Pseudophakic retinal detachment.

We reviewed 165 cases of non-referred pseudophakic retinal detachment. Fifty-three eyes (32%) had sustained significant complications due to cataract surgery prior to retinal detachment; 45 eyes (27%) presented with grade B, C or D proliferative vitreoretinopathy. The overall reattachment rate was 84%. This was significantly higher in the posterior chamber lens group than in the iris-fixated lens or anterior chamber lens groups (93 vs. 69 and 81%). Our data show that uneventful extracapsular cataract extraction plus posterior chamber lens implantation offers the best chance of successful detachment surgery.

[Intranasal midazolam in pediatric ophthalmology].

Intranasal midazolam was tested in pediatric patients undergoing ophthalmological examination. 15 children aged 3.5 months to 10 years received 0.35-0.5 mg.kg-1 intranasal midazolam in association with a corneal anesthesia. The onset of sedation was rapid and permitted adequate ocular examination in all cases. In 8 of 15 cases where surgery followed immediately, the induction of inhalational anesthesia was easier. No local or general adverse reaction was noted. This non-invasive method of anesthesia may be compared to the intrarectal administration of midazolam, but the rapid onset of sedation and a cleaner route seem more suitable in surgical environment.

[Chronic vitritis in pseudophakic patients].

We present 15 cases of chronic endocular posterior inflammation, so called vitritis, following cataract surgery. All these cases occurred after uneventful extracapsular surgery plus posterior chamber intraocular lens implantation. Aqueous and a vitreous taps did not reveal any germ but they were done long after the onset of inflammation. Vitrectomy for optical purpose may be helpful for these patients. The two main complications have been retinal detachment and cystoid maculopathy. Retinal detachment can be cured but this requires quite often sophisticated surgical procedures. We found no effective treatment for cystoid maculopathy.
