Mistreatment of the aged in the home environment in northern France: a year survey (1990).

As the percentage of the population which is aged increases in the developed countries, public health problems arise from the limited autonomy and physical or mental infirmity of the elderly. Cohabitation arrangements, often used to permit the elderly to stay at home, can give rise to conflict and the mistreatment of the aged. We analysed questionnaires distributed in 1990 to 25 home nursing units and two hospitals in a part of the northern region of France. Fifty-five observations of mistreatment were collected, 22 from hospital and 33 from home-based inquiries. The types of mistreatment and the characteristics of the victims and perpetrators are detailed and discussed. Penal and legal aspects of mistreatment of the aged in France are discussed.

[Lateral approach of the knee with tibial tubercle osteotomy for prosthetic surgery].

The authors describe a lateral approach to the knee associated with a tibial tubercle osteotomy and a plasty using the fat pad to provide a complete closure of the wound. This lateral approach has been performed successfully in 98 cases, with only 3 skin necrosis. It offers a large exposure of the joint, facilitates placement of tibial component trial fit, and allows the self centering of the quadriceps patellar-tendon mechanism at the end of the intervention. The lateral approach disputable in case of genu varum is recommended in all cases of genu valgum or important deformities.

[Chondrosarcoma of the foot. Apropos of a case--literature review].

Primitive chondrosarcoma of the foot is rare. About 50 cases have been reported. It often poses diagnostic problems, and the confirmation of malignancy necessitates a broad resection. The tumor is radioresistant and chemotherapy is minimally effective, as illustrated by the present report.

[Fractures-luxations of the Lisfranc joint. Apropos of 39 cases].

Lisfranc fracture-dislocations are relatively rare (two per thousand). They generally occur in multiple trauma victims and may pass unnoticed. Open reduction followed by pinned osteosynthesis and plaster immobilisation ensures a good anatomical and functional result.

[Fracture of the base of the first metacarpus].

The authors report a group of 73 cases of fractures of the base of the first metacarpal treated between 1974 and 1989. Fifty-three articular fractures (73%) and 20 non-articular fractures are described. Sixty-eight patients were treated surgically: 49 diverging pin insertions were used, 15 Lars Thoren technique, 5 rigid osteosyntheses with AO miniaturized material. Forty-two patients were seen again after one year or more. We classify the results as follows: very good: 29 (69%), good: 10 (24%), bad: 3 (7%). The relative simplicity, the reliability and the good results of the double diverging pin insertion lead the authors to apply this method in most fractures of the base of the first metacarpal, either articular or extra-articular.

[Laparoschisis: therapeutic problems. Apropos of 48 cases].

A non-selective series of 48 neo-nates was treated during a period of 8 years (1978-1985), with follow-up more than four months for recent observations. Surgical treatment comported Gross-technique in 32 cases; immediate total-closure in 7 cases and progressive reintegration (Schuster) in 10 cases. Long-term parenteral nutrition was performed in 38 neo-nates. The etiology of different deaths (toxic++-infection, mechanical obstruction) is discussed. The percentage of survivors was progressively raised: 70% since 3 years, and better in the last 11 cases, all with good result.

Treatment of the idiopathic nephrotic syndrome.

Most of the paediatric patients presenting with nephrotic syndrome (NS) have a nephrosis (minimal glomerular changes with or without focal lesion NS) and most of them are characterized by a clear-cut steroid response: a complete remission is rapidly obtained with high dose of glucocorticoids. Many of them, however, relapse during or after the treatment's withdrawal. Most often, these patients can be managed for several years with long-term alternate-day steroid therapy. Alkylating agents must be used exclusively when the doses of steroids required result in severe toxicity. In other types of NS (steroid resistant nephrosis, membranous and membranoproliferative glomerulonephritides) the efficiency of drugs is not established. Whatever the cause for the NS, adequate supportive therapy is necessary. The constant aim is to allow the patient a normal physical, school and social activity.

[Bone and vascular injuries of the popliteal fossa. Apropos of 21 cases].

The authors report twenty-one cases of bony and vascular trauma of the popliteal fossa. The first step in the appropriate treatment of these lesions is to be aware of the associations of the two types of lesion, independent of the degree of bony damage; an isolated dislocation may be associated with a sub-adventitial rupture of the popliteal artery. Surgical treatment consists of bony fixation followed by vascular repair, which requires autologous popliteo-popliteal or femoro-popliteal venous by-pass grafts in 90 per cent of cases. This repair should concern both the artery and the vein, as the immediate prognosis depends on the complete repair of both vessels. The immediate results essentially depend on the nature of the traumatic aetiology: gunshot wounds, or motor vehicle accidents, and on the quality of the vascular repair. The long term prognosis depends on the quality of the orthopaedic treatment. In every case, the classical reputation of the severity of such associated lesions is fully justified.
