[Abortion applicants--reasons, prevention, information].

In 1989, 300 women who were admitted for abortion at Buskerud Central Hospital answered questions about their reasons for deciding to terminate the pregnancy, and about prevention and information. The material was compared with a previous study of journals from 1977. In both cases the age group 15-20 years constituted 25% of the material. In 1989, 53% gave problems at home and economy as reasons for abortion, compared with 66% in 1977. In 1989, 34% emphasized school and education as reasons, compared with 14% in the previous study. 36% indicated that preventive measures had failed. 81% had received satisfactory information about the operation. 50% were in doubt about their decision.

Twins discordant for vater association. Obstructed labor of the second twin due to ascites and persistent cloaca without communication to the exterior.

The unusual delivery of a dead second twin with rare malformations is presented. The first twin, born live following a normal labor, had no malformations. The birth of the second twin was obstructed by massive ascites, and its abdomen had to be perforated before delivery. The sex could not be determined due to lack of the internal genitalia and the fetal appearance of the external genitals. The left kidney and ureter were hypoplastic. The right ureter and distal part of the colon were dilated and opened into a large primitive cystic cloaca without communication to the exterior. The ascites was probably caused by the urinary obstruction. These malformations probably represent one of the earliest arrested developments of the embryonic hindgut. The presence of a tracheo-esophageal fistula and a single umbilical artery, together with the anal atresia and the renal anomalies, could indicate that the anomalies formed part of the VATER association.

Study of ampicillin levels in maternal serum, umbilical cord serum and amniotic fluid following administration of pivampicillin.

Premature rupture of the membrane is often followed by intrauterine infections. Antibiotic prophylaxis is recommended, but the effect has been debatable. Thirty pregnant women received pivampicillin 350 mg 4 times daily. The ampicillin levels in amniotic fluid and in the serum of the fetus and of the mother were determined and related to pivampicillin dose and time after administration. In the amniotic fluid the average concentration was 3.4 micrograms/ml, which is considered therapeutic. In maternal and fetal serum the ampicillin concentrations were low and took an almost parallel course. To obtain the same ratio between ampicillin serum levels and MIC (minimum inhibitory concentration) in the pregnant as in the non-pregnant subject, a double dosage should be used.
