Solitary plasmacytoma of the lung with light chain extracellular deposits: a case report and review of the literature.

AIMS: We present the clinical and histopathological findings of an unusual pulmonary plasmacytoma associated with light chain deposits. METHODS AND RESULTS: The tumour was located in the main left stem bronchus 40 mm from the carina. Histologically, it was composed of sheets of well differentiated plasma cells. Large extracellular deposits of amorphous material were observed in the tumour. These deposits were Congo red negative and contained kappa light chains. They were electron dense granular and non-filamentous. No plasmacytosis was identified by bone marrow biopsy and no monoclonal spike was shown by serum and urine electrophoresis. CONCLUSIONS: Our case is unusual in being endobronchial and showing light chain deposition.

[Delayed detection of cystic fibrosis in a patient with Mycobacterium avium complex infection].

We report a case of pulmonary infection by Mycobacterium avium complex revealing in a 21 years old patient a cystic fibrosis heterozygous for the delta F 508 deletion. The role of this bacteria in the lung infection is suggested by repeated isolation. This infection caused bronchitis, hypoxemia and pulmonary nodules at CT scan. The clinical and radiological signs improved after treatment with four antibiotics whereas only clarithromycin showed in vitro activity. An obstructive disease due to allergic bronchopulmonary aspergillosis developed and was controlled by steroid therapy.

Acute silicosis due to inhalation of a domestic product.

A 20-yr-old woman with no history of pulmonary disease presented with acute dyspnea and fever. After various investigations and treatments the symptoms persisted and a lung biopsy was performed. Pathologic analysis disclosed the presence of silicotic crystals within lung tissue and mediastinal lymph nodes. The patient admitted having repeatedly inhaled a domestic scouring powder rich in silica during the preceding 6 months. This is the first report of acute pulmonary silicosis developing after exposure to a common household product.

[Intestinal metastasis of pulmonary cancers. Apropos of 2 cases].

We report 2 new cases of intestinal metastases from lung cancer. Such metastases are not exceptional, but their clinical manifestations are rare and their diagnosis during evaluation of lung cancer is difficult. This diagnosis could be suggested by the presence of iron deficiency anaemia or melena in a patient being investigated for lung cancer. Other metastases are often present, and the prognosis is fatal at short term.
