Wolf-Hirschhorn syndrome--two case-study reports focusing particularly on long-term survival.

BACKGROUND: The clinical features of this rare chromosomal syndrome are described in two individuals. Our limited knowledge of the natural history of this disorder has made it difficult to counsel parents as well as outline specific treatment and management plans. METHOD: Interviews were undertaken with subjects, carers and next of kin, and the literature was searched. RESULTS: Pertinent features of the syndrome are reported. MAIN CONCLUSIONS: These two cases highlight that long-term survival may occur and that accurate diagnosis is paramount so that healthcare professionals are able to provide accurate information.

Tardive dyskinesia: probing abnormal metabolism with promethazine.

Ten in-patients with tardive dyskinesia (TD) (mean AIMS score 16.7) and 8 controls were recruited to the study, and 3 h after oral administration of promethazine a blood sample was taken for assay of levels of promethazine and its immediate metabolites by high-performance liquid chromatography (HPLC). The TD group had a variety of indicators of impaired or slow metabolism compared to the controls. There was a significant difference in the ratio of promethazine to promethazine sulphoxide (P < 0.05) between patients with TD and the control group. The TD group but not the controls showed increasing metabolic impairment with age. This small study confirms the previous reports of impaired neuroleptic metabolism in TD, particularly in the elderly.

Neurinoma of the facial nerve.

This paper deals with 2 cases of peripheral facial paralysis, due to intratemporal facial neuroma of tympanic (case No. 2) and vertical (case No. 1) segments of the Fallopian canal. The authors present a literature review of the intratemporal facial neuromas with special reference to clinical-surgical and differential diagnostic aspects. It is difficult to detect a Schwannoma of the facial nerve during a peripheral facial paralysis; radiological studies of the mastoid and Fallopian canal are essential for this diagnosis. Treatment of neurinomas of the VIIth nerve is always surgical, the authors emphasize the necessity of an 'en masse' tumor-nerve removal and the use of a nerve graft.
