[Fibrin-coated collagen fleece in thoracic surgery. Initial clinical experience].

After resecting procedures to the parenchyma of the lung the resection site was sealed with a collagen vleece combined with fibrin glue in 26 patients. No patient exhibited rebleeding. An air leak was present up to the first postoperative day in 19 patients, up to the third postoperative day in 5 patients. In two patients, duration of air leak extended beyond the third postoperative day, this was defined as therapeutic failure. The use of collagen vleece combined with fibrin glue represents a valuable contribution in thoracic surgery.

[Preoperative autologous blood donation in thoracic surgery].

The donation by the patient of two units of his own blood shortly before (on average on the 9th and 7th day) scheduled major surgery on the chest is feasible in 70 to 80% of patients. In our group (n = 170) it resulted in a highly significant reduction in the use of banked blood units: a 76% reduction vis-a-vis the number of units transfused in a group operated on in the previous year and comprising comparable patients receiving comparable procedures, in whom no autologous blood was administered. The procedure is cost-effective.

Bronchoscopic photodynamic therapy of bronchial carcinoma.

Photodynamic therapy with photosensitizers is a new method of treating malignant tumors. In the area of bronchoscopy it is employed for the selective therapy of early carcinomas at central localizations. We report on our first attempts at treatment with hematoporphyrin derivative in an inoperable patient. The modalities of irradiation and the recurrence-free progress of, to date, 2.5 years, are described.

[Coincidence of pulmonary fibrosis and bronchial carcinoma (author's transl)].

Eight cases of endothoracal carcinoma associated with pulmonary fibrosis or with interstitial pneumonia are reported. The interstitial pneumonia and the fibrosis and carcinoma are confirmed in all cases. Statements on incidence cannot be made: they are more or less random observations in recent years. There are, however, grounds for supposing that this coincidence is by no means rare. Problems arise in differential diagnosis and the decision to operate, for irradiation therapy and for chemotherapy. The question of a possible causal connection, whether in the sense of a paraneoplasia or of cancer development due to previous damage to the lung is left open.
