[Percutaneous and intracavitary radiotherapy of non-operated cervix carcinoma].

At the Krankenhaus der Barmherzigen Schwestern in Linz 153 patients with cancer of the uterine cervix in the stages I to III have been treated from August 1980 to December 1985. In 146 patients (95.4%) complete remission could be achieved. The 5-years overall survival probability (Kaplan-Mayer) in the stages I to III is 79.3%, 62.6% and 40.8% respectively. The 5-years recurrence free survival rate is 91.5%, 59.6% and 52.9% respectively. These results are equivalent to the data achieved by treatments with Radium application. Using the high-dose-rate afterloading method the complication rate is lower.

[Smoking and pregnancy].

Smoking is associated with risks for complications of pregnancy, for negative effects of fetal outcome (birth weight, mental development), with delayed conception, with higher risks for cardiovascular disease at oral contraceptive use and for cancer of cervix uteri.

[The involutional age from the viewpoint of the gynecologist].

Besides the normal aging processes, which affect all human beings, regardless of their gender the involutional years of women have a number of special characteristics. These are based on the fact, that one group of organs-the reproductive system - hold their function earlier than the others. Because of this fact the involution of women ist basically different from that of men. During these years she often require therapeutic help by a physician not only for drug therapy but also psychologically. The drug-therapy seems to be relatively simple: balance of the oestrogen deficit. But today the oestrogen therapy became more difficult and has to be adjusted individually. The decision has to be made in regard to the type of hormone, the dose, the time sequence, the question of the gestagen component, the risk for malignancy, the effect on clothing and vessels etc. Old and new types of therapy including diet, tea, climate and bathing cures, tranquilizers, drugs for the autonomic nervous system, homeopathic therapy, acupuncture etc. also have their place. We will discuss these questions in the afternoon session extensively since the involutional years of women from the view of the gynaecologists are a very interesting and important field of medicine still for therapy and research.

[Influence of screening examinations on stage distributions of cervical carcinoma at a gynecological department ].

Introducing gynecologic (cytologic) screening examinations has proved to alter stage distributions of cervical carcinomas as seen at our department of gynecology. This fact has been established on 1562 patients from 1960-1981. There was decrease of later stage and an increase of early stage neoplasms. Except in a few cases late stage carcinomas of cervix are only seen in women who did not undergo screening examination.

[Vaginal exstirpation of uterus following gynaecological or obstetrical laparotomies (author's transl)].

Of a total of 3,213 vaginal exstirpations of the uterus, this operation was performed in 132 women following gynaecological laparotomies. In spite of this, the incidence of complications was not increased. It is concluded that the advantage of the vaginal route of operation in hysterectomies need not be refused to patients in whom laparotomies have been performed if certain conditions are observed.

[Intraoperative Histological Investigations in Gynaecology (author's transl)].

This study corroborates the great value of pathologist consultation in cases of diagnostic difficulty arising during the course of gynaecological operations. Joint discussion of the macroscopic appearance and/or interpretation of the frozen section appearance proved indispensable in the decision as to the appropriate operative procedure and the extent of the surgical measures in 147 cases submitted to intr-operative frozen section investigation. The highly satisfactory results are discussed and attributed largely to the efficient cooperation between pathologist and surgeon.
