This article reviews 305 silicone elastomer ring vertical gastroplasties that were used to treat morbid obesity. All patients underwent surgery 2 or more years ago. The average weight loss was 101 pounds, 35.7% of initial total body weight, or 60.8% of excess body weight. Fourteen patients failed to lose an adequate amount of weight and in four (1.3%) of these patients the surgery failed because the gastric partition disrupted; in these cases, another operation was required. There were no pulmonary emboli, gastric perforations, or deaths in this series. Banding of the vertical gastroplasty stoma with silicone elastomer rings is safe and effective and offers distinct technical advantages over other methods of banding.
Gastroplasty for the treatment of morbid obesity has developed over the last 20 years from inconsistent suture technique operations to precisely constructed procedures. To date, the best results are achieved with the most recently developed techniques, vertical (mesh) banded gastroplasty, and silicone elastomer ring vertical gastroplasty. These operations produce a 33 to 37 per cent weight loss at 2 years with a failure rate of 6 to 8 per cent.
Vertical gastroplasty is a safer and simpler operation than gastric bypass for morbid obesity, but to produce permanent long-term weight loss, a permanent support is needed for the outlet of the newly created gastric pouch in vertical gastroplasty. The most important finding in this study of 1,463 patients with vertical gastroplasties was the development of a safe and practical use of a Silastic ring to permanently support the outlet. Using an uncovered Silastic ring, erosion of the ring has not been observed and long-term weight loss of 36.7 percent of original weight or 63.4 percent of excess weight has been obtained, which is equivalent to that reported for gastric bypass or other gastric reduction procedures. In addition, lower mortality and lower morbidity rates have been achieved than those reported in studies using other gastric reduction methods. Silastic ring vertical gastroplasty with an uncovered ring is the safest and most effective of the gastric reduction procedures in our experience.
Femoral neuropathy occurs when occult retroperitoneal bleeding impinges on the appropriate nerve roots. The syndrome involves the acute onset of groin and thigh pain with characteristic flexion and external rotation of the hip. It may mimic other conditions such as acute arterial occlusion. Thorough knowledge of the anatomy of the femoral nerve explains the clinical features and leads the clinician to suspect the occurrence of this syndrome. Three cases have been reviewed that exhibited this condition as a result of retroperitoneal bleeding, a complication of systemic heparin therapy. The hemophilia that afflicted Alexis, the Crown Prince of Russia and son of Tsar Nicholas and Tsarina Alexandra, resulted in this clinical syndrome. The consequences enabled the sinister starets, Gregory Rasputin, to become intimately involved with the royal family, influencing the response of the Tsar to the political events in Russia, thereby playing an important role in setting the stage for the 1917 Russian communist revolution.
