[Dynamic biliary manometry: display modelling and graphic interpretation].

Tendencies of development of biliary manometry have been analyzed. Key advantages and problems of manometric investigation of biliary tracts have been summarized. New method of graphic registration and pressure monitoring in biliary tracts called biliary manometry has been suggested. Characteristic types of manometric curves were determined using stand modelling, their physical and mathematical analysis was conducted, clinical analogues have been suggested. The emphasis has been made on expediency of its further elaboration and clinical application.

[Minimally invasive interventions for concurrent diseases of extrahepatic biliary ducts and pancreas].

The results of treatment of 689 patients with calculous cholecystitis and of 12 with cholecystogenic acute pancreatitis (AP), to whom laparoscopic interventions were performed, are adduced. In 50 patients the indexes of operative stress were studied up. The laparoscopic cholecystectomy efficacy was established in patients with cholecystogenic AP in the absence of obturational jaundice.

[Combined intensive therapy and anesthesiological support for a surgical intervention in patients with necrotic pancreatitis].

The results of surgical treatment of 93 patients with necrotic pancreatitis are adduced. Of 71 patients, operated on using laparotomy, 18 (25.3%) have died, of 22 patients, in whom the laparoscopy (LS) was used, 1 (4.5%) have died. In 17 patients the laparotomy was omitted due to combination of LS with complex conservative treatment. Of 44 patients operated on 3 (6.8%) have died during the period of the improved tactics introduction.

[Morphological changes and function of gastric and pancreatic mucosa after selective proximal vagotomy in patients with pyloroduodenal ulcer].

The pathomorphological changes in the gastric mucosa and acid production were studied in 118 patients with ulcer disease of a pyloric portion of the stomach and duodenum before and after selective proximal vagotomy in combination with drainage operation. Before the operation, the atrophic changes were the most pronounced in the gastric antrum. Increase in debit of chlorohydric acid was caused by increase in the volume of gastric juice and acid production. Stimulation with food contributes to considerable increase in gastrin concentration in the blood of the patients in low level of glucagon and insulin. Early after the operation, the signs of acute inflammation were revealed in the mucosa of the body and antrum. In increase of a basal level of gastrin and glucagon, acid production significantly decreased. Long-term atrophic gastritis of the body of a stomach was noted mainly in patients with delayed evacuation of its contents. Normalization of the level of hormones correlated with the indices of acid-producing function of the stomach.
