The treatment of port wine stains with a dye laser: a study of 644 patients.

Between June 1989 and December 1996, the flashlamp pumped pulsed dye laser was used to treat port wine stains (PWS) in 644 patients, age range 3 months-93 years (mean 21). The efficacy of the treatment was assessed after more than one year of follow-up. Each factor that might affect the efficacy was then evaluated statistically. Broad lesions required more laser treatments than narrow lesions and clearing tended to start from the periphery of the lesion, indicating the three-dimensional depth of the broader PWS. Patients who had been given previous treatments such as argon laser required about two more laser treatments than those who had not, but there were no clear differences in the efficacy of dye laser treatment between the two groups.

Perforator-based flap for coverage of lumbosacral defects.

Lumbosacral defects on 20 patients were covered with a perforator-based flap. Cutaneous perforators derived from the 9th and 10th intercostal arteries, the 4th lumbar artery, and multiple gluteal perforators that penetrate the gluteus maximus muscle were used as vascular pedicles. Minor complications occurred in five cases. Using this method, minimal morbidity of the donor site is expected because the gluteus maximus need not be sacrificed. Accordingly, perforator-based flaps are especially indicated for ambulatory patients, but for paraplegic patients as well. Even in the event of recurrence, another perforator-based or musculocutaneous flap can be elevated from the ipsilateral side because of the presence of multiple perforators in the lumbosacral and gluteal regions.

Reconstruction of posttraumatic defects of the foot by flow-through anterolateral or anteromedial thigh flaps with preservation of posterior tibial vessels.

Massive posttraumatic defects of the foot in 4 patients and a tibial malunion in another were repaired by flow-through anterior (anterolateral and anteromedial) thigh flaps. Posterior tibial vessels were used as the recipient vessel just below the level of the medial malleolus. Soft-tissue defects in foot-injured patients were covered with flow-through anterior thigh flaps interposing the descending branch of the lateral circumflex femoral system between the transected posterior tibial vessels. In another patient, a combined anteromedial thigh flap and vascularized iliac bone cross-leg "chimeric" flap were successfully transferred for the reconstruction of a malunion of a tibial fracture, which had been accompanied by severe vascular damage to the thigh. The utilization of derivative branches from the lateral circumflex femoral system facilitates revascularization of ischemic areas and simultaneous transplantation of multiple components, as well as preserves recipient vessels. Owing to the stable blood supply, these flaps can be readily processed into both deepithelialized and thin flaps.

[Ultrastructural research on intimal cell of the ganglion--scanning and transmission electron microscopic study ].

The surface ultrastructure of the ganglion intimal cells was studied by scanning electron microscopy (SEM), in comparison with the transmission electron microscopy (TEM) of the sectioned samples. Thirty ganglia mainly obtained from the hand were used for this study and the whole inner surface of the ganglion was studied and compared in relation to the production of the ganglion fluid. From the surface structure observed by SEM, cellular configuration was divided into 4 groups: 1. Smooth surfaced (Sm) group. 2. Irregular typed (Ir) group. 3. Elongated (El) group. 4. Rough surfaced (Rs) group. SEM views at higher magnifications demonstrated granular protrusion of the vesicles, 0.2-0.3 mu in diameter, on the cellular surface of the Sm and Ir group. However, granular vesicles were seen on the surface of the El group, which probably are very young and similar to fibroblastic cells. In Rs group, craters, 0.04 mu in size, which can be pits after fluid secretion, were detected on the surface. By TEM surveys, ruthenium red (RR) positive substances were found in the vesicles of the intimal cells and also in the intercellular space at the inner to intermediate layers. Such granular vesicles mainly containing of mucopolysaccharide positive by RR staining can play the important role for the production of the ganglion fluid. Intimal cells of the ganglion were different in the cellular surface part to part and such difference can indicate the differentiation of cellular activity especially in the ganglion fluid synthesis. On the other hand, the RR positive substance was found in and around mesenchymal cells at the inner to intermediate layer. This can also confirm that these multi-functional mesenchymal cells have the action to produce the ganglion fluid.

A study of the evoked electromyography of the superior orbicularis oris muscle in man.

A study was made of the impulse conduction speed by moving recording electrodes at 5 mm intervals, successively, after stimulating facial nerves on one side in normal subjects. The findings showed that the speed of the potentials was fast and the amplitude was large, up to a distance of 0.5 cm across the midline. This makes us believe that there is an intermingling, of approximately one cm, of the left and right superior orbicularis oris muscles. Evoked potentials were recorded up to 1.5 to 2.5 cm across the midline--but as the speed reduced abruptly and the amplitude became small, we believe these potentials were conveyed by volume conductors. Six patients with facial palsy were studied, but in none were these findings indicating any reinnervation from the normal side. However, the conduction speed was delayed in them (as compared to normal subjects).
