[Temporary filencleisis as an operation of glaucoma (author's transl)].

This report concerns temporary filencleisis by means of an absorbable thread as a conjunctival covered procedure in 46 eyes, and as a scleral covered procedure in 104 eyes. The scleral covered method has the advantage of more reliable pressure regulation. In addition, during the course of the cure fewer complications (anterior chamber problems) were observed. The period of observation averaged two to three years. Regulation of pressure was achieved by the scleral covered procedure in 89% of the cases, although in 50% of the cases only in combination with subsequent local medication. Vision, postoperative corneal astigmatism and the effect of the number of threads employed in suturing the scleral flap on the healing process and on postoperative suppression of tension are discussed.

Sodium and chloride transport in the isolated human cornea.

22Na and 36Cl fluxes in isolated human cornea preparations were measured under short-circuited conditions. At pH 7.3-PH 7.6 sodium net flux was directed from aqueous humour to tear side, chloride net flux was statistically not different from zero. Alkalinization of the bathing solution to pH 8.6 stimulated unidirectional sodium and chloride fluxes from aqueous humour to tear side resulting in net sodium and chloride fluxes towards the tear side which both were statistically different from zero. Cyclic AMP (10(-3) M) was found to stimulate sodium and chloride unidirectional fluxes from aqueous humour to tear side, thus leading to significant net sodium and chloride fluxes in the same direction. Epinephrine (10(-4) M) increased the unidirectional chloride flux from aqueous humour to tear side more pronounced than in the opposite direction, producing a significant net chloride flux towards the tear side. The results are consistent with the hypothesis that the electrolyte pumps may under certain conditions contribute to the dehydration of the stroma.

[Neuroophthalmological complications through liquor leakage after surgical operation on the spinal canal for diagnostic i.e. therapeutic purposes (author's transl)].

The rare liquor leakage syndrome with cranial nerve lesion, which preferably affects the eye-muscle nerves, can be caused either by further leakage of liquor cerebrospinalis through the dura hole made by the lumbar puncture - or after a surgical operation on the spinal canal leaving a dura closure, which is not tight. This does not occur after a suboccipital puncture. A report is given on three cases with different eye-muscle paresis after a simple diagnostic lumbar puncture, after a lumbosacral myelography and after a resection of the sacral nerve roots including the filum terminale and caudal dural sac. The prognosis of the eye-muscle paresis is good. Restitutio ad integrum resulted. A fourth case with temporary loss of sight of one eye is reported after a lumbar air encephalography, but the eye nerves were damaged before as a result of previous tuberculous basal meningitis. Only partial recovery of the visual power occurred.

[Experiences with the non-contact-tonometer (author's transl)].

The Non-Contact-Tonometer as developed by the American Optical Corporation, is in use superior to the traditional methods of intraocular-pressure measurements according to Schiötz and Goldmann: Without any manipulation on the eye the intra-ocular pressure can be measured rapidly and easily. Owing to the construction of the tonometer however measurements are only possible with smooth corneae. In addition to this the results of measurement with the NCT are not as exact as those of the traditional methods. Finally there is up to the present no possibility of standardising the new tonometer.

[Double-sided occlusion of the great vessels of the neck after X-ray treatment in childhool (author's transl)].

This paper reports on two cases of bilateral blockage of the carotid vessels many years after X-ray treatment of the neck lymph nodes in childhood. The X-ray overdoses resulted in the damage and shrinkage of the skin and upper connective tissue of the neck. The time of delay from X-ray treatment to the first signs of vessel blockage was 45 years and 51 years. The first patient, male aged 62 years, has dilated vertebral arteries. Nine years after bilateral antiglaucomatous operation (with cataract removal) the intraocular vessel processes have slowed and there is good compensation of the carotid vessels as shown by CNS function. The second patient, female aged 62 years, exhibited moreover a block of the right vertebral artery and stenosis of the left vertebral artery. This resulted in acute blindness of the left eye and then of the right eye 1 1/2 years later. Puncture of the anterior chambers and Iridencleisis (right eye) were performed, but without success. Together, with loss of acuity, she had constantly increasing neurological deficiencies and psychopathological changes consistent with thrombosis of the left vertebral artery. Pathiological and anatomical stenosis in the region of X-ray treatment should have warranted surgical intervention to restore normal circulation.
