzerland are now becoming long, and it seems thus essential that all usable organs can be retrieved for transplantation.

Hepatic trauma: risk factors influencing outcome.

This retrospective study analyses the fate and associated risk factors of 99 patients who underwent laparotomy for hepatic trauma from 1977 to 1986. Blunt trauma (88 patients) and stab wounds (7 patients) had mortality rates of 36 and 14 per cent respectively. The overall death rate was 35 per cent. Multiple trauma patients had a significantly higher mortality for each additional system that was seriously injured. Pre-operative shock raised the mortality from 20 to 58 per cent (P less than 0.001). For patients over 50 years of age, the mortality rate increased from 30 (younger patients) to 63 per cent (P = 0.028). Minor hepatic wounds required relatively simple surgical measures in 60 patients, yet 13 died (22 per cent) of other causes. More sophisticated surgical management was applied to 27 major hepatic lesions, with 10 deaths (37 per cent). Twelve patients (seven with minor and five with major hepatic wounds) died before surgical treatment of the liver injury could be undertaken. The overall mortality rate of major hepatic wounds was 47 per cent and for minor wounds 30 per cent. However, the difference was not significant (P = 0.152). Resection was resorted to in 15 patients, 5 of whom subsequently died (33 per cent). Classical hepatic lobectomy led to 4 deaths (50 per cent). Among the 35 deaths, 6 were due to the hepatic trauma itself (5 exsanguinations and 1 intra-abdominal sepsis). Isolated hepatic injury was fatal in three patients. We believe that conservative surgical measures should be used whenever possible in patients with hepatic trauma, especially when risk factors are encountered.

[Management of ligament injuries of the anterior ankle joint with ligament suture and a joint plaster cast].

Treatment of ligamentous injuries of the ankle joint by sutures functionplaster. 139 patients had been checked up after treatment by suturing and functionplaster because of ruptures of ligaments of the ankle joint. Four days after a funktionplaster has been applied the leg has been fully loaded. Four weeks later the plaster was taken off. At this time the ankle joint can be fully loaded and is mobile. 127 patients were absolutely restored, ten patients had slight complaints, in one case a slight instability has been remaining. One patient showed a ligament rupture of the ankle joint following newer distorsion. Twelve persons had superficial wound necroses. In one case there was an infection of the wound, the time of treatment was 78 days. This patient complains of pains by loading.

Cyclodextrin-glycosyltransferase from Klebsiella pneumoniae M5a1: cloning, nucleotide sequence and expression.

The structural gene encoding cyclodextrin-glycosyltransferase of Klebsiella pneumoniae strain M5a1 was cloned; it is expressed both in Escherichia coli and in K. pneumoniae and the gene product is secreted into the extracellular space. Determination of the nucleotide sequence revealed an open reading frame coding for a single polypeptide of 655 amino acid (aa) residues. The enzyme is synthesized as a precursor with an N-terminal signal peptide of 30 aa residues, which is proteolytically processed between two alanine residues during export. The primary structure of CGT bears homology with the sequences of amylases from both prokaryotic and eukaryotic origins.

[Emergency surgery of complicated gastroduodenal ulcers: risk factors and results].

Between 1972 and 1981, 290 patients underwent emergency surgery in this Department for hemorrhagic (n = 104) or perforated (n = 186) gastric or duodenal ulcer. The introduction of cimetidine in 1977 did not influence the annual number of operations for complicated ulcer. Operative mortality was 15% for haemorrhage and 11% for perforation. Age, associated diseases, total bleeding, and delay between perforation and surgery were the most significant risk factors. In the case of inaugural complication or of brief peptic disease, simple closure, with a recurrence rate of 21%, remains the procedure of choice.

Tomographic imaging of the human thyroid with a positron camera before and after partial thyroidectomy.

A high-density avalanche chamber positron camera was used for tomographic imaging of the human thyroid before and after partial thyroidectomy. Images were made between 6 and 24 h after oral administration of the positron-emitting radionuclide, Na-124I with activities varying between 0.1 and 0.3 mCi before the surgical intervention and with activities between 0.03 and 0.05 mCi following partial thyroidectomy. The results of thyroid imaging performed on 50 patients and their surgical relevance are discussed; as an illustration, one typical case is presented. As a consequence of the high spatial resolution of the camera (2.5 mm full width at half maximum), the functional volume of the thyroid may be estimated from the transaxial tomographic sections before and following partial thyroidectomy, correct to about 10%. The thyroid surface, defined by the contours from each transaxial section, may be displayed using three-dimensional shaded-graphics techniques. This new imaging technique makes possible a fully three-dimensional description of the thyroid in vivo and contributes significantly to the surgical follow-up.

[Late results of total colectomies for inflammatory colitis].

A retrospective study of 41 patients colectomized for inflammatory colitis has been conducted with a mean follow-up of 90 months. The results of rectum preservation and of ileo-rectal anastomoses were disappointing, with a respective success rate of 15% and 44%. 15% of patients had a change of diagnosis after colectomy, all in favour of undetected Crohn's disease. The results of mucosal proctectomy with ileoanal anastomosis are highly encouraging. An attempt is made to specify the indications for each type of surgery.

[Percutaneous drainage of intra-abdominal fluid collections using computed tomography].

Percutaneous catheter drainage guided by computed tomography has been used to treat 45 intraabdominal collections in 32 patients. The 45 drainages involved 15 pancreatic cysts of chronic pancreatitis, 15 spontaneous abscesses and 15 postoperative abscesses. The collections were treated either by one puncture only or by drainage, with a success rate of 87.5%. Morbidity was 2.2% (one right empyema after puncture of a postoperative right subphrenic abscess). Mortality was nil. Three inadequate drainages due to enteric communication required secondary surgery. It is concluded that, if the indications are correct, percutaneous drainage guided by CT is as good as surgery and involves lower morbidity and mortality.

Influence of nandrolondecanoate on the pituitary-gonadal axis in males.

Different anabolic steroids can exercise different effects on the pituitary-gonadal axis in males. During a pilot study regarding the possible beneficial effect of the anabolic steroid nandrolondecanoate (ND) on bone metabolism in patients with rheumatoid arthritis additional endocrinological parameters were studies. A significant decrease was found in the serum levels of testosterone, androstenedione and FSH and the ratio of testosterone/oestradiol. There was a significant increase in the serum levels of oestrone. The levels of oestradiol, SHBG, LH and cortisol remained unchanged. An inhibitory effect of ND on testicular testosterone secretion is assumed. The decrease in androstenedione levels is explained by the diminished testosterone secretion. The rise in oestrone levels is explained by peripheral aromatizing of ND to oestrogens. The presented findings are in accordance with the hypothesis that sex steroids can act directly on the pituitary resulting in selective FSH and LH secretion. The possible role of the ratio testosterone/oestradiol in controlling gonadotrophin output is discussed.

Lack of influence of the anabolic steroid nandrolondecanoate on bone metabolism.

Eleven male patients with rheumatoid arthritis were treated with the anabolic steroid nandrolondecanoate and parameters of calcium and bone metabolism were studied. No changes were found in the concentrations of calcium, phosphorus or hydroxyproline in serum and urine (corrected for the creaturia). A significant decrease (P less than 0.01) was observed in the levels of alkaline phosphatase, but this was attributed to a change in liver enzymes, because the gamma glutamyl transpeptidase fell. Acid phosphatase levels showed an increase, possibly as a result of an effect of nandrolondecanoate on the prostate. No change was found during treatment in the serum levels of parathyroid hormone, calcitonin, 25 hydroxyvitamin D, 24, 25 dihydroxyvitamin D and 1.25 dihydroxyvitamin D. A seasonal fluctuation was observed for 25 hydroxyvitamin D and 24, 25 dihydroxyvitamin D, but not for 1,25 dihydroxyvitamin D. It was concluded that short-term treatment with the anabolic steroid nandrolondecanoate did not result in changes in parameters of calcium and bone metabolism.

Association of severe rheumatoid arthritis with heterozygosity for alpha 1-antitrypsin deficiency.

Genetic types of alpha 1-antitrypsin (protease inhibitor types, or Pi types) were determined in 108 patients with rheumatoid arthritis. These patients were selected for severely destructive disease and had classical rheumatoid arthritis according to ARA criteria, were seropositive, and had joint erosions shown by X-ray. Heterozygotes for the deficiency Z allele (Pi types MZ, SZ, etc.) were found among 9.2% of patients and 3.5% of a control adult population. The increased frequency in patients was statistically significant. Heterozygotes were most frequent among female patients with an early onset of disease. Heterozygosity for alpha 1-antitrypsin deficiency may be a factor in familial recurrence of rheumatoid arthritis. Among 98 patients with juvenile rheumatoid arthritis not selected for severity, 4.1% were Z heterozygotes compared with 1.3% of control children, not a statistically significant difference. Reduced concentrations of alpha 1-antitrypsin in Z heterozygotes may be inadequate to inhibit the proteolytic enzymes released into the joints of adults with rheumatoid arthritis during phagocytosis of immune complexes. This may be a factor promoting severe joint destruction.

[Survival following surgery for bronchopulmonary cancer. Results after five years].

The authors study the actuarial survival of 253 primary bronchopulmonary cancers operated in Geneva between 1967 and 1976. The overall operative mortality was 9.88%. The overall survival of resections alone was 26.6% at 5 years. The small size of the tumor, the absence of mediastinal extension and an epidermoid histology are the best survival rate factors: 43.2% of stage I cases survive at 5 years against 4.86% of stage III cases. The accidental discovery of as asymptomatic tumor gives a hope of survival of 43.9% at 5 years. Yet again this demonstrates the importance of mass screening by radiophotography. Surgery thus maintains its central role in the treatment of this disease, whilst chemo-, radio- and immunotherapy have not yet revealed their full possibilities.
