DSM-III diagnoses compared with factor structure of the psychopathology instrument for mentally retarded adults (PIMRA), in an institutionalized, mostly severely retarded population.

The Psychopathology Instrument for Mentally Retarded Adults (PIMRA) was used to diagnose 163 mentally retarded adults in an institution according to DSM-III axis 1 categories. The relevant 49 items of the PIMRA were subsequently factor analysed. The factors were compared with the diagnoses to examine similarities or differences. The nine factors extracted contained 49.3% of the variance in the data, and were able to categorize correctly 69.3% of the cases with regards to diagnosis through a discriminant analysis. This is statistically different from chance expectations (p less than 0.001) and gives an inter-method agreement of 87.86% and a kappa of 0.62. Some of the factors were recognized as important aspects of traditional psychiatric categories. This makes these diagnostic categories more credible as valid diagnoses for persons from a population like the one studied.

Seasonal mood variation: an epidemiological study in northern Norway.

In an epidemiological study carried out in the town of Tromsö in northern Norway an extensive questionnaire was sent twice, in summer and winter, to 1000 subjects to investigate the degree of symptoms of mood variation (depression or hypomania) during the polar winter night and midnight sun seasons. The results suggest that the general population north of the Arctic Circle shows a major seasonal mood variation. It is suggested that seasonal mood variation is a chronobiological mood disturbance related to and probably precipitated by extreme variations in light. The therapeutic implications and preventive measures are discussed.

[Who is left in the institutions? Some problems in connection with the process of deinstitutionalization].

The medical and psychiatric diagnoses of 168 clients aged 16 to 65 years in a Norwegian institution for the mentally retarded were registered together with medication, level of functioning, and types of problem behavior. Most of these clients (64.9%) were profoundly or deeply retarded, and only 6.5% were independent of continuous supervision or help. Medical diagnoses were found in 87.5%, psychiatric diagnoses in 89.1% (DSM-III, axis I). Daily use of medication was found in 81.0% of the clients, and 48.9% used psychotropic drugs on a daily schedule. 58.3% of the clients had exhibited violent behavior during the last year, defined as assaults on persons, self-mutilation or destructiveness. The article discusses the implications of these observations for primary health care, which is expected to supply the necessary service to these clients after 1991.

Influence of the cation on the side-effects of urographic contrast media.

The incidence of side-effects produced by meglumine diatrizoate and meglumine-, meglumine-calcium-, and sodium-calcium-magnesium metrizoate, was compared in 800 urographies. Patients older than 60 years seem to have less side-effects than younger patients. The incidence of sensation of warmth seems to be higher in patients who have had previous urographies, compared to those who are examined for the first time. The incidence of this effect is higher for the sodium-calcium-magnesium salt of metrizoate, than for other salts of metrizoate.
